Each towing vessel must be maintained and operated so as to minimize fire hazards and to ensure that all bilges and void spaces are kept free from accumulation of flammable materials and liquids. Fuel leaks were discovered in the following areas: Piping to fuel filter on port generator, Fuel piping on forward side of port generator (copper tubing), Fuel injector on cylinder head of port generator, Port main engine under fuel filter, starboard generator piping near fuel filter, and supply valves from fuel tank into engine room. Also, significant oil leaks were discovered on the port main engine water pump and oil soaking was discovered on the lagging of the starboard generator exhaust. Repair all leaks and replace oil-soaked lagging.
