Electrical equipment on a vessel must be installed and maintained to protect the vessel from electrical hazards. During dry dock, MI's observed significant electrolysis on starboard shaft and both propellers. Determine source of electrolysis and affect permanent repairs. Submit documentation to USCG at [email withheld].

Where flexible nonmetallic hose is permitted for use in piping systems by this section, it must meet SAE J-1942 (incorporated by reference; see 46 CFR 175.600) or be specifically approved by the Commandant. Fuel hoses installed do not meet the standard. Replace all non compliant hoses with hoses meeting SAE J-1942 for fuel.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date. Vessel could not provide charts. Provide charts to the satisfaction of USCG MI.

The emergency lighting required by paragraph (a) of this section must automatically actuate upon failure of the main lighting system. The pilothouse emergency lighting was inoperable. Repair/replace emergency lighting to the satisfaction of USCG MI.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Vessels medical kit did not contain all required equipment, specifically missing burn cream. Ensure medical kit meets requirements in 46 CFR 199.175(b)(10).

Vessel control systems and other miscellaneous systems and equipment required by this part must be suitable for the purposes intended. The port generator heat exchanger had a leak, which may have led to a minor electrical casualty. Vessel may not get underway until satisfactory repairs have been completed to the satisfaction of the attending marine inspector.

46 CFR 182.455 (b) (4) The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. MI's observed that fuel shutoff valves were not able to be operated from outside the space. Vessel Shall enact repairs to the satisfaction of attending USCG Marine Inspector.

46 CFR 182.500 (a) A vessel must be provided with a satisfactory arrangement for draining any watertight compartment. MI's observed that bilge pumps in Port/Starboard auxiliary spaces were inoperable. Vessel shall enact permanent repairs to inoperable pumps, and verify operation of six pumps that were not tested to the satisfaction of attending USCG Marine Inspector.

46 CFR 185.364 On vessels described by 46 CFR 175.110(c), flammable items not otherwise covered by the regulations of this sub chapter, such as rechargeable batteries, including lithium ion batteries utilized for commercial purposes, must be handled, stored, and operated in a way that mitigates the risk of hazardous conditions. Vessel does not have designated charging station for li-ion batteries nor instructions for passenger and crew storage, charging and use of li-on powered devices onboard. Provide designated charging station in regularly occupied space and ensure crew and passengers are instructed on appropriate safety measures to avoid hazardous conditions. Action code 705: Vessel is restricted to day trips only on Lakes, Bays, and Sounds route until deficiency is rectified.

46 CFR 181.450 (a) (2) An interconnected detection and alarm system must: Be installed such that the actuation of alarm in one area results in both audible and visual alarms in all areas required by 46 CFR 181.405(c) or 118.400(d) to be protected by the interconnected detection and alarm system. While testing system, Marine Inspectors were unable to witness activation of alarm system in the machinery space via test of smoke detector in topside locations. Ensure system is interconnected to alarm in all spaces when activated. Action code 705: Vessel is restricted to day trips only on Lakes, Bays, and Sounds route until deficiency is rectified.

Incorrect stowage of flammable materials. Aerosols, paint, etc., stowed in interior forward closet.

Life jackets stowed in location where not readily accessible.

Life jacket stowage area requires proper marking.

The owner or managing operator of a vessel shall ensure that the vessel complies with the laws and regulations applicable to the vessel and that the vessel is otherwise satisfactory for the intended service, including an inspection of the following: (3) Certificates and operating manuals, including certificates issued by the FCC. The vessel did not have a valid Radio Safety Certificate onboard.

All escape hatches and other emergency exits used as a means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: 'EMERGENCY EXIT, KEEP CLEAR", unless such markings are deemed unnecessary by the cognizant [name withheld] door, hatch, or scuttle, used as a means of escape must be capable of being opened by one person, from either side. The means of escape from the passenger berthing spaces is located in the aft head, with no indications of the emergency exit in that space. The means of escape is through the aft head, which is capable of being locked, preventing access to the emergency exit. Action 705: Correct prior to overnight voyages.

Doors on a cooking appliance must be provided with hinges and locking devices to prevent accidental opening in heavy seas. Sea rails, with suitable barriers to prevent accidental movement of cooking pots, must be installed on a cooking range. The oven door does not have a means of being closed and locked. The stove does not have sea rails to secure pots while being used. Action 705: Correct prior to use underway.

A space which is so open to the atmosphere that a fixed gas fire extinguishing system would be ineffective as determined by the cognizant OCMI, is not required to have a fixed gas fire extinguishing system. Vessel had a 5 gallon gas can full of gasoline stowed in a container which did not have a fixed extinguisher installed or contain any venting. Recommend stowing the gas can on deck in an open space.

The frame or support for each portable fire extinguisher permitted by paragraph (a) of this section must be welded or otherwise permanently attached to a bulkhead or deck. The two (02) engine room fire extinguishers stowed on the aft deck for the engine rooms were not permanently secured to the bulkheads within their mount.

Table 181.500(b) of this section indicated the minimum required classification for each space listed. Extinguishers with larger numerical ratings or multiple letter designations may be used if the extinguishers meet the requirements of the table. Based on Table 181.500(b) the vessel is required to have a 2-A fire extinguisher in the accommodation space outside of the galley. Current fire extinguisher meets the requirements for the accommodation space OR the galley but not both. Action 705: Vessel is restricted from using the galley until an additional fire extinguisher is installed in accordance with Table 181.500(b).

Whenever damage to hull plating or structural members that may affect seaworthiness of a VSL. is discovered, the OCMI may conduct an ISE in any affected space and may require the VSL. to be dry docked/taken out of service to assess the extent of damage. VSL. shall transit during daylight hours only on the ICW to be hauled out at Pier 77 in Galv., TX. VSL. shall effect permanent repairs to all damaged areas to the satisfaction of the USCG MI.
