The Coast Guard recorded this vessel and the following in the same
inspection activity — normally a tug and the barges in its tow. A recreational or
unclassed hull in the same activity is held and not published, so it does not appear
here: these lists are a floor, not a complete manifest.
A towing vessel must be maintained and operated so the watertight integrity and stability of the vessel are not compromised. While transiting outbound the C&D canal west of the [name withheld], the vessel ran aground and made contact with the canal bank on the port quarter. Vessel is enroute to MCallister Pier in Philadelphia and waiting for diver's to access damage.
Provide a damage survey attesting to the watertight integrity and stability of the vessel prior to vessel's movement to the satisfaction of the vessel's Third Party Organization (TPO).

The grab lines for the lifebuoys for the port and starboard bridge wings are wasted and require replacement.

Resolution: Per ABS report no. 6151687, the grab lines were replaced on the lifebuoys on the port and starboard bridge wings.
The master must ensure that all hatches, doors, and other openings designed to be watertight or weathertight function properly.
Weathertight door at aft of pilothouse is wasted/holed on the inner core/side at several points.
Update 11/30/23: The deficiency due date has been extended to the next load line renewal/dry dock, on 10/26/24.

Resolution: Per ABS report no. 6151687, ABS has accepted the deficiency and raised statutory finding no. 62 for this item. In agreement with CG, ABS has extended the due date for replacing the door until the next ILLC renewal/dry dock, which is due on 10/26/24.

The subject vessel is certificated for an oceans route. Both GPS units at forward of main operating station appear affected by GPS rollover and show a date of 17 March 2004.

Resolution: Per ABS report no. 6151687, the primary GPS unit was replaced with a new unit. It was determined that the secondary unit mentioned on the 835 was a repeater of the primary unit, so it now displays the correct date/time as well.

Resolution: The diesel fuel canisters were relocated to the designated storage cabinet prior to the completion of the COI inspection.
At least 30 days prior to the expiration of the Certificate of Inspection, a vessel's owner, charterer, managing operator, agent, master, or individual in charge shall notify the Coast Guard if the vessel will be required to be reinspected for certification or will be operated in such a manner as to not require a Certificate of Inspection. The notification required by paragraph (a) shall be in writing.
The subject's vessel's COI expired on Sunday 10/15/23. Initial written notification regarding the need for the COI renewal exam was received via email by Sector Maryland-NCR at 1531 local time on Friday 10/13.
Provide a corrective action plan, to the satisfaction of the TPO that issued the TSMS certificate (ABS) and the CG, that addresses how the Company will ensure compliance with COI expiry dates moving forward.
