During inspection of the CO2 system in was noted that two CO2 bottle flex hose connections were left loose possibly allowing CO2 to collect in the storage room upon activation of the system; creating a dangerous condition to crew members. Insure fixed CO2 system is properly inspected and tested. Third party inspector to return inspect system to ensure it is properly installed and functioning.

Piping subject to internal pressure from oil in the tank must be fitted with a positive shutoff valves. The valve shall be provided with a means of emergency manual operation regardless of the status of the power operating system. During this inspection it was noted that the hydraulic oil reservoir sight gauge to the quick closing valves indicated empty. Additionally, the weekly testing of these quick closing valves had not been completed. Test and prove proper operation of each quick closing valve in the presence of a marine inspector or class surveyor.

Vessel struck its dredge head tearing a hole in the bottom plate in way of the starboard cofferdam located between frames 115 and 135. Temporary cement coffer damn repair is satisfactory until permanent repairs can be completed. Permanent repairs shall be completed within 60 days. Vessel shall complete daily soundings and weekly inspections of repair and log them accordingly. Vessel shall report any abnormalities to ABS and USCG. Additionally, complete an ABS survey of the void space once every three months beginning September 2019 to verify fitness for service with temporary repairs and provide a report to the local USCG [name withheld] shall be completed at the next dry docking, but no later than 30 September 2020. ***Note: this requirement supersedes the CG-835 issued on 5/28/2019.***
