The master, owner, or operator of a vessel equipped with ballast water tanks and operates in waters of the US must follow a ballast water management plan. MIs noted the BWTS was not operational at the time of inspection. Make repairs the the treatment system before ballasting operations are conducted. 46 CFR 151.2050

Electrical installations on vessels must ensure: protection of crew, other persons, and the vessel from electrical hazards and system integrity through compliance with the applicable system requirements (IEEE, NEC, IEC, etc) to which plan review has been approved. MI noted a portable AC unit, in the port side main deck store space, was being used in a permanent way utilizing an extension cord for power. Plan review has not been conducted for the installation. Remove the unit and extension cord and conduct required plan review for approved installation. 46 CFR 111.01-1

MI was notified of repairs and/or alterations being conducted on vessel. No repairs of alterations affecting the safety of the vessel with regard to the hull, machinery or equipment shall be made without the knowledge of the [name withheld] and subsequent required testing shall be approved by the OCMI prior to departure.

The installation of all systems of a marine engineering nature, together with the details of dsign, construction, and installation, shall be in accordance with the req.s of subchapter "F". Vessel suffered loss of prop[ulsion of the port main engine (due to stern tube leaking), Vessel required to conduct repairs.

Fire detection systems, if installed, must meet the applicable requirements of 46 CFR 76. A fire detection and alarm system must automatically reset to a normal operating condition after alarm and fault situations are cleared. Fire detection panel located on the bridge is registering a fault that is unable to be cleared. 46 CFR 95.05-1(a) 46 CFR 76.27-10(c)

It shall be the duty of the owner, master, or person in charge to require and have performed at least once in every twelve months the tests and inspections of all fixed fire extinguishing systems on board, as described in § 91.25–20. All fire hoses must be subjected to a test pressure equivalent to the maximum pressure to which they may be subjected in service, but not less than 100 p.s.i.. Fire hoses had not been tested to the appropriate test pressure in the past twelve months. 46 CFR 97.15-60 46 CFR 91.25-20(a)(4)

The installation of all lifesaving appliances and arrangements must be in accordance with Subchapter W. Radio lifesaving appliance installations and arrangements must meet the requirements of 47 CFR 80. At least three two-way VHF radiotelephone apparatus must be provided on every cargo ship of 500 tons gross tonnage and upwards. Two-way VHF radiotelephone apparatuses must conform to performance standards as specified in § 80.1101. The equipment specified in this subpart must also conform to the appropriate performance standards listed: (7) Two-Way VHF radiotelephone: (i) IMO Resolution A.809(19), as revised by IMO Resolution MSC.149(77). Vessel was not equipped with dedicated, sealed primary batteries for the required three two-way VHF radios as mandated by IMO Resolution MSC.149(77) 12.2. 46 CFR 96.06-1 46 CFR 199.60(a) 47 CFR 80.1095(a) 47 CFR 80.1101(a)

Raw water overboard piping has a pin hole leak in Void #5, port side. Make temporary repairs using ABS approved Belzona pipe wrap until permanent repairs can be made to the satisfaction of the attending USCG Marine Inspector at the next scheduled drydock in September of 2022. Ensure periodic checks of the space are made to ensure temporary repair is maintaining watertightness and report any changes to nearest USCG Marine Safety Detachment/Sector Inspection Division.

A gyro compass heading repeater, or other means, to supply heading information visually at the emergency steering position. The gyro repeater was not displaying the correct heading. Provide tech report.

A schematic diagram of the CO2 system shall be provided at the pull station if the CO2 cylinders are not within the protected space. Provide a schematic drawing for both the E-Gen and paint locker CO2 system at each pull station.

At least half of the lifebouys shall be fitted with a self-igniting light. All batteries were found expired. Replace expired batteries.

Whenever there is a hazardous condition on board a vessel or caused by a vessel or its operation, the owner, agent, master, operator, or person in charge must immediatley notify the nearest Coast Guard Sector Office. Vessel reported port main engine malfuntion. One Tug of adequate horsepower shall escort the vessel inbound.

No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. Vessel must make repairs to satisfaction of OCMI prior to departure.

Vessel reported gearbox malfunction. Propulsion machinery including main engines, reduction gear, shafting, and bearings must be maintained to ensure proper operation. Vessel shall provide satisfactory proof of repair prior to departure.

An inspection shall be made whenever any important repairs or alterations to the vessel are undertaken. Vessel must receive an in-service inspection from the USCG prior to its departure from the MARRS Shipyard facility.

At least two lifebuoys, each with attached self-activating smoke signals, must be stowed where they can be quickly released from the navigating bridge and should, when released, fall directly into the water without striking any part of the vessel. MIs observed the lifebuoys on the bridge-wings were not able to be quickly released and fall directly into the water without striking any part of the vessel. Make arrangements to permit the quick release of the bridge lifebuoys into the water without striking any part of the vessel.

Markings shall be provided as necessary for the guidance of persons onboard in the event of an emergency. MIs observed the total flood water mist activation station on bridge was incorrectly labeled "FIRE ALARM". Remove ambiguous labeling from the total flood water mist activation station.

Over heating bearing on the STBD reduction gear.

Replace the STBD Main Pillow Block temperature sensor. Conduct operational testing to the satisfaction of the attending Coast Guard Marine Inspector. When the vessel is underway the crew shall conduct Engine Room rounds at 30 min intervals to monitor the temperature of the STBD Main Pillow Block.

Prove proper operation of the emergency clutch out system to stop main propulsion shaft rotation on both Port and STBD Shafts. The vessel is required to demonstrate Procedure No. 4 from Section 3 of the Periodic Safety Test Procedures (PSTP) to the satisfaction of the attending Coast Guard Marine Inspector.
