When table 151.05 refers to this section, the following apply: Gravity type cargo tanks shall be fitted with an approved pressure-vacuum relief valve of not less than 21/2-inch size, which shall be set at a pressure of not less than 3 pounds per square inch gauge, but not in excess of the design pressure of the tank. Subject barge had one Tankteck (KLPH-6) pressure vacuum valve installed with P/V setting at +1.5 and -0.5. Adjust P/V setting to meet 46CFR151.50-5(e) or request the cargo requiring this setting be removed from the approved CAA. Barge is restricted from carrying these cargoes until the operator can prove proper setting of the P/V or have said cargoes removed from the CAA.

Vessel was involved in a bridge allision with the 4th Street Bridge when transiting the [name withheld]. Starboard arm of drawbridge came down and caused damage to the king post and the starboard pump engine. Repairs shall be made to the king post and the starboard pump engine to the satisfaction of the attending marine inspector prior to the vessel conducting cargo operations. 46 CFR 31.10-25

Prove proper operation of the emergency shut downs on the starboard cargo engine. Engine not installed at time of inspection.

Install guards around engine pulleys.

Prior to operation additional control loading arm must be testing under Coast Guard oversight.
