Resolution: Separators Spares & Equipment had attended and replaced the OCM with one that has valid calibration until 22JAN29. They completed a function test and left the OWS in satisfactory operation.
In general, compliance with the standards established by ASB will be considered as satisfactory evidence of the structural efficiency of the vessel. However, in special cases, a detailed analysis of the entire structure or some integral part may be made by the Coast Guard to determine the structural requirements. As per the Marine Safety Manual Vol. II Sec A.5, the M/V GO [name withheld] suffered a Class 1 structural failure, which has affected a main strength member to the extent that it compromises the vessel's ability to operate safely within its assigned parameters. The vessel's main strength deck has multiple cracks in the aft section in the proximity of the struts for the net supporting arms. As a result of this structural failure, the vessel owner or representative is required to:
1. Submit a Coast Guard form 2692 in accordance with 46 CFR 4.05-10.
2. Provide the OCMI, CG Marine Safety Center, and ABS with a structural failure analysis.
3. Provide the OCMI, CG Marine Safety Center, and ABS with a permanent repair proposal for consideration.
