Barge was wired with means to provide electrical power via portable cable to other vessels. Cable lacks a safety ground and posses a electrical shock risk to operating personnel. (3) wire vice (4).
Also, barge was not provided with ground detection indicators as required, and the overcurrent protection devices may exceed the allowed 115%.
System must be tagged out to prevent injury to personnel and electrical plans must be revised, submitted and approved by the Coast Guard Marine Safety Center.
This arrangement was a known issue and must be resolved to the satisfaction of an attending marine inspector fo this vessel and all sister barges operated bt Apex Oil.

Vessel has not completed annual/periodic inspection inside of it's COI anniversary date (29MAY2020) window. Vessel must perform COI/annual/periodic inspection to the satisfaction of local OCMI prior to any cargo operations or vessel movement.
