Manned tank barges and tank ships authorized to carry Grade A, B, C, or D liquids at any temperature, or Grade E liquids at elevated temperatures, shall be provided with a combustible gas indicator suitable for determining the presence of explosive concentrations of the cargo carried. Vessels installed fixed gas detection system was found not operating properly. Repair gas detection system to the satisfaction of the a Coast Guard marine inspector.

All tank vessels shall have efficient guard rails or bulwarks on decks and bridges. All rails shall consist of solid or tubular sections or chains or wire ropes. Numerous turnbuckles holding the safety chains in place were found wasted. Properly secure all openings in the deck rail on the periphery of the vessel, to the satisfaction of a Coast Guard marine inspector.

On all tank ships where the plans and arrangements will possibly permit, all passageways leading to living quarters, or places where anyone may be regularly employed, shall be provided with not less than two avenues of escape so located that if one of such avenues is not available another may be. The emergency escape hatch, located on top of the thermal fluid heater compartment, was unable to be opened safely from the interior due to a broken assist spring. In addition, the access ladder on the outside of the compartment, which acts as part of the escape route, had two wasted rungs on the bottom, creating an unsafe condition for crew. Repair escape hatch and ladder rungs, to the satisfaction of Coast Guard marine inspector.

Electrical installations on vessels must ensure maintenance of system integrity to which plan review has been approved. Inert gas generator high temperature alarm cable overhead penetration has temporary repair. Properly repair penetration.

Electric installations on vessels must ensure maintenance of services necessary for safety under normal and emergency conditions. Protection of passengers, crew, other persons, and the vessel from electrical hazards. MI observed that the electrical shore tie supplying power to the barge from the tug was no longer serviceable. Vessel shall enact permanent repairs to the satisfaction of attending USCG Marine Inspector. 46 CFR 111.01-1

Each cargo vessel must carry the number of lifebuoys prescribed in table 199.271 of this section. The vessel only had 5 lifebuoys on board. Procure the required 12 lifebuoys and install on board to the satisfaction of the coast guard.

In order to maintain system integrity, each individual electrical installation in a hazardous location must comply specifically with Articles 500-505 of NFPA NEC 2002 (incorporated by reference; see 46 CFR 110.10-1), as modified by 46 CFR 111.105-3, or with the IEC 60079 series (as defined in 46 CFR 111.105-1 and incorporated by reference; see 46 CFR 110.10-1), but not in combination in a manner that will compromise system integrity or safety. Hazardous location equipment must be approved as suitable for use in the specific hazardous atmosphere in which it is installed. The use of nonapproved equipment is prohibited.Covers for angle drives are degraded and in need of replacement (#1-4) Vessel is authorized a one way transit to Beaumont with the following restricitons; Must maintain inerted tanks at 3.5% O2, Lock out tag out all cargo pump engines, temp repair covers. No Cargo ops authorized until repairs made in Beaumont to satisfaction of attending Coast Guard inspector/local OCMI. 46 CFR 111.105-5

Except as provided in paragraphs (b) through (g) of this section, each tank vessel must undergo drydock, internal structural, and cargo tank internal examinations as follows: (1) except under paragraph (a)(2) of this section, vessels that operate in salt water must be inspected in accordance with the intervals set forth table 31.10-21(a). This tank barge is subject to a 5.0yr interval for Dry dock, which expires February 12, 2021. Conduct a Dry dock Examination to satisfaction of a CG Marine Inspector prior to conducting any cargo operations. Any requests to conduct a voyage associated with pursuing a Dry dock contract shall be communicated with the local OCMI.

Fired thermal fluid heaters must have a low fluid level cutout device or a low flow device. Due to lack of thermal fluid, proper test was not able to be conducted. Prior to use of thermal fluid heater, USCG must witness satisfactory operational test and shutdown.

Manned seagoing barges must be fitted with anchors, chains and hawsers in general agreement with the standards established by the American Bureau of Shipping. Vessel port anchor is missing and windlass is inoperable. Until port anchor satisfactorily replaced, vessel restricted from anchoring, and transits must be performed in accordance with NOLA MSIB "Carrollton Gauge at 15 Feet and Falling" dated 07 Feb 2020.

