Marine Inspector inspected bilges for removed fuel oil, finding additional pockets of fuel oil. All deck plates shall be removed in the engine room in order for Marine Inspector to check the condition of the bilges. 46 CFR 137.200(d) 705c - prior to conducting commercial towing ops.

Each owner or managing operator must develop procedures to routinely examine, maintain, and replace capstans, winches, and other machinery used to connect the towing vessel to the tow. Marine Inspector inspected aft tow winch engine located on the upper tier aft engine room. Identified excessive amount of hydraulic leaks and a fuel soaked oil absorbent rags. 46 CFR 143.460(d)

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford protection against hazards to life, property, and the environment. MI observed a multitude of deficiencies indicating that a serious substandard condition is not being proactively managed by the Company, vessel owner, and/or operator. 46 CFR 140.205(a) 705c - prior to conducting commercial towing ops.

All seals and gaskets must be properly maintained to prevent leaks of flammable or combustible liquid, as those terms are defined in 46 CFR subpart 30.10, into the machinery space. Marine Inspector witnessed dirty oil tank at the port side of fwd engine room bulkhead leaking into the bilge. 46 CFR 143.22(a)

Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems, must be maintained to ensure proper operation. Marine Inspector identified port generator with exhaust leaks throughout the length of exhaust piping. 46 CFR 143.205(c)(1) 705c - prior to conducting commercial towing ops.

Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems, must be maintained to ensure proper operation. Marine Inspector identified both port/stbd stern tube shaft seas pouring water. 46 CFR 143.205(c)(1)

The cognizant OCMI may required review of the vessels watertight and weathertight integrity. While conducting hose test, Marine Inspector witnessed water pouring into port/stbd galley doors, both hatches on the aft lazarette, and fwd hatch on forepeak. Make satisfactory repairs. 46 CFR 144.330

Piping and machinery components that exceed 220 degrees celcius, including fittings, flanges, valves, exhaust manifolds... must be insulated. Marine inspector identified both port/stbd main diesel engines had areas where insulation had been removed from the turbo sections. 46 CFR 140.505(c) 705c - prior to conducting commercial towing ops.

Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems, must be maintained to ensure proper operation. Marine Inspector identified both port/stbd main diesel engines with exhaust leaks throughout entire length of exhaust piping. 46 CFR 143.205(c)(1) 705c - prior to conducting commercial towing ops.

All machinery and equipment that is not in proper working order (including missing or malfunctioning guards or safety devices) must be removed; made safe through marking, tagging, or covering; or otherwise unusable. Marine Inspector identified port generator missing a guard pulley system of the front engine. 46 CFR 140.505(c)

Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems, must be maintained to ensure proper operation. Marine Inspector observed a fan blower on both port/stbd main diesel engines to cool the main engines due to an overheating problem. 46 CFR 143.205(c)(1) 705c - prior to conducting commercial towing ops.

Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems, must be maintained to ensure proper operation. Marine Inspector identified overboard valve for the grey water piping with heavy corrosion on both sides of the valve where valve screws into pipe. 46 CFR 143.205(c)(1)

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. Marine Inspector was unable to witness testing of all bilge alarms due to lazarette space being inaccessible. 46 CFR 142.220(a)

Until an inspection has been made to determine that such operation can be undertaken with safety, no alterations, repairs, or other such operations...must be made within or on the boundaries of fuel tanks. Marine Inspector observed some type of soft patch on the port/middle fuel oil tank inside the engine room. 46 CFR 143.230 705c - prior to conducting commercial towing ops.

All crew members must be provided with health and safety information and training that includes: (1) Content and procedures of the owner or managing operator's health and safety plan; (2) Procedures for reporting unsafe conditions; (3) Proper selection and use of PPE appropriate to the vessel operation; (4) Safe use of equipment including deck machinery, rigging, welding and cutting, hand tools, ladders, and abrasive wheel machinery found onboard the vessel; (5) Hazard communication and cargo knowledge; (6) Safe use and storage of hazardous materials and chemicals; (7) Confined space entry; (8) Respiratory protection; and (9) Lockout/Tagout procedures. Marine Inspector observed that there was no proof all of the topics of the health and safety plan were being covered for the annual refresher training. 46 CFR 140.515(a)(1)-(9)

