A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel. Forward port thruster experienced a hydraulic leak with estimated 250 gallon discharge.

Each pressure vessel, other than one exempted by this section, must be subjected to a hydrostatic test at a pressure of 1 1/4 times the maximum allowable working pressure twice within any five-year period, except that no more than three years may elapse between any test and its immediate predecessor. The following pressure vessels will not normally be subjected to a hydrostatic test: Pressure vessels which have been satisfactorily examined internally by a marine inspector and in which no defects have been found which impair the safety of the pressure vessel. Pressure vessel examination due on 11APR2026 of 05 pressure vessels, an extension of the examination window has been granted until 30APR2026.

All marine engineering detais such as piping, valves, fittings, boilers, pressure vessels, etc., and their appurtenances installed on the vessel, shall be designed, constructed, and isntalled in accordance with the provisions of subchapter F (Marine Engineering) of this chapter. Vessel reported a 15'x7' crack on main deck. Vessel must make satisfactory repairs IAW approved repair plan and conduct satisfactory non destructive testing prior to carrying cargo. 46 CFR 90.20-1

EPIRB registration onboard was expired.

If, during an internal structural, cargo tank internal examination, or underwater survey, damage or deterioration to the hull plating, structural members, or cargo tanks is discovered, the Officer in Charge, Marine Inspection, may require the vessel to be drydocked or otherwise taken out of service to further assess the extent of the damage and to effect permanent repairs. MI found excessive distortion in the forward most transverse deck beam in Ballast Water Tank 5-6C. Repairs to be completing during Jan 2026 dry dock.

Exhaust pipe installations must conform to the requirements of ABYC P-1 and Chapter 6 of NFPA 302 (both incorporated by reference; see § 58.03-1) and the following additional requirements: Protective guards, jacketing, or covers shall be provided wherever persons or gear might come in contact with the exhaust system where the temperature exceeds 200°F (93°C). Dry exhaust does not have covers on exhaust piping.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Marine Vessel Rules. Hydraulic leak on pressure block attached to propulsion engine in #2 Van.

Valves installed on the outside of the oil tanks must be arranged with a means of manual control locally at the valve and remotely from a readily accessible and safe location. The valves do not fully close when operated remotely.

Every system, which may be exposed to pressures higher than the system's maximum allowable working pressure, must be safeguarded by appropriate relief devices. Relief valve attached to the piping system on the aft ballast system is leaking while pressurized.

The inspection shall be such as to ensure that the workmanship of all parts of the vessel and its equipment is in all respects satisfactory. Observed fuel leak on #3 van priming pump. 46 CFR 91.20-15

No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. Observed temporary repair on #4 van aftercooler outlet pipe and pipe must be replaced to the satisfaction of the Coast Guard. 46 CFR 91.45-1

Each outlet or junction box must have a cover. ID’d junction boxes without covers located near #1 and #4 HPUs. 46 CFR 111.81-1

Shutoff valves of a suitable type shall be installed in the fuels supply lines, one as close to the tank as practicable. Observed fuel shutoff valve leaking diesel fuel at #4 service tank. 46 CFR 56.50-75 (b)(4)

Each cargo vessel must carry the number of lifebuoys prescribed in table 199.271 of this section. 2 lifebuoys on aft portion found with deteriorated grab lines, replace or repair. 46 CFR 199.271

Each vessel must carry not less than 12 rocket parachute flares approved under approval series 160.136. Provide flares. 46 CFR 199.60

All vessel owner operators must submit all NOIs, NOTs, annual reports, Discharge Monitoring Reports (DMRs), and other reporting information as appropriate electronically. Submit Annual report. 40 CFR 122 CG-543 Policy Lty 11-01 VGP 2013 1.14

It is mandatory that the design, fabircation and erection of piping constructed under the regulations of this Sub-chapter demonstrate leak tightness. Vessel must submit a repair plan for approval. MI will attend for final repair test. 46 CFR 56.97-1(a)

