A ship engaged in the carriage of dangerous goods in any cargo spaces shall be provided with a fixed carbon dioxide or inert gas fire-extinguishing system complying with the provisions of the Fire Safety Systems Code. Four feeder lines for the vessel's CO2 system serving the cargo holds were found to be fractured. CITE: 74SOLAS(2009) II-2/10.7.2

The vessel's cooling water pump caught on fire and was damaged. Replace/repair damaged pump and determine the cause of the casualty to prevent its recurrence to the satisfaction of RO and Coast Guard. Vessel shall have two assist tugs while maneuvering in port until item is rectified. 74 SOLAS (14) I/11.(a)

Local indicators for SD-P08 and SD-P11 did not operate. Bridge indicator constantly flickers on half the indicators and dims when multiple doors are opened.