Marine Inspector inspected the lazarette finding 3/8" hole and 18" diameter wet spot near the hole that was wet in the bulkhead forward shared by both aft F/O tanks. Location from port side two bays in. Gauge entire bulkhead in the presence of the Marine Inspector. Completion of gauging will determine if other areas need to be addressed. Additionally gauging will be conducted on the main deck above lazarette and bottom shell plating. 46 CFR 137.200(d)

All bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable. Marine Inspector noted diesel accumulation in the bilge. Remove bilge slops from bilge and remove all deck plating to gain access to bilge. 46 CFR 142.220(a)

Marine Inspector identified numerous holes in main deck port side overhead in the engine room that had been temporarily patched previously. Brings concerns for all main decks from lazarette forward to bow for thickness of metal. Conduct gauging for main deck starting front of lazarrette to the bow including both port and stbd sides in the presence of CG Marine Inspector. 46 CFR 137.200(d) 705c - prior to conducting commercial towing ops.

Master's responsibilities. The master of a towing vessel must ensure that drills are conducted and instructions are given to ensure that all crewmembers are capable of performing the duties expected of them during emergencies. This includes abandoning the vessel, recovering persons from the water, responding to onboard fires and flooding, or responding to other threats to life, property, or the environment. Annual inspection is incomplete and require drills to be conducted by Captain and crew in the presence of CG Marine Inspector. 46 CFR 140.420(a) 705c - prior to conducting commercial towing ops.

There must be an installed or portable bilge pump for emergency dewatering. Any portable pump must have sufficient hose length and pumping capability. All installed bilge piping must have a check/foot valve in each bilge suction that prevents unintended backflooding through bilge piping. Marine Inspector was unable to test system due to contaminated bilges. Conduct operation test of the bilge system showing satisfactory operation pumping all ompartments that have bilge suction points. 46 CFR 143.275 705c - prior to conducting commercial towing ops.

To stop the flow of fuel in the event of a fire or break in the fuel line, a remote fuel shutoff valve must be fitted on any fuel line that supplies fuel directly to a propulsion engine or generator prime mover. (c) The valve must be installed in the fuel piping directly outside of the fuel oil supply tank. (d) The valve must be operable from a safe place outside the space where the valve is installed. e) Each remote valve control must be marked in clearly legible letters, at least 25 millimeters (1 inch) high, indicating the purpose of the valve and the way to operate it. Marine Inspector to witness proper operation of all emergency fuel shutoffs, location and proper labeling. 46 CFR 143.260(b)(c)(d)(e) 705c - prior to conducting commercial towing ops.

Each towing vessel must be equipped with navigation lights, shapes and sound signals in accordance with the international regulations for prevention of collisions at sea or 33 CFR 84 as appropriate. Vessel is missing day shapes for towing vessels, two balls and one diamond. Provide to the satisfaction of the attending MI. 46 CFR 140.720

Each towing vessel must have a reliable means to provide notification when an emergency condition exists for high bilge levels and they must be visible and audible at each operating station. Bilge alarm for the main engineering space only shows visible on bridge and bilge alarm is not present in MSD space. Ensure high bilge alarms are both audible and visual on bridge and at the aft conning position. 46 CFR 143.230

Annual inspections will be similar for certification but will cover less detail unless the marine inspector finds deficiencies or determines that a major change has occurred since the last inspection. Vessel must conduct a man overboard drill in the presence of the attending MI with the crew that will be operating the vessel. 46 CFR 137.200 (b)

Machinery and electrical systems must be designed and maintained to provide for safe operations of the towing vessel and safety of persons on board under normal and emergency conditions. The air lines suppling the STBD throttle controls are leaking by at the connection. Repair to the satisfaction of the attending MI. 46 CFR 143.205 (a)

Towing vessels must meet the communication requirements of 33 CFR 26 and 33 CFR 164.72. Port side VHF radio does not have vessels GPS properly configured. Correct to the satisfaction of the attending MI. 46 CFR 140.715

Illuminated magnetic compass. The compass must be readable from the each operating station. The vessels magnetic compass is not illuminated. Repair to the satisfaction of the attending MI. 46 CFR 140.725

The design of the system and its installation on the towing vessel is certified and inspected by a registered professional engineer with experience in fire detection system design, by a tech with qualifications as a National Institute for Certification in Engineering Technologies level IV fire alarm engineering Technician or by an authorized Class Society with equivalent experience. Vessel could not provide proof of the fire detection system certification. Supply to the satisfaction of the attending MI. 46 CFR 142.330 (a)(7)

