When a distribution system, whether primary or secondary for power, heating or lighting, with no connection to earth is used, a device capable of continuously monitoring the insulation level to earth and giving an audible or visual indication of abnormally low insulation values shall be provided. MI identified a ground fault on a 220V system. 74 SOLAS (2020) II-1 / 45.4.2

Rescue boats shall have sufficient mobility and maneuverability in a seaway to enable persons to be retrieved from the water, marshal liferafts and tow the largest liferaft carried on the ship when loaded with its full complement of persons and equipment or its equivalent at a speed of at least 2 knots. Rescue boat helm is loose and cover for breaker panel under helm is missing and retro-reflective tape needs to be replaced. SOLAS(14)III/34, LSA Code 5.1.1.7

No electrical equipment shall be installed in any space where flammable mixtures are liable to collect, e.g. in compartments assigned principally to accumulator batteries, in paint lockers, acetylene stores or similar spaces, unless the Administration is satisfied that such equipment is: appropriately certified for safe usage in the dusts, vapours or gases likely to be encountered. Multiple explosion proof light fixtures in Cargo hold #1 are missing bolts that secure fixture and provide proper MEXG. Explosion proof light fixture outside paint locker missing gasket on tubular end connection. DNV to verify all explosion proof lighting in cargo hold areas. SOLAS(14) II-1/45.10.4

An immersion suit of an appropriate size complying with the requirements of section 2.3 of the Code shall be provided for every person on board the ship. 02 NON-SOLAS approved immersion suits were on the vessel and in use. SOLAS(14) III/32.3.2

Maintenance, testing, and inspections of life-saving appliances shall be carried out in a manner having due regard to ensuring reliability of such appliances. Rescue boat sling was found to have broken strands in the wire eye. Additionally, the sling has not been replaced since new in 2014. Repair or replace to satisfaction of RO. 74 SOLAS (20) III/20.3.1

The manning requirements for a particular vessel are determined by the OCMI after consideration of the applicable laws, the regulations in this part, and all other factors involved, such as: emergency situations, size and type of vessel, installed equipment, proposed routes of operation including frequency of port calls, cargo carried, type of service in which employed, degree of automation, use of labor saving devices, and the organizational structure of the vessel. Vessel shall prepare and submit a safe manning proposal due to a change in management of the vessel. Crew interviews, significant overdue maintenance tasks, STCW work/rest hour violations and failure of labor saving devices provided objective evidence that the current minimum crew compliment was insufficient to accomplish routine operations and was likely to lead to crew fatigue.

Tests must be conducted at periodic intervals specified by the Coast Guard to confirm that vital systems and safety features continue to operate in a safe, reliable manner. During inspection, vessel could not provide documentation showing that Automation Test Procedures were completed for 2021 and 2022. Furthermore, these test procedures are required to ensure the safety of the automated vital systems are equivalent to that of systems under direct manual operator supervision consistent with a Periodically Unattended Machinery Space endorsement.

The operator of a vessel with a capacity of 250 or more barrels of oil, shall provide transfer procedures that meet the requirements of this part and part 156 of this chapter for transferring to or from the vessel; and from tank to tank within the vessel. The vessel was not able to provide written transfer procedures containing the required contents as listed in 33 CFR 155.750.

The date when musters are held, details of abandon ship drills and fire drills, drills of other life-saving appliances and on board training shall be recorded in such logbook as may be prescribed by the Administration. Drill records included dates and participants but were missing details describing the drills. 74 SOLAS (14) III/19.5

All automatically controlled or remotely controlled or monitored vital systems addressed by part 62 of this subchapter must be subjected to tests and inspections to evaluate the operation and reliability of controls, alarms, safety features, and interlocks. Test procedures must be submitted to the Coast Guard for approval. Vessel has not submitted PSTPs to MSC for approval. The agreed upon date was at drydock 2020 as per the “Gap Evaluation For MSP Transfer to U.S. Registry” dated July 29, 2015.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Numerous weatherdeck lighting mounts and supports on the main deck were found wasted. Repair all wasted mounts/supports to the satisfaction of attending surveyor. 74 SOLAS (20) I/11(a)

The engine shall be provided with either a manual starting system, or a power starting system with two independent rechargeable energy sources. Stern launch lifeboat was not able to start on secondary battery. Crew replaced battery and satisfactorily tested. 74 SOLAS (20) III/34 LSA 4.4.6.2

The Company should establish procedures, plans and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of the personnel, ship and protection of the environment. Vessel has approximately 10 inaccurate tank level indicators associated with the ballast water system. Vessel did not have a written procedure in place that clearly identifies inaccurate indicators that could also be passed to oncoming crewmembers responsible for the ballast water system. Crew created document capturing details for inaccurate ballast tank level indicators. 74 SOLAS (20) IX/3.1 ISM Part A/7

74 SOLAS (2014 Cons) Chap. III Reg. 34 LSA Code 1.2.2.7 All life-saving appliances and arrangements must comply with the applicable requirements of the LSA Code. Life-saving appliances must be fitted with retro-reflective material where it will assist in detection and in accordance with the recommendations of the organization. The retro-reflective material on the the lifebuoys, lifeboat, and rescue boat are deteriorated. Place retro-reflective materila on lifebuoys, lifeboat, and rescue boat in accordance with resolution A.658(16) to the satisfaction of the classification society.

