74SOLAS (14) SOLAS IX Reg 3 ISM Reg 10 The company and the ship shall comply with the requirements of the International Safety Management Code. For the purpose of this regulation, the requirements of the Code shall be treated as mandatory. The company should ensure that non-conformities are reported, with possible cause (if known). During testing of the fixed gas detection, uncoupled unions were found in the sampling lines with a wrench in the vicinity. When the unions were tightened, a flow fault was found. The vessel's RO subsequently found that there was a clogged flame arrester in the compressor room. PSCO observed objective evidence in the form of loose unions, flow fault and RO report that a non-conformity had been identified, left not corrected and not reported. 50AC

74SOLAS(14) VII/13.1 IGC Code 13.6.8 A gas carrier shall comply with the requirements of the International Gas Carrier Code...The gas detection equipment should be capable of sampling and analyzing for each sampling head location sequentially at intervals not exceeding 30 minutes...Multiple sampling lines were found to be loose and freely passing air and span gas. After tightening by crew, spaces sampled were showing "flow fault" and unable to sample. 17ac

Electrical enclosures for ESD near booster pump, port and STBD booster pump controls, heater control, 3P cargo tank gauging device, 1P tank gauging device, center and STBD compressor controls all have cable glands that are loose or freely allow cable to be pulled out. There are no inspection proce3dures or schedule of inspection for electrical equipment in hazardous locations. 17ac