Manned tank barges authorized to carry Grade A, B, C, or D liquids at any temperature, or Grade E liquids at elevated temperatures, shall be provided with a combustible gas indicator suitable for determining the presence of explosive concentrations of the cargo carried. The No. 4 Port ballast tank sensor was inoperable. Rectify deficiency prior to loading cargo.

No person may operate any vessel equipped with installed toilet facilities unless it is equipped with: an operable Type I, II, or III MSD certified and labeled under 159.12, 159. 12(a), or 159.16. Vessel had an insufficient temporary repair of a silicon patch on the MSD holding tank. Within 30 days, perform permanent repairs.

Vessel transfer pipe systems must be satisfactorily tested annually: transfer hoses must be satisfactorily liquid pressure tested at 1.5x MAWP, pressure gauges must show pressure within 10% of the actual pressure, and relief valves must open at or below the test pressure at which it is set to open. Prior to loading or discharging cargo, complete annual cargo pipeline testing to satisfaction of USCG.

Any tank vessel equipped with internal combustion engine cargo pumps on weather deck shall be provided with a minimum of one remote manual shutdown. Proper test of the #3 cargo pump remote shutdown was not able to be conducted due to inoperable pump. Prior to use of #3 cargo pump, USCG must witness satisfactory pump operation and remote shutdown. 46 CFR 32.50-35

Each generator must meet the applicable requirements for construction and testing in section 4-8-3 of the ABS Steel Vessel Rules. The #3 SSDG was found to have a coolant water leak impacting its "continuous rating" as defined by ABS, and the #1 SSDG was inoperable. Within 30 days, correct to satisfaction of USCG Marine Inspector.

Places where anyone may be regularly employed shall be provided with not less than two avenues of escape so located that if one such avenue is not available another may be. Emergency escape hatches on deck houses unable to be opened from spaces they protect. Correct to satisfaction of USCG within 30 days. 46 CFR 32.40-60(b) and 46 CFR 32.02-1

Ballast piping systems must be so arranged as to prevent oil or water from one compartment to another. The #5 Stbd ballast tank has a piping leak, inhibiting ballast and stability of vessel. Within 30 days, correct to satisfaction of marine inspector. 46 CFR 56.50-50(h)

VCS liquid filled pressure-vacuum breaker installations must meet Commandant approval. Vessel P/V breaker was found to have low liquid level, nearly out of sight glass. Corrected on-site. 46 CFR 39.2011(c)

Install and operate a ballast water management system (BWMS) that has been approved by the Coast Guard under 46 CFR part 162. The BWMS must be installed in accordance with §151.2035(b) of this subpart. Following installation, the master, owner, operator, agent, or person in charge of the vessel subject to this subpart must properly maintain the BWMS in accordance with all manufacturer specifications. Unless otherwise expressly provided for in this subpart, the master, owner, operator, agent, or person in charge of vessels employing a Coast Guard-approved BWMS must meet the applicable ballast water discharge standard (BWDS), found in §151.2030 of this subpart, at all times of discharge into the waters of the United States. Current Ballast Water Management system has damage and internal leaking within the piping systems dedicated to the #5 Ballast Tanks. A previous NS5 work order detailed that the vessel was listing while carrying cargo, presenting a stability concern. Complete proper repairs and testing to satisfaction of CG Marine Inspector prior to conducting cargo operations. 33 CFR 151.2025(a)(1)

Gas detection systems must have a readout with meters that show flammable gas concentration over the concentration or volume ranges under paragraph (t) or (u) of this section. Gas detection system has fault, #4 sensor inoperable. Replace associated sensors/equipment and test to satisfaction of CG Marine Inspector prior to conducting cargo operations regulated under 46 CFR, Subchapter O. 46 CFR 154.1350(d)

Unless the cargo tanks are gas free, the master of each tankship to which this subpart applies shall ensure that the inert gas system is operated as necessary to maintain an inert atmosphere in the cargo tanks. Inert Gas System "double block and bleed" approved by MSC project C1-1600476 is inoperable, and fuel/air lines are beyond service. Conduct proper repairs and testing to satisfaction of CG Marine Inspector prior to conducting cargo operations that require the use of the IGS. 46 CFR 32.53-5