If, during an internal structural examination, damage or deterioration to the hull plating is discovered, the OCMI may require the vessel to be drydocked or otherwise taken out of service to further assess the extent of the damage and to effect permanent repairs. During the inspection and a survey conducted by the operator and ABS, hull damage was noted in the Forward Rake Tank (starboard turn of bilge, 9" x 9" indent; transverse deep frame, buckle in plate at starboard lightening hole; port side longitudinal bulkhead buckle at lightening hole; and below headlog bottom plate 7-9 frames inboard of longitudinal bulkhead port side) and Aft Rake Tank (15" x 6" indent in aft log 5 & 6 frames inboard from starboard side; indentations inboard of starboard longitudinal bulkhead and 1st, 2nd, and 3rd frames; indentations of aft headlog, located across 3 frames midway between centerline bulkhead and starboard bulkhead; and aft log indentation between 6 & 7 frames port side of centerline bulkhead) as referenced in the operator's report. The vessel is to complete permanent repairs during its next scheduled drydock, no later than 30JUN2026.

O/O to provide alternate means of providing means to satisfactorily examining pressure vessels.

Valves installed on the outside of the oil tanks must be arraned with a means of manual control locally at the valve and remotelyfrom a readily accessible and safe location. The vales on the fuel tank do not have a means of closure from a safe location.

Submit plans for approval and demonstrate compliance with the vital system automation requirements in 46 CFR 62.

In general, structural standards shall be in compliance with ABS Rules. Make proper repairs to the fractures noted in the fwd and aft rakes during ISE.

Electric installations on vessels must ensure maintenance of services necessary under normal and emergency conditions. The cabling for the deluge pumps in 6C require chaffing gear and are not properly supported.

The owner or operator of a vessel equipped with ballast tanks that operates in the United States and uses only water from a U.S. public water system, must maintain a record where the water came from and previously cleaned the ballast tanks. Sediment is present in voids used as ballast tanks.

Nothing in this subpart shall be construed as limiting the inspector from making such tests or inspections as he deems necessary to be assured of the safety and seaworthiness of the vessel. Vessel is to provide certificates for towing terminal gear.

Electric installations on vessels must ensure the services under normal and emergency conditions are maintained. Fuel leak identified on the elbow of the return fuel line on the #2 SSG.

Vent outlets from all tanks which may emit flammable or combustible vapors, must be fitted with a single screen of corrosion resistant wire of at least 30 by 30 mesh, or two screens of at least 20 by 20 mesh spaced not less than one-half inch nor more than 1/ ½ inches apart. The vent outlet on the fuel tank (forward part of the vessel) did not have mesh on it. In addition, there was a missing screen/vent termination on the internally installed tank vent pipe on the #2 SSG.

Wire must be in an enclosure. There were multiple dead ended wires in the vicinity of the forward HP1/Fuel tanks and under the garage near the center stairs.

Each cable entering a box or fitting must be protected from abrasion and must meet the following: each opening through which a conducted enters must be closed. The 120/208 volt junction box by the forward winch drum has multiple cables that are not protected or closed within the box.

Each vessel must carry lifebuoys approved under the approval series 160.150. The lifebuoy on the forward part of the vessel was holed.

Electric installations on vessels must ensure the services under normal and emergency conditions are maintained. Due to significant issues identified with SSG #1, provide report from the manufacturer that SSG #2 is suitable for service. Report to address and include; status of any pending or overdue service, condition of fuel lines, proper installation of lagging and insulation, proper installation of all wiring and associated components.

Hydraulic fluid systems and piping shall be suitable for operation. Install appropriate chaffing to all areas of hydraulic hoses and fuel piping that rub or make contact with fixed structure.

Independent diesel fuel tanks shall meet appropriate design and construction standards. All worn or wasted fasteners, vents and surfaces are to be repaired or renewed to original condition.

Equipment must be installed to prevent damage from dripping liquids. Hole in the aft control station overhead drips water on installed computer control systems. Holes in the T2 HPU allow water access on to installed machinery and hydraulic systems.

