Nothing in this subpart limits the marine inspector from conducting any tests or inspections he or she deems necessary to be assured of the vessel's seaworthiness or fitness for its route and service. Attending MI shall conduct Steering test, Observe Main Engines operating properly, conduct drills. - 46 CFR 137.200(d) - 17c

All bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable. Excessive oil in bilge. Accumulation of approximately 8 to 10 inches of dirty oil, fuel and water was observed in the bilge in the Engine Room. Remove oily water. - 46 CFR 142.220(a) - 17c

All seals and gaskets must be properly maintained to prevent leaks of flammable or combustible liquid, as those terms are defined in 46 CFR subpart 30.10, into the machinery space. Port Main engine fuel pump was observed to have steady leak of diesel. Stbd Main engine fuel pump was observed to have a weeping leak. Stbd Main engine Racor Fuel filter found with oil in metal catch basin. Repair to the satisfaction of attending Marine inspector. - 46 CFR 143.220(a) - 17c

Survival craft must be stowed in accordance with the requirements of § 199.130 of this chapter, Liferafts must have float free arrangement… hydrostatic release unit. Life-raft Hydrostatic release expired one month ago. Replace. - 46 CFR 141.310 - 17c

Each towing vessel equipped with rudder(s) must have a means to monitor and control the position of the rudder(s) at each operating station. 1. Rudder angle indicator centerline main bridge control station was inoperable. 2. Rudder angle indicator repeaters shows a range of 26* to 35* Re-calibrate rudder angle indicators with proper rudder range. - 46 CFR 143.225 - 17c

Essential systems or equipment must be regularly tested and examined. Tests and examinations must verify that the system or equipment functions as designed. If a component is found unsatisfactory, it must be repaired or replaced. 1. Port aft hydraulic steering ram was found with a broken seal. 2. Steering pump No.1 flex hose was found blistered. Replace. - 46 CFR 143.245(a) - 17c

Marine Sanitation Device must be free of defects that would allow toxic substances to escape to the interior of the vessel. The MSD was observed to have a failed gasket on the inlet side of the device where sewage effluent was leaking out of top of the box. Repair. - 33 CFR 159.95(a)(1) - 17c

A towing vessel that is certificated for Oceans route whose contract for construction prior to August 27, 2003 must have an approved 160-B semi-portable fire-extinguishing system to protect the engine room. Semi-portable fire-extinguisher was mounted to the second deck, but its hose was not long enough to provide firefighting capabilities to the engine-room at its current mounted location. Install hose of adequate length. - 46 CFR 142.315(a)(2) &(i) - 17c

All persons employed or engaged onboard vessels to which STCW applies will be issued a medical certificate valid for 2 years. Medical Certificates for Master and OS found expired. - 46 CFR 10.301 - 17c

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. STBD main engine exhaust leak. Repair. - 46 CFR 143.205(a) - 17c

Electrical components that pose an electrical hazard must be in an enclosure. Multiple dead-end wires found though out the engine room, a wire nut spliced wire was found not in an enclosure. Breaker panel was found with missing cover. Remove electrical hazards. - 46 CFR 143.400(g) - 16c

Piping and machinery components that exceed 220 °C (428 °F), including fittings, flanges, valves, exhaust manifolds, and turbochargers, must be insulated. STBD Generator Exhaust Lagging missing. Replace. - 46 CFR 143.220(b) - 16c

Flammable and combustible products must not be stored in machinery spaces, unless they are stored in a suitable container that meets the requirements of § 142.225 of this subchapter. Multiple catch basins and plastic buckets used to catch oil were observed through out the engine room. Remove. - 46 CFR 143.220(c) - 16c

Each vessel must carry a certificate with a valid endorsement while subject to the Bridge-to-Bridge Act. Bridge to Bridge certificate was found expired. - 47 CFR 80.1005 - 50c

Each towing vessel must be equipped with an industrial type first aid cabinet or kit, appropriate to the size of the crew and operating conditions. Each towing vessel operating on oceans, coastwise, or Great Lakes routes must have a means to take blood pressure readings, splint broken bones, and apply large bandages for serious wounds. First Aid Kit Expired equipment expired or missing, replace first aid kit. - 46 CFR 140.435 - 17c