Operational tests and checks of all safety and limit controls, combustion controls, programming controls, and safety relief valves must be conducted by the owner, chief engineer, or person in charge at the inspection for certification, periodic inspection, and when directed by the Officer in Charge, Marine Inspection, to determine that the control components and safety devices are functioning properly and are in satisfactory operating condition. These tests and checks must be conducted in the presence of a marine inspector and must include the following: proper prepurge, burner ignition sequence checks, operation of the combustion controls, limit controls, fluid flow controls, fluid level controls, high temperature control, proper postpurge control, and verification of the flame safeguard. Thermal fluid heater coils are damaged and some have been removed, rendering the entire system inoperable. Conduct proper repairs and testing to satisfaction of CG Marine Inspector prior to conducting cargo operations of elevated temperature cargoes. 46 CFR 61.30-20

The high-level alarm required by paragraph (a) of this section must— (3) Activate a visible and audible alarm so that it can be seen and heard on the vessel where cargo transfer is controlled and in the cargo deck area. Audible alarm for High Level / Overfill system did not meet requirements. Repair or replace and test to satisfaction of CG Marine Inspector prior to conducting cargo operations. 46 CFR 39.2007(d) 46 CFR 39.2009

At each inspection for certification and periodic inspection the marine inspector shall conduct such tests and inspections of the main propulsion and auxiliary machinery and of its associated equipment, including the fluid control systems, as he feels necessary to check safe operation. Safety shutdowns for installed generators have not been tested. Test and provide documentation to satisfaction of CG Marine Inspector within 30 days. 46 CFR 61.20-3(a)

All U.S. flag vessels which engage in foreign voyages or international voyages by sea (other than solely in Great Lakes voyages) are subject to this part. Current Load Line certificate has expired. Renew certificate prior to conducting any voyage outside of the U.S. Boundary line. 46 CFR 42.03-5(a)

The instructions supplied by the manufacturer must contain directions for each of the following: Safe operation and servicing of the device so that any discharge meets the applicable requirements of §159.53. Vessel could not provide any certificates, documentation or logs pertaining to the use and maintenance of installed MSD. Provide to satisfaction of CG Marine Inspector prior to discharging any effluent.

Operational readiness. Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Liferaft hydrostatic releases are expired or have a missing expiration date. Replace to satisfaction of CG Marine Inspector within 30 days.

A tank vessel owner or operator must submit plans, calculations, and specifications for a new tank vessel VCS to the MSC for review and approval before installing the system. A permanent or portable vapor processing unit onboard a tank vessel will be reviewed, together with the tank vessel, as a complete and integrated system. Current VCS approval has been completed by the classification society ABS, who is no longer associated with Owner/Operator. Provide documentation of MSC approval or new classification society approval to satisfaction of CG Marine Inspector prior to conducting cargo operations.

Before issuing an original certificate of inspection, the following number of copies of the stability booklet required by §170.110 must be submitted for approval; three copies for plan review being conducted by the Coast Guard Marine Safety Center. Current stability booklet has been completed by the classification society ABS, who is no longer associated with Owner/Operator. Provide documentation of MSC approval or new class approval to satisfaction of CG Marine Inspector prior to conducting cargo operations. 46 CFR 170.080

Vsl must complete an in-service and/or annual inspection prior to departure to ensure compliance with applicable provisions of 46 CFR subchapter D.

Line-throwing appliances. Each vessel must have a line throwing appliance approved under series 160.040. The vessels line-throwing appliance's are expired as of March 2019. Prior to departure local MI will validate that all line-throwing appliances are valid and not expired.

Each vessel must carry distress flares. Vessels distress flares are expired. Prior to departure local MI will validate that all distress flares are valid and not expired.

