Manholes and flush scuttles in position 1 or 2 or within superstructures other than enclosed superstructures shall be closed by substantial covers capable of being made watertight. Found a blackwater plastic pipe to be passed through an opened blank pipe penetration rendering the freeboard deck to not be watertight. The crew additionally installed a new valve in the tank top of the black water tank to connect the plastic pipe.

Where penetrations of watertight bulkheads and internal decks are necessary for access, piping, ventilation, electrical cables (etc.), arrangements are to be made to maintain the watertight integrity. Found an electrical penetration through the watertight bulkhead between the STBD Engine room into the engineer's workshop damaged, rendering the penetration no longer watertight. The wire rack leading into the penetration is damaged on the engine room side, pulling the wires downward. Additionally, starboard switchboard room found with damaged through deck electrical penetrations.