Each towing vessel must perform the applicable tests in table 143.245 (b) Pressure vessel relief valves twice in 5 with no more than three years elapsing since last test. The port side unfired pressure vessel (80 gal / 10.6 cubic feet) safety relief could not be popped to prove functionality. Replace in kind with a safety relief that will open before 200 PSI as air receiver has a 200 PSI MAWP. Safety relief is directly on top of UPV. 46 CFR 143.245 (b)

Closure devices must be provided for deckhouse or hull penetrations, which open to the exterior of the vessel and which may allow water to enter the vessel. The STBD side engine room door gasket has a 1/4" gap at the top of the door. Replace the gasket to the satisfaction of the attending MI. 46 CFR 144.320 (b)

A 40B portable fire extinguisher must be located near the flammable storage cabinet. The portable extinguisher at the flam storage cabinet was missing a pin, corrected on the spot. 46 CFR 142.225 (d)

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford protection against hazards to life, property and the environment. Vessel did not have a lock out tag out log to properly secure equipment for maintenance. Corrected on the spot. 46 CFR 140.205 (a)

Each EAP education program must include at least the following elements... a service hot line telephone number for the crew member assistance. Hot line was not posted in crew galley. Corrected on the spot. 46 CFR 16.401 (a)

No person may operate any vessel equipped with installed toilets facilities unless it is equipped with an operable type II or III device. Vessel has installed type II device but did not have any chlorine for the treatment of the waste. It was provided prior to the MI departure. 33 CFR 159.7

A towing vessel subject and choosing USCG inspection option must undergo an annual inspection within 3 months before or after the COI anniversary date. Vessel's COI anniversary date is 29 April, vessel was drydocked until end of July and has postponed the annual inspection that was scheduled for 7 August. Vessel is restricted from operations until an annual inspection is conducted to the satisfaction of the attending MI. 46 CFR 137.200

An internal structural examination...consists of a complete examination of the vessel's main strength members, including the major internal framing. Framing in the MSD compartment was observed to be wasted and in need of renewal.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Fuel tanks have salt water ingress. Make all affected tanks gas free and ready for inspection.

AIS must be maintained in effective operating condition, which includes the accurate broadcast of a properly assigned Maritime Mobile Service Identity (MMSI) number. AIS broadcasts inaccurate MMSI number.

The bridge-to-bridge radiotelephone station will be inspected on vessels subject to regular inspections. If after such inspection, the Commission determines that the Bridge-to-Bridge Act, the rules of the Commission and the station license are met, an endorsement will be made on the appropriate document. No Bridge-to-bridge Safety Certificate on board.

Sufficient lighting suitable for the marine environment must be provided within crew working and living areas. Lights in crew berthing were missing guards.

Any towing vessel with alternating watches (shift work) or overnight accommodations must identify the duties and duty stations of each person onboard during an emergency, including: fire, abandoning the vessel, man overboard, etc. The vessel has two WQSB with duties that are not the same. Provide crew one list of required emergency duties.

Towing vessels are required to have a COI for the route and intended operation. Prior to conducting towing vessel operations, have a valid and current COI onboard.

Each cable and wire must be supported in order to avoid chafing or other damage. Wire run in overhead above engine room missing support.

Closure devices must be provided for deckhouse or hull penetrations, which open to the exterior of the vessel and which may allow water to enter the vessel. These devices must be suitable for the expected route. Deadllight covers had broken securing points.

Towing vessels must be equipped with a properly operating illuminated compass. The compass has excessive air and requires additional fluid.

Repairs conducted on an existing vessel must comply with the standards applicable to the vessel at the time of construction with the regs in this part. Vessel notified MSD FT Macon of vessel taking on water in laz. M/I’s attended vessel and verified damage. Vessel appears to have 03 holes on STBD side in laz compartment located behind rub rail. Vessel needs to submit repair plan to CG identifying materials and procedures taken to rectify. Vessel needs to request permission prior to transiting about the port.

Vessels on a coastwise voyage beyond 3 miles from shore are required 6 day and 6 night flares. Vessel had on board 6 hand red flares, 3 expired orange smoke, and 3 rocket parachute flares (expiring end of March 2022). Vessel is restricted to a coastwise route within 3 miles from shore until 6 more day/night flares are provided.

