(a) Inspection and testing. All portable fire extinguishers, semi-portable fire-extinguishing systems, fire-detection systems, and fixed fire-extinguishing systems, including ventilation, machinery shutdowns, and fixed fire-extinguishing system pressure-operated dampers on board the vessel, must be inspected or tested at least once every 12 months... All portable fire extinguishers and semi-portable CO2 in engine-room have not been inspected within the last year. 46 CFR 142.240(a)

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. The following alarms must be provided: (3) Auxiliary generator engine low lubricating oil pressure;(4) Auxiliary generator engine high cooling water temperature. Vessel did not have visible or audible alarms for auxiliary generators. 46 CFR 143.230(a)(3)&(4)

Each towing vessel operating on oceans, coastwise, limited coastwise, or beyond 3 nautical miles from shore upon the Great Lakes must carry a Category 1, 406 MHz satellite Emergency Position Indicating Radio Beacon (EPIRB) that meets the requirements of 47 CFR part 80.Vessel required EPIRB on board for Coastwise route. 46 CFR 141.380(a)

Immersion suits carried on towing vessels must meet the requirements of § 199.70(c). Each immersion suit or anti-exposure suit must be marked in such a way as to identify the person or vessel to which it belongs. Immersion suits did not have name of vessel marked. 46 CFR 141.350(b), 46 CFR 199.70(c)(3)

Each towing vessel must perform the applicable tests in Table 143.245(b) of this section. The tests required by this section must be recorded in accordance with part 140 of this subchapter. Vessel did not have records of testing pressure relief valve for air compressor receiver. 46 CFR 143.245(b)

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Vessel did not have service records for boiler. 46 CFR 143.205

A minimum capacity of at least 300 LPM (80 gpm) at a discharge gauge pressure of not less than 414 kPa (60 psi), measured at the pump discharge. Portable fire pump did not have a discharge pressure gauge.

Each remote valve control must be marked in clearly legible letters, at least 25 millimeters (1 inch) high, indicating the purpose of the valve and the way to operate it. Emergency fuel shut off was not labeled with directions of how to operate. 46 CFR 143.260(e)

"A skiff used as a survival craft does not require Coast Guard approval but must:(f) Be marked in accordance with §§ 199.176 and 199.178 of this chapter." Skiff needs to be marked with the name and hailing port, the number of persons for which the boast is equipped, and The Type II retro-reflective material approved under approval series 164.018 must be placed on the boat to meet the arrangement requirements in IMO Resolution A.658(16). Marking required in 14 days.

All crewmembers must be provided with health and safety information and training. Health and safety plan missing numerous items required by 46 CFR 140.510. Annual refresher training required by 46 CFR 140.515(d) is not recorded. Rectify within 14 days.

All towing vessels must be equipped with an industrial type first aid kit that is appropriate to the vessel’s operational conditions and crew size. First aid kit items were expired. Replace within 30 days.

The following list of items must be recorded in the TVR, official logbook, or in accordance with the TSMS applicable to the vessel: (1) Personnel records, in accordance with § 140.400; (2) Safety orientation, in accordance with § 140.410; (3) Record of drills and instruction, in accordance with § 140.420; (4) Examinations and tests, in accordance with § 140.615; (5) Operative navigational safety equipment, in accordance with § 140.620; (6) Navigation assessment, in accordance with § 140.635; (7) Navigation safety training, in accordance with § 140.645; (8) Oil residue discharges and disposals, in accordance with § 140.655; (9) Record of inspection of towing gear, in accordance with § 140.820; and (10) Fire-detection and fixed fire-extinguishing, in accordance with § 142.240. Examinations and tests of vessel equipment and safety systems not being recorded. Rectify by next annual inspection.

"All pressure vessels must have the MAWP indicated by a stamp, nameplate, or other means visible to the crew." MAWP on air compressor not visible on tank. Rectify within 14 days.

Emergency lighting must be provided for all internal crew working and living areas. Emergency lighting sources must provide for sufficient illumination under emergency conditions to facilitate egress from each space. Re-install phosphorescent tape on egress routes within 14 days.

Vessel needs to label all water tight hatches watertight.

Vsl needs to send in proof of DAPI enrollment.

A towing vessel subject to this subchapter and choosing the Coast Guard inspection option, or required to have the Coast Guard inspection option, must undergo an annual inspection within 3 months before or after the COI anniversary date. vessel did not recieve annual inspection in 2022.

ALL SEALS AND GASKETS MUST BE PROPERLY MAINTAINED TO PREVENT LEAKS OF FLAMMABLE/COMBUSTIBLE LIQUID. THERE WAS A LEAK IN THE VICINITY OF THE SHAFT BETWEEN THE PRIME MOVER AND GENERATOR ON THE STARBOARD SIDE GENERATOR. GENERATOR NOT TO BE UTILIZED UNTIL THE SOURCE OF THE LEAK IS DISCOVERED AND THE CAUSE OF THE LEAK REPAIRED. 46CFR143.220(a)

FIRE DETECTORS, CONTROL PANELS, REMOTE INDICATORS, AND FIRE ALARMS SHALL BE APPROVED UNDER APPROVAL SERIES 161.002. THE HILLER SAFE FIRE/BILGE FLOODING ALARM DETECTION PANEL/SYSTEM DOES NOT MEET THIS STANDARD AND IS NOT AUTHORIZED FOR USE ON THIS VESSEL. PROVIDE A PLAN TO REPLACE THE PANEL WITH AN APPROVED PANEL. 46CFR142.330(a)

ELECTRICAL SYSTEMS AND EQUIPMENT MUST FUNCTION PROPERLY AND MINIMIZE SYSTEM FAILURES AND FIRE AND SHOCK HAZARDS. THE VESSELS GROUND DETECTION PANEL WAS REGISTERING A 4.75 AMP GROUND. 46CFR143.400(a) 19NOV2020: RECEIVED AN EMAIL AND PHOTO FROM THE OWNER, STATING THAT A J-BOX WAS FOUND DEFECTIVE AND HAS BEEN REPLACED AND SHOWING 0 AMPS OF GROUND ON THE DETECTOR PANEL

VESSELS OPERATING ON A COASTWISE ROUTE, LESS THAN THREE MILES FROM SHORE SHALL HAVE 3-DAY AND 3-NIGHT DISTRESS SIGNALS. THE VESSEL VALID ONLY HAD 2-NIGHT AND 1-DAY SIGNAL ON BOARD. 46CFR141.370

A PORTABLE FIRE PUMP SHALL HAVE A PRESSURE GAUGE AT THE DISCHARGE OUTLET. THE PORTABLE FIRE PUMP DID NOT HAVE A DISCHARGE GAUGE. 46CFR142.325(e)(1)

EACH TOWING VESSEL SHALL HAVE A SAFETY PROGRAM FOR THE RECOVERY OF A DERELICT BARGE. THERE WAS NO EVIDENCE OF THE STATED PROGRAM OR TRAINING/DRILLS ASSOCIATED WITH IT. 33CFR155.230(b)

EVERY TOWING VESSEL OF GREATER THAN 26-FEET IN LENGTH MUST HAVE A RADIOTELEPHONE IN COMPLAINCE WITH THE BRIDGE TO BRIDGE ACT. THE VESSEL DID NOT HAVE A VALID BRIDGE TO BRIDGE OR RADIOTELEPHONE CERTIFICATE. 33CFR26.03

A VESSEL OF 40-FT. OR GREATER SHALL HAVE A GARBAGE MANAGEMENT PLAN. THE VESSEL DID NOT HAVE A GARBAGE MANAGEMENT PLAN ON BOARD. 33CFR151.51(d)