The system is arranged and installed so a fire in the engine room automatically sets off alarms on a fire detection control panel at the operating station. Heat Detector in E/R was tested and continued alarming once activated. O/O repaired with spares in stock. System was back to normal with no faults. - 46 CFR 142.330(a)(3) - 10c

Each towing vessel must carry one appropriately-sized lifejacket for each person onboard. Missing One Lifejacket. - 46 CFR 141.340(a) - 10c

Each towing vessel operating on oceans, coastwise, limited coastwise, or beyond 3 nautical miles from shore upon the Great Lakes must carry a Category 1, 406 MHz satellite Emergency Position Indicating Radio Beacon (EPIRB). Replace damaged EPIRB.

Each towing vessel 79 feet or more in length operating on oceans and coastwise routes that does not have an installed fixed fire-extinguishing system must have the following: At least two firefighter's outfits that meet NFPA 1971 and Two self-contained breathing apparatus of the pressure demand, open circuit type, approved by the National Institute for Occupational Safety and Health (NIOSH). Provide documentation showing the servicing of the Firefighting sets.

All installed bilge piping must have a check/foot valve in each bilge suction that prevents unintended backflooding through bilge piping. Provide evidence that the bilge suctions have a check/foot valve installed.

All machinery and equipment that is not in proper working order (including missing or malfunctioning guards or safety devices) must be removed; made safe through marking, tagging, or covering; or otherwise made unusable. Install STBD generator flywheel machinery guard.

Piping and machinery components that exceed 220 °C (428 °F), including fittings, flanges, valves, exhaust manifolds, and turbochargers, must be insulated. Install lagging around port and STB MDE’s turbochargers.

All seals and gaskets must be properly maintained to prevent leaks of flammable or combustible liquid, as those terms are defined in 46 CFR subpart 30.10, into the machinery space. Repair leaks and clean up oil on the Starboard generator.

The owner or managing operator must implement procedures to identify and mitigate health and safety hazards. Secure freezer and refrigerator located in the galley.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Remove loose cables in the 02 deck of the engine room as well as place adequate outlet covers on all electrical outlets.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Secure engine harness cabling located FWD STBD side of the STBD MDE.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Provide marine grade battery charger for STBD generator battery bank.

Each towing vessel must have either a self-priming, power-driven, fixed fire pump that is capable of Being energized remotely from a safe place outside the engine room and at the pump. Install remote fire pump controllers.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Replace or repair ground detection button on the main switchboard.

All pressure vessels must have the MAWP indicated by a stamp, nameplate, or other means visible to the crew. Stencil MAWP on Port and STBD air receivers.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Mount and replace PORT generator oil pressure gauge.

A towing vessel subject to this subchapter and choosing the Coast Guard inspection option, or required to have the Coast Guard inspection option, must undergo an annual inspection within 3 months before or after the COI anniversary date. VSL must undergo an Annual inspection before it can operate.

The owner, managing operator, or master of each towing vessel must maintain the required navigational safety equipment in a fully-functioning, operational condition. The compass light as found not operational, replace lightbulb and prove proper operations of compass light. 46CFR140.620.

The VHF radios must be connected to a GPS receiver or have a VHF-DSC Marine Radio with integral GPS. The VHF radios have been found not to be displaying GPS coordinates. Prove proper operations of VHF-DSC radios by connecting them to a GPS receiver to display GPS coordinates; or install VHS-DSC radios with integral GPS. 47CFR80.

Each towing vessel must be equipped with at least two portable, battery-powered lights. One must be located in the pilothouse and the other at the access to the engine room. The two portable battery-powered lights has been found not operational. Provide at least two operational portable battery-powered lights. 46 CFR 143.410 (c)

Alarms must be visible and audible at each operating station. The alarm located at the operating station may be a summary alarm; if the alarm at the operating station is a summary alarm, the specific alarm condition must be indicated at the machinery or bilge location. The redundant alarm panel installed in the operating station was not found properly operating by resetting and shutting-off every time the high bilge water alarm was activated. Prover proper operation of redundant operating station alarm panel in the presence of an attending U.S. Coast guard marine Inspector, and provide a roving watch to monitor the main alarm panel located in the engine room, and enter the watch in the logbook.46CFR143.230 (b)(1).

