CONCERNED WITH REGULAR ENTRIES INTO THE FOWARD PEAK SPACE WHICH WAS MARKED AS RESTRICTED, REVIEWED SECTIONS 4.5 AND 4.11 OF THE BOUCHARD SAFETY MANAGEMENT MANUAL (24FEB2017 EDITION) WHICH OUTLINES THE COMPANYS SAFE WORK PRACTICES REQUIRED FOR PPE AND CONFINED/RESTRICTED SPACE ENTRY WHICH INCLUDES A JSA, ATMOSPHERIC TESTING, COMMUNICATIONS AND LOGBOOK ENTRIES. ALSO THE COMPANY POLICY IS AT A MINIMUM PPE SHOULD BE LONG SLEEVES AND LONG PANTS. THERE WAS NO OBJECTIVE EVIDENCE THAT THESE PROTOCOLS ARE BEING FOLLOWED. THERE WAS NO ENTRIES IN LOGBOOK AND SEVERAL POB IN SHORT SLEEVE SHIRTS. PRIOR TO ENTRY, COMPANY IS TO PROVIDE DOCUMENTATION AND IMPLEMENTATION OF SAFE WORK PRACTICES.