Valves, Fittings, and Accessory Equipment. Provisions shall be made by the use of offsets, loops, bands, expansion joints, etc., to protect the piping and tank from excessive stress due to thermal movement and/or movements of the tank and hull structure. Expansion joints shall be held to a minimum and where used shall be of the bellows type and subject to special approval by the Commandant. #4 cargo pipe line port aft has 2 expansion joints just forward of pump discharge that have over expanded and bulge out making the designed purpose of the expansion joint invalid. Vessel crew has already notified company and placed the section of piping in the tag out system and also tagged out the pump. Vessel has placed on order both expansion Joints. However, since each joint is a special order/fabrication it is still 2 weeks from the date of delivery. Issued a 30 day period to vessel to fully install and test new expansion joints and to be reinspected by USCG MI before placing the section of pipe back into service.

Objective evidence discovered in an expanded SMS exam revealed a failure to implement requirements of the SMS as evidence by deficiencies #1-3. An internal SMS audit and corrective action report is required to be completed to the satisfaction of the cognizant OCMI within 3 months of this date, 22 April 2019.

Pressure/Vacuum breaker liquid level low. Vessel was emitting vapors during transfer which required VCS. Must correct liquid filled pressure vacuum breaker to operate according to manufactures specifications prior to resuming cargo operations.

#1 emergency pull cable for fixed foam fire fighting installation is missing. Repair to satisfaction of attending marine inspector prior to conducting discharge operations.

Fixed CO2 extinguishing cylinder for the gear oil locker found broken from its mount and laying adrift on the deck. Once temporarily secured, repair permanent installation to the satisfaction of the Marine Inspector. 46 CFR 31.10-18

Portable dry-chemical fire extinguishers FE No. 10 and FE No. 2 were found undercharged. Safety and firefighting equipment checklist reflects that portable/semi-portable etinguishers were last inspected as satisfactory on 12/1/17. Properly charged spares replaced. Company to provide documentation of [name withheld] attesting to the implementation of Section 9.2 of the company Safety Management Manual. 46 CFR 31.10-18 33 CFR 96.210(c)

Vessel's Ballast Water Alternate Management System reported as inoperative since May 2017. As a "new vessel", provide notification to nearest COTP and proceeed as directed prior to discharging untreated ballast water in the waters of the US. Alternately, provide valid USCG extension letter. Prior to discharge of untreated BW. 33 CFR 151.205(g)

Vessel's Ballst Water Treatment Plan does not include the provisions outlined in 33 CFR 151.2050(g), including procedures for cooridnating the shipboard BWM strategy with the Coast Guard as well as detailed reporting requirements and procedures for ports in the US. Update plan as required to the satisfaction of the Coast Guard. 33 CFR 151.2050(g)

Start air guages for ballast water pump gens. #1 and #3 inoperative. Make repairs and prove proper opeation to the satisfaction of the attending Marine Inspector. 46 CFR 61.01-1

The fire doors and frames forming part of the continuous class A structural fire boundary in the cargo pump room (port and starboard)are gapped. Additionally, marine cable transit for the adjacent accomodation space not properly installed. Make repairs and prove proper operation to the satisfaction of the attending Marine Inspector. 46 CFR 32.57-10

Ballast water pumps #1 and #3 noted with significant leakages past the packing glands. Make repairs and prove proper operation to the satisfaction of the attending Marine Inspector. 46 CFR 61.01-1

Section 4.5 part B of the Bouchard Safety Management Manual (SMM)(2017 ed.) prescribes the safe working practices for entry into restricted access spaces. The forepeak has been identified as a restricted access spaace and is regularly accessed by the crew. There is no objective evidence to indicate that these safe work practices have been implemented prior to entry. Prior to entry, company to provide documentation of corrective action attesting to the implementation of Section 4.5 of the company SMM. 46 CFR 35.20-20 33 CFR 96.210(c)

High level/Over-fill Alarms are not functioning properly. After offload of current cargo, vessel is not authorized to conduct cargo operations until repairs are made to the alarms.

Three metallic expansion joints (2 on cargo pipe and 1 on the vapor line were found to cracked on the outer bellows.

Ensure approved testing procedures for vital equipment is on onboard for conducting periodic safety tests.

Repair pull station cable conduit for foam extinguishing system.

Replace life raft launching procedures placard at port aft raft launching station.
