Manholes and flush scuttles in position 1 or 2 or within superstructures other than enclosed superstructures shall be closed by substantial covers capable of being made watertight. Found a blackwater plastic pipe to be passed through an opened blank pipe penetration rendering the freeboard deck to not be watertight. Fire doors in stairway enclosures shall satisfy the following requirements... the doors shall be self closing. In the main vertical stairway, found the main deck, A-deck, B-deck, C-deck, and all engine space "A" class doors to not self-close; the self-closing devices are inoperable. 74 SOLAS (01) II-2/30.4.1 Action Taken: 60 a c

An alarm system shall be provided indicating any fault requiring attention and shall be capable of sounding an audible alarm in the main machinery control room. STBD cement space bilge alarm would not sound or indicate when tested by the vessel's crew. 74 SOLAS (04)II-1/51.1.1 Action taken: 60 a c

Where penetrations of watertight bulkheads and internal decks are necessary for access, piping, ventilation, electrical cables (etc.), arrangements are to be made to maintain the watertight integrity. Found an electrical penetration through the watertight bulkhead between the STBD Engine room into the engineer's workshop damaged, rendering the penetration no longer watertight. The wire rack leading into the penetration is damaged on the engine room side, pulling the wires downward. Additionally, starboard switchboard room found with damaged through deck electrical penetrations. 74 SOLAS (04) II-1/25-9(1) Action Taken: 16 a c

Plans (fire control plan) and booklets shall be kept up to date, any alterations being recorded thereon as soon as practicable. The on board fire control plan does not list the firefighting equipment (call points, smoke detectors, portable fire extinguishers, etc.) that is present on the main deck to support installed deck equipment (PAMS, generators, hydraulic power units, etc.) The fire control plan posted on the bridge is not the most recent & approved copy of the FCP. 74 SOLAS (01) II-2/20.1 Action Taken: 16 a c

After any survey the ship under regulations 7, 8, 9 or 10 has been completed, no changes shall be made without the sanction of the administration. Ship found with a fuel oil pipe installed from the 711 port ULSD tank to a supply on the main deck; modification is not reflected on ship's drawings and is not approved by the vessel's class society. 74SOLAS(20) I/11(b) Action Taken: 16 a c

The frequency of the tests and inspections required by this section must be: for vessels, annually or as part as the biennial or mid-period inspections. The hydrostatic test of fuel oil piping shall be at least 1.5x MAWP of the system. Portable fuel hose on main deck is not tested to the above listed standard. Ship has made modifications to the sludge tank system. A pipe has been added interconnecting the aft dirty oil tank and the forward dirty oil tank. The hydrostatic test of fuel oil piping shall be at least 1.5x MAWP of the system. Ship's bunker manifold & piping is not tested to the above listed standard. Repair leak(s) in heliport jet fuel station prior to helicopter fueling.

Remove sandstorm mesh material from ventilation intakes.

Restore electrical penetration/transit for cable runs penetrating driller shack.

Repair crushed deluge nozzle on face of driller shack.

Renew/repair airlock doors to mud pit entries as necessary to demonstrate proper operation

Repair mud pit vetilation damper VD420303 and demonstrate proper operation.

Number 6 thruster local control RPM gauge gives false reading when thruster is rotated counterclockwise. Prove satisfactory operation of RPM gauge.

Renew broken light cover on forward port side of drill floor.

Replace receptacle outlet cover & remove flammable liquids (paints) from mud pump room.

PROVIDE FLAG STATE ACCEPTANCE OF CO2 COVERAGE FOR SPACE THAT IS MISSING 04 BOTTLES DUE TO SERVICE ASHORE.

Properly label and maintain immersion suits or remove via flag state exemption.

Remove holding straps from all portable fire extinguishers.