General arrangement plans showing for each deck the fire control stations, the various sections enclosed by fire-resisting bulkheads, together with particulars of the fire alarms, detecting systems, the sprinkler installation (if any), the fire extinguishing appliances, means of access to different compartments, decks, etc., and the ventilating systems including particulars of the master fan controls, the positions of dampers, the location of the remote means of stopping fans, and identification numbers of the ventilating fans serving each section. Vessel did not have a fire control plan.

Manholes and flush scuttles shall be closed by substantial covers capable of being made watertight. Manhole covers Port side 1, P2, P 3, P 4, P5, P6, P7, Starboard side 1, S2, S3, S4, S5, S6, and S7 were observed to have deteriorated rubber dogging bolt gaskets, rendering hatches potentially non-watertight.

Each lifejacket must have a lifejacket light approved under approval series 161.112 securely attached to the front shoulder area of the lifejacket and each lifejacket must have a whistle firmly secured by a cord to the lifejacket. Lights attached to lifejackets stowed in the forward stowage location were not an approved type and several were missing whistles.

Each outlet and junction box must be suitable for the environment in which it is installed and be constructed to the appropriate NEMA or IEC standard. Junction box in way of HPU1 was found to have temporary watertight repair with some sort of putty like material.

Fuel piping shall be effectively secured against excessive movement and vibration by the use of soft nonferrous metal liners or straps without sharp edges. Fuel piping from the forward starboard fuel tank was noted broken loose from it’s securing system on the deck.

Electric installations on vessels must ensure maintenance of services necessary for safety under normal and emergency conditions. Tow bridle lifting winch control switches were observed corroded, with one device completely broken free from drum.

Each outlet or junction box must be securely attached to its mounting and be affixed so as to maintain its designated degree of protection. Junction box observed “zip-tied” to conduit forward, starboard side of vessel, forward of the generator.

Each outlet and junction box must be suitable for the environment in which it is installed and be constructed to the appropriate NEMA or IEC standard. Open junction box/conduit adjacent to aft, center stairs.

At least one-half the total number of lifebuoys on the vessel must each be fitted with a self-igniting light approved under approval series 161.010. Forward, Starboard lifebuoy light found broken.

Each lifebuoy must be marked in block capital letters with the name of the vessel and the name of the port required to be marked on the stern of the vessel under § 67.123 of part 67 of this chapter. None of the lifebuoys were marked with the port.

Each liferaft must have a painter system providing a connection between the vessel and the liferaft. Aft liferaft painter not connected to vessel by means of weak link.

Each vessel must carry a category 1 406 MHz satellite EPIRB that must be stowed in its float-free bracket with the controls set for automatic activation and be mounted in a manner so that it will float free if the vessel sinks. EPIRB Hydrostatic Release Unit for EPIRB found expired.

Fluid power hose and fittings shall meet the requirements of subpart 56.60 of this subchapter. Top hydraulic hose installed in way of aft starboard thruster noted to have cut in cover, exposing interior metallic reinforcement leading to corrosion and weakening.

Each box for a small battery installation must have openings near the top to allow escape of gas. If the installation is in a non-environmentally-controlled location, the installation must prevent the ingress of water. Battery installations on deck outside of forward generators were observed to have water inside.

Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel failed to conduct annual inspection within the 3 months before or after its anniversary date.

Repair damaged steel on Port Side wing extension voids & deformed/cracked frames to the satisfaction of the attending Marine Inspector.

Repair cracked deck to the satisfaction of the attending CG Marine Inspector prior to departure.

Properly label distribution panels "LVF" and "C" in forward control room.

Secure junction box covers in garage, renew / replace missing covers, and properly support wire runs throughtout the vessel.

Provide VGP.

Establish Ballast Water Plan.

Renew worn 2" hydraulic lines for DP 3.

Re-secure exhaust lagging on the #1 and #2 prime mover engines to cover exposed exhaust piping.

Install 30x30 mesh flame screens on all fuel tank vent openings.

Provide bench test report for safety relief valve installed on IPA & ISOPAR pressure vessels and install valve in such away that the label is readable.

Provide approved lights on (2) PFD's.

Stencil vessel name on ring buoys.