Each towing vessel must have Ring Life buoys approved in accordance with 46 CFR 160.050 or 160.150. The grab lines on the ring buoys in starting to fray when handled. Replace grab lines, or buoy as needed.

Update MMSI into VHF radios. 1 VHF radio has the incorrect MMSI, 1 does not have MMSI input. 17Mar2022: MMSI put into one of the radio's other will need to be sent back to the manufacturer to reprogram.

Discharge of oil to the sea from any ship is prohibited. 33CFR151.10. the suction side of the fire pump is arranged so that it may take suction from the oily waste tank. Properly terminate oily mixture supply line to the fire pump. Prior to movement.

Fixed fire pump must deliver water from two highest hydrants simultaneously at a pressure of 50 psi and at least 80 gallons per minute, and be able to be energized remotely from a safe place outside the engine room. 46CFR142.325. fire pump not able to deliver sufficient psi or gpm. Provide pump capable of delivering 50 psi and 80 gpm and demonstrate ability to energize from remote location and local controller. Prior to movement.

A remote fuel shut off valve must be installed in any fuel line supplying fuel to a main engine or generator. 46CFR143.260. Not all fuel shut offs are operational. Make repairs and demonstrate proper operation of all fuel shut offs. Prior to movement.

Fuel piping must be made of steel or copper, each pipe wall must have a minimum wall thickness of .035 inch. 46CFR143.260(d). non metallic flex fuel hose is acceptable only if: it is used in lengths of less than 30 inches, is visible and easily accessible, fabricated with compression fitting compliant with SAE J1475 or SAE J1942. All fuel lines from the supply line to the fuel filters are longer than 30 inches and all flex hose. All fuel lines must be installed in compliance with the above requirements. Prior to movement.

Each towing vessel must have a fire-detection system installed to detect engine room fires which complies with 46 CFR 142.330. Fire detection system not compliant or operable. 46 CFR 142.330. Prior to movement.

All towing vessels must be operated to minimize fire hazards 46CFR142.220. all 4 fuel tank site glasses not of approved type and fuel oil dispensing hoses not authorized. Install approved site glasses with isolation valves and terminate fuel dispensing stations with plastic hoses. Prior to movement.

Bilge system must be install and operational 46CFR143.275. unable to demonstrate proper operation of bilge system. Demonstrate ability to take suction from all suction lines and make strainer at end of valve available for inspection. Prior to movement.

All machinery and electrical equipment must be maintained and operated to ensure safe operation of the towing vessel 46 CFR 143.205. Both main diesel engines, generators and steering gear not able to be tested. Demonstrate proper operation of all equipment. Prior to movement.

Discharge of oil to the sea from any ship is prohibited. 33CFR151.10. Bilge system and fire main systems are tied together and not labeled permanently. Label and separate bilge system from fire main. Correct prior to 16AUG2020.

Fuel piping must be made of steel or copper, each pipe wall must have a minimum wall thickness of .035 inch. 46CFR143.260(d). Fuel supply line from filter to pump has bleeding loops installed on all fuel filter systems to bleed system. Remove flex hose bleed systems and repair piping properly. Correct prior to 16AUG2020.

Fuel piping must be made of steel or copper, each pipe wall must have a minimum wall thickness of .035 inch. 46CFR143.260(d). Generator fuel piping is thin plastic lines. Install in accordance with regulation or show system approval as installed from manufacturer. Correct prior to 16AUG2020.

Electrical systems must be installed to function properly and prevent shock or fire hazards. 46CFR143.400(a). Battery chargers for main engine and generator banks are not installed at the moment. Install marine approved permanent chargers and demonstrate proper operation. Correct prior to 16AUG2020.

All towing vessels must be operated to minimize fire hazards 46CFR142.220. All Raycore filters requiring flame deflector bowls, must have them installed. Install flame deflectors as required on all fuel filters. Correct prior to 16AUG2020.

Cable runs must be installed with metal supports at least 48 inches apart. 46CFR143.575(2). Many electrical cable runs in engine room and other spaces not properly supported. Provide metal supports for horizontal and vertical wire runs every 48 inches or less. . Correct prior to 16AUG2020.

Electrical systems must be installed to function properly and prevent shock or fire hazards. 46CFR143.400(a). All electrical connections must be made within an enclosure. Ensure all electrical connections in engine room and any other spaces are made within an enclosure. Correct prior to 16AUG2020.

