Provide a visual indicator to illuminate when the automatic bilge pump in the engine compartment is operating.

Provide, mark (Fire Bucket), and attach a lanyard to 2 additional 2.5 gallon minimum fire buckets (3 total on board).

Mark the vessel's name in block capital letters on the ring life buoy.

Provide a new hydrostatic release for the EPIRB and mark to expire MAY 2027 prior to operating beyond 3 miles from shore.

Renew the retroreflective material on 3 adult life jackets as identified or replace the life jackets. Passenger count reduced to 46.

Provide a valid FCC Safety Radio Certificate.

Vessel currently has 09 red hand flare distress signals (day/night). Provide 03 additional red hand flare distress signals or orange smoke distress signals so the vessel has a total of 12 distress signals on board.

Vessel currently has 02 portable fire extinguishers on board. Provide 01 10:B-C portable fire extinguisher and mount at the Operating Station.

On a vessel of not more than 19.8 meters (65 feet) in length carrying not more than 49 passengers, a public address system is not required if a public announcement made from operating station without amplification can be heard throughout the accommodation spaces of the vessel during normal operating conditions, to the satisfaction of the cognizant [name withheld] cannot successfully communicate between decks in current state. Rectification may be achieved through a battery powered bullhorn which may serve as the public address system if audible throughout the accommodation spaces of the vessel during normal operating conditions. The bullhorn's batteries are to be continually maintained at a fully charged level by use of a battery charger or other means acceptable to the cognizant OCMI.

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. Lazarette visual indicator is inoperable.

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have a means to reset devices used to automatically shut down ventilation systems and engines. Crew unable to demonstrate PEFFES Engine Override.

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard. Master is unable to provide sufficient instruction to crew members due to lack of public address system or bullhorn.

Supervisory personnel must receive at least 60 minutes of Employee Assistance Personnel training. Master has not received training.

One of the two portable battery lights must be located at the operating station. Light was not available at the operating station.

Life floats were not properly affixed to weak link.

Stenciling on the Lifebuoy was not visible.

One orange flare missing.

Hydrostatic Release on EPIRB expired. Replace Hydrostatic release.

High Water audible alarms for aft and forward spaces were barely audible. Ensure alarms are able to be heard over the sound of the running engine.

Three way valve was in the overboard direction and overboard valve was not secured in the closed position. Secure overboard valve closed.

Lagging is worn in the vicinity of the turbo. Replace lagging to protect against accidental burns.

Electrical panel located under the operating station is utilizing automotive style fuses. Fuses must be of the cartridge type.

Port fuel tank shut off valve is not fully operational. Valve must fully close utilizing remote shut off.

Visual indicator for pre-engineered(sea fire) system is not operational at the operating station. Repair visual indicator.

Post all pages of the stability letter at the operating station.

Post all pages of COI in a location visible to passengers.

02 Portable fire extinguishers were found with corrosion around the neck and handle. Replace with acceptable fire extinguishers per COI.

Both life floats need new SOLAS tape, max persons labeled, painter lines re-secure, patched and repainted. Life floats paddles must have vessel name on them.

Ring buoy SOLAS tape was no longer reflective and buoy was coming apart at the seams. Replace/repair IAW 46 CFR 160.050.

VHF radio was missing both the MMSI number and the GPS coordinates.

Conduct underway drills in the presence of local Marine Inspectors.

Provide operator license for review to local Marine Inspectors.

No written drug and alcohol program. Provide.

Provide documentation of pre-employment testing and enrollment of all crew members in chemical testing program.

Life ring water light battery is expired. Renew battery. Vessel restricted to operating during day light hours.

Life float batteries are expired and one light has water in strobe. Renew/repair. Vessel restricted to operating no more than three (03) miles from land.

Demonstrate proper operation of forward high water alarm.

Demonstrate proper operation of audible alarm and manual override of fixed gas fire system.

Label capacity of life float , provide paddles for each life float (2).

Provide visual indication of high level alarms for each compartment (forward, engine, steering)

Provide visual indication of automatic bilge pump for forward compartment.

Label ring buoy with vessel name.

Renew starboard cutless bearing

Stability letter was not posted as required.

Second lifefloat was not in serviceable condition, reduced passenger capacity to 44.

Vessel only had 9 of required 12 approved flares.