Electrical wire attached to the fan in the garbage room was discovered with three daisy chains creating unsafe electrical hazards. Correctly install fan to the satisfaction of the classification society.

In the event of alterations involving the safety of the vessel, the applicable plans shall be submitted for approval covering the proposed work. A fixed fire water cooling system was installed in holds 1 & 2 without USCG plan approval of knowledge of the OCMI. Vessel shall be limited to the cargo's specified on the current Document of Compliance For The Carriage of Dangerous Goods, dated 05/28/20 until sprinkler discharge rate and drainage requirements can be proven.

Fire protection and fire fighting systems and appliances shall be maintained and ready for use. The water mist line above the #2 SSDG had a clogged nozzle. Repair and demonstrate proper operation to the satisfaction of the attending surveyor. 74SOLAS(14), II-2/14.2.1.1.1

Structural fire protection including fire-resisting divisions and protection of opening and penetrations in these divisions shall be kept in good order so as to ensure their required performance. 05 engine room ventilation dampers were found with gaps between 1"-6" where DNV-GL rules require "metal-to-metal" contact. 74SOLAS(14), II-2/14.2.1.1.1

The emergency bilge suction was seized closed. 74SOLAS(14), I/11

Automatic starting arrangements, operational, and control systems shall include provisions for manually overriding the automatic controls. Failure of any part of such systems shall not prevent the use of manual override. The boiler controls at the PLC cabinet on the boiler flat caused the system to reboot after switching to automatic from manual, preventing operational and testing of the boiler automation for at least 30 minutes. Have a manufacturer's technician or authorized technician repair the boiler automation to the satisfaction of the attending surveyor and complete all boiler automation testing to the satisfaction of the attending surveyor. 74SOLAS(14), II-1/31.4

Machinery spaces of category A shall be adequately ventilated so as to ensure that when machinery therein are operating at full power in all weather conditions, an adequate supply of air is maintained to the spaces for the safety and comfort of personnel and the operation of the machinery. The PLC cabinet, starboard side, above the SSDGs overheats in hot ambient conditions and needs the cabinet doors open with a fan blowing in to keep the equipment operating. Rectify and demonstrate proper operation to the satisfaction of the attending surveyor. 74SOLAS(14), II-1/35

Doors required to be self-closing shall not be fitted with hold back hooks. Self-closing doors leading into the galley are fitted with hold back devices. 74SOLAS(14), II-2/9.4.2.2

Terminations and joints in all conductors shall be made as to retrain the orginal electrical, mechanical, flame retarding and where necessary, fire resisting properties of the cable. Emergency power battery room has power cable leading to/in way of the batteries that has an exposed end. 74SOLAS(14), II-1/45.5.6

Each emergency generating set arranged to be started automatically shall be equipped with starting devices approved by the administration with a stored energy energy capability of at least three consecutive starts. A second source of energy shall be provided. Secondary means of starting did not work.

Auxiliary boiler has a cracked lower tube sheet. Provide tech report detailing the completed repairs and the auxiliary boiler's fitness for service to USCG.

Wire rope must be replaced at next available port.

Vsl notified Sector NC (Jan.13) of the following deficiency which occurred at sea during transit. Failure of the rescue boat engine raw water pump. Make appropriate repairs and submit class report to marine inspector. Provide vessel documentation showing required spare parts for rescue boat engine.

Vessel must meet section 3.2.1.4 of NVIC 01-13, CH-1.

Vessel required to submit before and after photos of repair to fuel gauge line. Fuel was coming out of gauge and dripping onto stator.

Provide LRIT Conformance Test Report.

Provide valid EPA Certificates for all of vessel's engines, etc required. The EIAPP Certificates issued by DNV-GL on behalf of Marshall Islands are sufficient in this interim period.

Ballast Water Treatment System has been inoperable since delivery from the shipyard. Vessel is directed to conduct ballast water exchanges as necessary per the other methods listed in 33 CFR 151.2025(a) until the BWTS is repaired.