Exposed hazard, such as rotating machinery must have a guard installed to prevent personnel hazard. 46 CFR 144.820. Steering pump coupling for both pumps has no guard. Install guard to protect personnel. Correct prior to 16AUG2020.

The owner or managing operator of an existing vessel not operating under a stability document must be able to show at least one of the following: 1) The vessel's operation or a history of satisfactory service does not cause the stability of the vessel to be questioned by either the Coast Guard or a TPO engaged to perform an audit or survey. (2) The vessel performs successfully on operational tests to determine whether the vessel has adequate stability and handling characteristics. 3) The vessel has a satisfactory stability assessment by means of giving due consideration to each item that impacts a vessel's stability characteristics which include, but are not limited to, the form, arrangement, construction, number of decks, route, and operating restrictions of the vessel. Vessel does not have a Stablity Letter. 46 CFR 144.300. Submit email requesting stability letter based on satisfactory service over the last however many years of service. Correct prior to 16AUG2020.

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels. No program in place. 46 CFR 16.230(a). Establish program demonstrate compliance. Correct prior to 16AUG2020.

Prior to getting underway for the first time on a particular towing vessel, each crewmember must receive a safety orientation in compliance with 46 CFR 140.410(b) No crewmember orientation. 140.410(b). Provide safety orientation for inspection. Correct prior to 16AUG2020.

Each vessel must carry a certificate with a valid endorsement while subject to the Bridge-to-Bridge Act (see 47 CFR 1001(c) for applicability). No valid FCC Bridge-to-Bridge Certificate onboard. 47 CFR 80..1005. Provide FCC Bridge-to-Bridge Certificate. Correct prior to 16AUG2020.

The officer in charge of a navigational watch must conduct a navigation assessment for the intended route and operations prior to getting underway. The navigation assessment must incorporate the requirements of pilothouse resource management of §140.640. No Navigation assessments logged. 140.635(a). Demonstrate compliance with applicable regulation. Correct prior to 16AUG2020.

Each towing vessel must be equipped with the following navigational-safety equipment: Notices to Mariners published by the National Geospatial-Intelligence Agency, or LNMs published by the Coast Guard. No Notice to Mariners onboard. 33 CFR 164.71(b)(2)(ii)(B). Provide all required publications. Correct prior to 16AUG2020.

Each towing vessel must be equipped with navigation lights, shapes, and sound signals in accordance with the International Regulations for Prevention of Collisions at Sea (COLREGS) or 33 CFR part 84 as appropriate to its area of operation. Sound signaling device not operable. 46 CFR 140.720. Provide sound signaling device. Correct prior to 16AUG2020.

Towing vessels must be equipped with the following equipment, as applicable to the area of operation: (b) Search light, controllable from the vessel's operating station and capable of illuminating objects at a distance of at least two times the length of the tow. Search lights not operable. 46 CFR 140.725(b). Demonstrate operation of search lights. Correct prior to 16AUG2020.

Towing vessels must be equipped with the following equipment, as applicable to the area of operation: (d) Illuminated magnetic compass. Compass is not illuminated. 46 CFR 140.725(d). Demonstrate proper operation of compass light. Correct prior to 16AUG2020.

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention in compliance with 46 CFR 143.230. No alarms in wheelhouse. 46 CFR 143.230. Provide all required alarms. Correct prior to 16AUG2020.

Each lifejacket must have a lifejacket light approved under approval series 161.012 or 161.112 securely attached to the front shoulder area of the lifejacket. No life jacket lights on life jackets. 46 CFR 141.340(g)(1). Correct prior to 16AUG2020.

Each towing vessel operating north of lat. 32° N. or south of lat. 32° S. must carry the number of immersion suits as prescribed. Vessel needs 2 more immersion suits to accommodate 6 crewmembers. 46 CFR 141.350. Correct prior to 16AUG2020.

The owner or managing operator must maintain valid proof of registration of an EPIRB. No proof of EPIRB registration onboard. 46 CFR 141.380(d). Correct prior to 16AUG2020.

Vessel must have line-throwing appliance in accordance with 46 CFR 141.385. The line throwing appliance must have: (2) An auxiliary line that: (i) Is at least 450 meters (1,500 feet) long; (ii) Has a breaking strength of at least 40 kilonewtons (9,000 pounds-force); and (iii) Is, if synthetic, of a dark color or certified by the manufacturer to be resistant to deterioration from ultraviolet light. No apliances (4), or auxiliary line for the line throwing appliance. 46 CFR 141.385(b) Correct prior to 16AUG2020.