Prove proper operation of fwd fire pump.

Plan submittal shall be made to the Marine Safety Center for (2) new 120 gallon air receivers and distribution piping that were installaed on board the vessel. System can not be used until approved plans are provided

Extensive cracking of deck plating was discovered during routine vessel visit. Repair proposal shall be submitted to the OCMI for approval prior to commencing repairs. Repairs shall be completed prior to vessel operating.

Provide a Coast Guard approved non-tank vessel response plan IAW 33 CFR 155.5015 prior to operating.

Develop and implement approved chemical testing program IAW 46 CFR 16 prior to operating.

Provide a category I, float free 406 MHZ EPIRB that is properly registered IAW 46 CFR 199.510 prior to operating.

Implement a training and drill program and conduct and log drills IAW 46 CFR 199.180 prior to operating.

Provide (2) certified lifeboatmen or persons specially trained in the launching of survival craft IAW 46 CFR 199.100 prior to operating.

All seavans (containers) used for mission control, service or temporary offices shall be CG approved and meet the structural fire protection requirements of 46 CFR 92.07-10.

Temporary unapproved fuel tank and flexible lines shall be remvoed from the vessel and replaced with approved materials meeting the requirements of 46 CFR Subchapter F.

All doors on the main deck shall be weathertight IAW 46 CFR 42.15-10.

Install navigation lighting applicable to a vessel of this size, service and maneuvering characteristics IAW the COLREGS NLT the vessels next dry docking.

Install an anchoring system as required by 46 CFR 96.07-5 NLT the vessels next dry docking.

Submit plans for approval and demonstrate compliance with the vital system automation requirements in 46 CFR 62.

Provide SOPEP that meets the requirements of 33 CFR 151.26.

Provide EPA required vessel general permit IAW the National Polluton Discharge Elimination System and Clean Water Act.

All machinery space openings shall comply with the provisions of 46 CFR 42.15-75 prior to operating.

Reinstall rails/lifelines where missing on the port quarter of the vesel IAW 46 CFR 42.15-75 prior to operating.

Stencil vessel name on aft life ring and provide battery for waterlight on fwd life ring IAW 46 CFR 199.721 prior to operating.

Repair louvers for HPU prime mover space #4 and demonstrate proper operation IAW 46 CFR 42.15-35 prior to operating.

Provide evidence of testing for safety relief valves on cylinders for compressed air system IAW 46 CFR 61.10-5 prior to operating.

Equip all type I PFD's with Coast Guard approved lights IAW 46 CFR 199.70(b)(4)(i) priot to operating.

All electrical installations shall comply with the applicable requirements of 46 CFR Subchapter J, to include removal of household fixtures, installations not for marine environment and installation of proper wire supports.

Repair excessive hydraulic oil leak on thruster #3 inboard block prior to operating.

Renew deteriorated hydraulic hose flushing line for thruster #2.

Demonstrate proper operation of low lube oil shutdowns for all HPU prime movers IAW 46 CFR 58.05-10 prior to operating.

Spaces with only one egress / exit shall not extend more than 40 feet. Install secondary means of escape per 46 CFR 92.10

Install and prove proper operation of navigation lights for a vessel greater than 50 meters in length in accordance with the International Rules of the Road (COLREGS).

Identify and properly label all circuit breakers.

Provide and properly label 16 type I personal floatation devices.

Provide non-conductive deck covering suitable for the switchboard voltage.

Provide six properly labeled life ring buoys suitable for Ocean service. Three to be equipped with floating water lights and lines.

Establish training and drill program.

Provide Waste Management Plan.

Provide Garbage Placard

Provide and install anchor.

Provide and maintain an Oil Record Book as required by 46 CFR 1551.25 and MARPOL Annex I Reg 17.

Establish and maintain life-saving equipment and maintenance record.

Post MARPOL Placard.

Prove proper operation of general alarm.

Replace electrical cover in starboard Cat main engine room.

Perform safety function test on both Cummins main engines and both ship service diesel generators.

Apply for and obtain Vessel General Permit from the EPA.
