The owner or operator of an OSV shall make the vessel available for inspection by a marine inspector each time an accident occurs, or a defect is discovered that affects the safety of the vessel. Vessel had an allision that cased structural damage to the vessel that requires repairs. Vessel shall return to port for repairs.

The initial inspection of lifesaving appliances and arrangements for certification includes a demonstration of the proper condition and operation of the rescue boat launching appliance at loads ranging from light load to 10 percent overload. Davit was replaced, weight test to be conducted prior to departure.

Exterior emergency lighting was inoperable during dark ship conditions. Troubleshoot and make repairs prior to departure.

No OSV that carries flammable/combustible liquid may have electrical equipment installed within 3-meters of a source of vapor on a weather deck unless the installation is intrinsically safe. The electrical wiring for the gauging equipment at the PORT & STBD methanol transfer areas, within the hazardous zone, had been spliced open and sealed with electrical tape. Replace wiring, as required, and replace corroded/broken metal clamps holding wiring in place on bulkhead prior to carriage of cargo. 46 CFR 129.520; 701c.