Each hose used to transfer fuel to a vessel that has a fill pipe for which containment cannot practically be provided must be equipped with an automatic back pressure shutoff nozzle. No containment or back pressure shutoff nozzle is provided at the fuel transfer station. 33 CFR 154.500(h). Correct prior to 16AUG2020.

Rails or equivalent protection must be installed near the periphery of all decks accessible to crew. Equivalent protection may include lifelines, wire rope, chains, and bulwarks that provide strength and support equivalent to fixed rails. Starboard focsle bulwarks removed for repairs. 46 CFR 144.800(a). Correct prior to 16AUG2020.

The owner, managing operator, master or officer in charge of a navigational watch of a towing vessel must ensure the strength of each component used for securing the towing vessel to the tow and for making up the tow is adequate for its intended service. The semi-portable fire extinguisher in the fiddley is not operable. 46 CFR 142.315(a). Correct prior to 16AUG2020.

Closure devices must be provided for deckhouse or hull penetrations, which open to the exterior of the vessel and which may allow water to enter the vessel. These devices must be suitable for the expected route. The soft patch around the exhaust stacks on the 02 deck through the fiddley overhead is not watertight, heavily corroded, and not bolted down. 46 CFR 144.320(b). Correct prior to 16AUG2020.

Piping and tanks exposed to the outside of the hull must be made of metal and maintained in a leak free condition. Air intake piping in fiddley overhead are heavily corroded and holed through. 46 CFR 143.270. Correct prior to 16AUG2020.

Each towing vessel must be equipped with a means to detect smoke in the berthing spaces and lounges that alerts individuals in those spaces. This may be accomplished by an installed detection system, or by using individual battery-operated detectors meeting UL 217 No smoke detection installed in berthing areas 142.330(b)

Towing vessels more than 65 feet in length must use navigation lights that meet UL 1104 (incorporated by reference, see §136.112 of this subchapter) or other standards accepted by the Coast Guard. Navigation light bulbs not approved. 46 CFR 143.415(a). Correct prior to 16AUG2020.

An existing vessel may be deemed by the OCMI, or TPO, to be in compliance with this subpart provided that either: (a) The vessel is built, equipped, and maintained to conform to the rules of a recognized classification society appropriate for the intended service and routes, but not classed; or (b) The vessel has been both in satisfactory service insofar as structural adequacy is concerned and does not cause the structure of the vessel to be questioned by either the OCMI, or TPO engaged to perform an audit or survey. (02) weathertight doors on both sides of the wheelhouse are heavily corroded and not weathertight; (02) watertight doors on both sides of the galley are heavily corroded and not watertight; (01) watertight door on the port side fiddley is deformed, corroded, and not watertight; (01) watertight door on the starboard side fiddley is not watertight. 46 CFR 144.200. All owners and operators of an existing towing vessel must have a valid COI prior to the specified phase-in period. This vessel is one of the vessels that has not obtained a valid COI. The vessel shall be made available for a Coast Guard COI inspection within 90 days5. If an inspection is not completed prior to this date, the action for this deficiency shall be changed to a 60 (Rectify deficiencies prior to movement) and the vessel may no long be permitted to operate.

For vessels other than those covered in paragraphs (b) and (c) of this section, the name of the vessel must be marked on some clearly visible exterior part of the port and starboard bow and the stern of the vessel. The hailing port of the vessel must be marked on some clearly visible exterior part of the stern of the vessel. Vessel not in compliance with 46CFR67.123.

Each towing vessel must be equipped with navigation lights, shapes, and sound signals in accordance with the International Regulations for Prevention of Collisions at Sea (COLREGS) or 33 CFR part 84 as appropriate to its area of operation. Starboard light cracked.

The week link for the life raft was not installed and the liferafts pull coard was made off to the vessel. Liferaft needs to be corectly rigged with week link.

EPIRB was mounted in the wheal house. EPIRB needs to be mounted outside so it can function as a float free.

The "Y" connection connecting the 2 CO2 bottles, for the Simi fixed CO2 hose real, above the engine room has been cut by the welder mounting bracket. Hose needs to be replaced and the hose chaffing issue from the mounting bracket for the welder needs to be corrected.

Vessel does not have Fuel Oil Transfer Procedures as required by 33 CFR 155.720 or that meet the requirements of 33 CFR 155.750