A fire detection control panel or a remote indication panel must have visual alarm alarms to identify the zones or zones of origin of the fire. Crew was unfamiliar with the different alarm zones identified in the fire alarm detection panel upon testing. Provide a diagram or list that identifies the different fire zones and post it next to the fire detection panel.46 CFR 142.330 (3) (iii).

Means must be provided for stopping each fan in a ventilation system serving machinery spaces and for closing, in case of fire, each doorway, ventilator, and annular space around funnels and other openings into such openings. The means of securing the starboard side funnel exhaust ventilation cover was found to have missing the securing bolt and unable to be closed. Prove the proper operation of the ventilation cover in for the satisfaction of an attending USCG Marine Inspector. 46CFR 144.605.

An exposed hazard such as gears and rotating machinery, must be protected by a cover, guard or rail. The rotary machinery guard of the winch generator was found broken and laying on the deck, with the base of the securing screw also broken. Repair or replace broken rotary machinery guard and install on its location.46 CFR 144.820

All bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids, insofar as practicable. Remove and clean engine room bilges as it was found with a large considerable amount of oily bilge water.46 CFR 142.220.

All bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids, insofar as practicable. Remove all deck covers in the engine room and remove all debris, trash, rags and parts that were found in the engine room bilges.46 CFR 142.220.

Storage areas are kept free from accumulation of combustible and flammable materials insofar as practicable. Remove all flammable materials such as cardboard and excess clutter of equipment that are not in use from the engine room and deck above engine room, adjacent to the mooring equipment. 46 CFR 142.220

Electrical equipment and electrical cables must be suitably protected from wet and corrosive environments. The starboard side generator was found to have a battery without a leak proof tray. Install battery tray and secure battery.46 CFR143.400 (f)

Closure devices must be provided for hull penetrations, which open to the exterior of the vessel and which may allow water to enter the vessel. The slop-holding tank located in the engine room was found to have missing its closing device for at least 5 days. Re-install closing device and make sure it remains installed to prevent spill or water intrusion.46CFR144.320 (b).

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Two lighting fixtures has been found without its cover. Properly install lighting fixture covers.46 CFR 143.400.

All seals and gaskets must be properly maintained to prevent leaks of flammable or combustible liquid. All three generators, both port and starboard main diesel engines and towing gear winch at the stern of the vessel has been found leaking, creating a fire hazard. Properly maintain or replace gaskets and clean machinery, generators and towing winch. 46 CFR 143.220 (a).

The strength of each component used for securing the towing vessel to the tow and for making up the tow is adequate for its intended service. Properly replace a 10 to 12 feet segment of towing cable that was found to to have frayed wires in the stern towing winch. 46 CFR 140.801.

After any survey of the ship has been completed, no change shall be made in the structure or scantlings without the sanction of the Administration. The vessel was found with uncertified welders on board the vessel conducting hotwork by welding on the #2 starboard fuel tank vent without notifying the U.S. Coast Guard(Administration) nor the American Bureau of Shipping (Recognized Organization). Furthermore, tank vent pipe diameter was modified from 3.5" to 2.5" diameter.

No alteration or repair involving welding shall be made within or on the boundaries of fuel tanks without the inspection and approval of a marine chemist certificated by the National Fire Protection Association (NFPA). The vessel was found with welders on board conducting hotwork by welding on the #2 starboard fuel tank vent without the proper inspection and issuance of a Marine Chemist Certificate.

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford protection against hazards to life, property, and the environment. Vessel propeller has to be inspected, and diving report must be provided, to assure extend of damage by their towing cable on their propellers.

Vessels described in Table 136.250 of this section that operate on the Great Lakes or outside the Boundary Lines, as set forth in 46 CFR part 7, are subject to load line requirements in subchapter E of this. Vessel is to return to Ceiba, PR and await a damage survey from marine inspectors prior to movement.

All towing vessels must be operated in a manner that minimizes the risk of down-flooding and progressive flooding. In addition, the master must ensure that all hatches, doors, and other openings designed to be watertight function properly. Aft port ballast tank hatch cover and Stbd access hatch to steering gear compartment were both missing one bolt.

A workspace must be of adequate construction to provide for the protection of the crew. Engine room stair rail loose and unstable.

