The launching mechanism shall be so arranged that it may be actuated by one person from a position on the ship's deck and, except for secondary launching appliances for free-fall lifeboats, from a position within the survival craft or rescue boat. While lowering the port lifeboat, the crew utilized the davit remote control wire inside the lifeboat. The remote control stopped approximately 15-20 feet above the water which prevented the crew to lower the lifeboat to the water. Due to the vessel’s mooring condition, USCG was unable to verify operation of stbd lifeboat's davit remote control. Cite: SOLAS III/Reg 16.1 LSA Code 6.1.2.2

Posters or signs shall be provided on or in the vicinity of survival craft and their launching controls shall be easily seen under emergency lighting conditions Launching instructions for liferafts were not posted under an emergency light. Cite: SOLAS (2009) III/Reg 9.2.2

Electrical installations shall be such that the safety of crew and ship from electrical hazards will be ensured. Identified the following improper electrical installations: • Improper electrical connection (hole drilled in outlet and wire run through it with daisy chain to radio charger). • Improper electrical connection and wiring to stbd light for accommodation ladder. • 02 light diffusers were cracked in EDG room. Each individual referred to in paragraph (a) of this section must hold an MMC that serves as identification, with an appropriate endorsement for the position in which the seaman serves, and the MMC, along with a valid medical certificate, must be presented to the master of the vessel at the time of employment or before signing Articles of Agreement. Two self-closing fire doors were found propped open: (1) the A-0 self-closing door from accommodations to the wheelhouse; (2) the A-0 self-closing door to the steering gear room. Periodic safety test must be conducted at periodic intervals specified by the Coast Guard to confirm vital systems and safety features continue to operate in safe, reliable manner. The PSTPs were missing the approval letter, The crew was testing to an unapproved revision, and the crew was unfamiliar with properly recording the PSTPs to ensure annual completion. The Oil Record Book (ORB) Part I shall be completed on each occasion, on a tank by tank basis if appropriate, whenever any of the following machinery space operations take place in the ship: Collection and disposal of oil residues. There were no ORB entries for weekly soundings completed from 10 Nov 2023 through 08 Dec 2023. The Chief Engineer reported he did not make entries due to tank cleaning while in drydock. Except as provided by 15.725 of this part (sailing short), no vessel may be navigated unless it has in its service and onboard the crew complement required by the COI. Two Able Seamen (or 2 Ordinary Seaman with RFPNW) were missing from the provide crew list/COI required crew complement. Cite: 46 CFR 15.515(a)

The company should ensure that each ship is manned with qualified, certificated and medically-fit seafarers in accordance with national and international requirements. During a USCG Certificate of Inspection, the ship was missing two Able Seaman from its crew compliment. Also, two crewmembers did not have their medical waivers on board, as required by the medical certificate and medical waiver. Reviewed the SMS Form “Sign-On and Sign-Off Form” and noted no pre-screening or reference to medical waivers, only medications. Any main and auxiliary steering gear control system operable from the navigation bridge shall comply with the following: in the event of a failure of electrical power supply to the control system, an audible and visual alarm shall be given on the navigation bridge. All alarms were not relaying to the bridge for any of the pumps. Rectify issue prior to sailing from next foreign port (Hamburg, Germany 18JAN23).

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Port and starboard lifeboats both had large indents on both sides of each hull. Provide acceptable proof that both lifeboats are fit for service.

Engines installed on vessels that were constructed on or after January 1, 2000, with power output of more than 130 kW must be covered by a valid EIAPP certificate, certifying the engine meets the applicable emission standards of Annex MARPOL Annex VI. Provide EIAPP Certificates for all applicable marine diesel engines as proof of emissions compliance with the IAPP. 40 CFR 1043.30

All ships of 300 gross tonnage and upwards shall be fitted with an echo sounding device, or other electronic means, to measure and dispay the available depth of water. Vessel's echo sounding device is currently inoperable. Prove proper operations of echo sounding device prior to departure for Savannah, GA. 74 SOLAS Cons (2020) V/19.2.3.1

Engines installed on vessels that were constructed on or after January 1, 2000, with power output of more than 130 kW must be covered by a valid EIAPP certificate, certifying the engine meets the applicable emission standards of Annex MARPOL Annex VI. Provide EIAPP Certificates for all applicable marine diesel engines as proof of emissions compliance with the IAPP. 40 CFR 1043.30

In a space described in 113.25-9(a), where the general emergency alarm signal cannot be heard over the background noise, there must be a red flashing light or rotating beacon, in addition to the general emergency alarm signal. General alarm lights are not working in the bosun's stores or bow thruster space. Vessel submitted risk mitigation plan to decrease personnel access to affected spaces during routine operations.