All towing vessels must be operated in a manner that minimizes the risk of down-flooding and progressive flooding. In addition, the master must ensure that all hatches, doors, and other openings designed to be watertight function properly. Each weathertight door in the main deck at the port and starboard sides have inadequate gaskets, frozen dogs, and do not properly close.

Each vessel must have a stability letter issued by the Coast Guard and comply with its conditions of operation. This includes maintaining the bilges pumped to a minimum the content at all times. The engine room has a high amount of oily water in the bilges and it was as deep as 2.5 feet in some areas.

All seals and gaskets must be properly maintained to prevent leaks of flammable or combustible liquid into the machinery space. The fuel oil manifold main valve was found leaking and had a diesel soaked rag tied around it.

The owner and managing operator must ensure that the towing vessel is in compliance with this subchapter and other applicable laws and regulation at all times. USCG MI observed that the port side engine and generator were completely torn down and inoperable at the time of a scheduled Annual Inspection. Essential equipment was unavailable for inspection and testing.

The system is installed,tested, and maintained in accordance with the manufacturer's design manual. MI observed a flashing amber trouble light on the fire detection panel that could not be cleared by the vessel crew.

Each towing vessel must be maintained and operated so as to minimize fire hazards and to ensure the following: All bilges and void spaces are kept free from accumulation of combustible and flammable materials insofar practicable. Coast Guard Marine Inspectors observed an excessive accumulation of combustible materials (oil) in the machinery space bilge

Each remote valve control must be marked in clearly legible letters, at least 25mm (1 inch) high, indicating the purpose of the valve and the way to operate it. MI observed that the sticker, labeling the remote valve control for the fuel shut off valve was ripped and illegible.

A single Side band radiotelephone capable of operating on all distress and safety frequencies in the medium and high frequencies bands listed. MI observed the SSB radio had been removed due to age and lack of capability and not yet replaced.

All vessel equipment must be used in accordance with the manufacture's recommended recommend practice and in a manner that minimizes risk of injury or death. This includes machinery, deck machinery, towing gear,ladders, embarkation devices, cranes, portable tools, and safety equipment. Marine Inspector observed an unsecured and uncapped propane cylinder on deck.

Each towing vessel must be equipped with an industrial type first aid kit cabinet or kit, appropriate to the size of the crew and operating conditions. Marine Inspector observed many of the first aid kit items were expired

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Coast Guard Marine Inspector observed the following electrical hazards through-out the vessel: Dead-end exposed electrical wiring in way of batteries located in aft operating station. Unmounted electrical outlet above galley oven. Electrical box in forward Port side main machinery space did not have the required cover. Light bulbs missing protective covers in head and forward compartment. Galley stove top was missing one element, exposing interior electrical connecter.

Vessel fouled starboard propeller with main tow wire. Currently located at the ADM terminal in Southport, NC effecting repairs.

A VIOLATION OF 33 C.F.R. 86.23 IN THAT THE UTV DRIVER HAD IMPROPER BELL SIZE AND BELL WAS MISSING IT'S STRIKER.

A VIOLATION OF 46 C.F.R. 28.135 IN THAT THE UTV DRIVER FAILED TO MARK PFD'SWITH THE REQUIRED NAME OF THE VESSEL ON THE BACK OF THE PFD.

A VIOLATION IN THAT THE UTV DRIVER FAILED TO HAVE THE OFFICIAL NUMBER PERMANENTLY AFFIXED TO AN INTERIOR STRUCTURE.

A VIOLATION OF 46 U.S.C. 8104(D) IN THAT THE UTV DRIVER DOES NOT HAVE SUFFICIENT PERSONNEL TO STAND THE REQUIRED THREE WATCHES THAT IS REQUIRED FOR VESSELS OVER 100GT.

A VIOLATION OF 46 C.F.R. 28.135 IN THAT THE UTV DRIVER FAILED TO HAVE THE NAME OF THE UTV MARKED ON THE FRONT OF THE LIFERING.

A VIOLATION OF 33 C.F.R. 159.59 IN THAT THE UTV DRIVER FAILED TO HAVE PLACARDS ATTACHED TO MSD WHICH PROVIDE SAFETY PRECAUTIONS, OPERATING INSTRUCTIONS AND WARNINGS PERTINENT TO THE MSD.
