Each ring life buoy on a vessel must:(1) Be approved in accordance with § 160.050 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.150; The two main deck ring buoys were missing the required "Q" spec. Replace with in-kind required equipment.

Complete but simple instructions for the operation of a fixed gas fire extinguishing system must be located in a conspicuous place at or near each pull box and stop valve control and in the space where the extinguishing agent cylinders are stored. If the storage cylinders are separate from the protected space, the instructions must also include a schematic diagram of the system and instructions detailing alternate methods of releasing the extinguishing agent should the local manual release or stop valve controls fail to operate. Each control valve to a distribution line must be marked to indicate the space served. CO2 activation stations were missing operation instructions. Mount instructions by all operating stations.

Each entrance to a space storing carbon dioxide cylinders, a space protected by carbon dioxide systems, or any space into which carbon dioxide might migrate must be conspicuously marked as follows:(1) Spaces storing carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. VENTILATE THE AREA BEFORE ENTERING. A HIGH CONCENTRATION CAN OCCUR IN THIS AREA AND CAN CAUSE SUFFOCATION.” Tank room is missing this verbiage.

An alarm for a fixed gas fire extinguishing system must be clearly and conspicuously marked “WHEN ALARM SOUNDS-VACATE AT ONCE. CARBON DIOXIDE BEING RELEASED”. Engine room is missing this verbiage.

Spanner wrenches must be provided where a 40 millimeter (1.5 inch) diameter fire hose is required. Both fire hose stations are missing spanner wrenches. Provide correct wrenches at stations.

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessel's stability letter, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 25 millimeters (1 inch) numbers and letters at the entranceway to that deck. Label upper deck with appropriate number of passengers.

Watertight doors and watertight hatches must be marked on both sides in clearly legible letters at least 25 millimeters (1 inch) high: “WATERTIGHT DOOR—KEEP CLOSED” or “WATERTIGHT HATCH—KEEP CLOSED”. Label all deck hatches.

A crew accommodation space and a work space must be of sufficient size, adequate construction, and with suitable equipment to provide for the safe operation of the vessel and the protection and accommodation of the crew in a manner practicable for the size, facilities, service, route, speed, and modes of operation of the vessel. Vessel is missing required signage in crew only locations by the pilot house. Install sign on line stating "CREW ONLY" on port and stbd side of pilot house.

(1) Annual inspection. Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date.(i) You must contact the cognizant OCMI to schedule an inspection at a time and place which he or she approves. No written application is required. Vessel went past due on annual inspection. Contact local OCMI to schedule annual inspection.

The Certificate of Inspection must be posted under glass or other suitable transparent material, such that all pages are visible, in a conspicuous place on the vessel where observation by passengers is likely. Vessel has misplaced the original COI and has a copy posted showing only page 1. Once new COI is received, the original must be kept on board and the copy posted must show both pages. 46 CFR 176.302 Code 16 c

A vessel must comply with the applicable requirements for any radio and EPIRB installation including the requirements for a station license and installation certificates to be issued by the FCC as set forth in 47 CFR part 80. Vessel has misplaced FCC license, all other FCC documents were present and had current dates. Contact FCC inspector and get a reprint of the license. 46 CFR 184.502 Code 16 c

When a marine employer determines that a casualty or incident is likely to become an SMI defined by 4.03-2, alcohol testing of each individual must be conducted within 2 hours of the SMI. Vessel owner has an unapproved BAC testing device. Replace with an approved DOT testing device or testing strips. 46 CFR 4.06-3 Code 16 c

Fire Hoses: On a vessel of more than 65' each hose must be lined commercial hose that conforms to UL 19 or hose that is listed and labeled by an independent laboratory recognized by the Commandant as being equivalent in performance, by 50' in length x 1.5" diameter and have fittings of brass or other suitable material. Vessel currently has unlined hoses that are not approved. Replace to the satisfaction of the attending MI. 46 CFR 181.320 (b) Code 16 c

A bilge pipe piercing the collision bulkhead must be fitted with a screw down valve located on the forward side of the collision bulkhead and operable from the weather deck or if it is readily accessible under service conditions a screw down valve without a reach rod may be fitted to the bilge line on the after side of the collision bulkhead. In additions bilge piping must be 1.5" nominal pipe size. Collision bulkhead valve currently has a ball valve with flexible hose piping connection. Replace with a screw down gate valve and all hard pipe connection to main line. 46 CFR 182.510 (d) Code 50 c

A vessel must comply with the applicable requirements for any radio and EPIRB installation including the requirements for a station license and installation certificates to be issued by the FCC as set forth in 47 CFR part 80. Vessels EPIRB HRU was expired. Replaced while MI on board. 46 CFR 184.502 Code 10 c

Suitable fire hoses shall be in place as required. Both fire hoses were found with leaks.

A float free EPIRB shall be provided for vessels operating beyond three miles from shore. The HRU was found expired.

Vessels shall be provided with FCC certificates for the route and installed equipment. Bridge to Bridge and the Safety Radio Certificates were found expired.

The minimum capacity of the fire pump must be 50 gallons per minute at a pressure of 60 PSI. Fire pump gauge would not read pressure above 40 PSI. Demonstrate that the fire pump can maintain 60 PSI to the satisfaction of the attending Marine Inspector.

Each fire hose must conform to UL 19. Two firehoses were found leaking excessively and damaged beyond repair. Replace fire hoses in kind to the satisfaction of the attending Marine Inspector.

Rotating machinery must be properly protected by a cover, guard or rail. The port and stbd shafts are exposed and create a hazard to personnel. Rotating components on the fire pump were also exposed. Install adequate machinery guards to prevent personnel from coming into contact with rotating machinery to the satisfaction of the attending Marine Inspector.

Exhaust gas must not leak from the piping or any connections. 4 exhaust leaks were found in the engine room with unapproved temporary repairs. Properly repair or replace exhaust piping to the satisfaction of the attending Marine Inspector.

Fire hazards to minimized. Buckets of combustible oil were found under the port and stbd M/E’s. Remove oil from the buckets and dispose of properly to the satisfaction of the attending Marine Inspector.

Each item of survival craft equipment must be of good quality for the purpose it is intended to serve. Two life floats were discolored with inadequate retro-reflective tape. Restore or replace life floats so that they are orange in color and restore or replace retro-reflective tape in kind to the satisfaction of the attending Marine Inspector.

A vessel must carry a first aid kit under approval series 160.041 or with equivalent contents. The first aid kit did not meet the requirements of 160.041. Provide appropriate first aid kit.

A vessel must carry adequate up-to-date US Coast Pilot, CG Light List, Tide Tables. All publications were inaccessible due to computer failure. Prove that vessel can access appropriate publications aboard vessel to the satisfaction of the attending Marine Inspector.

The pump outlet must be fitted with a pressure gauge. Prove proper operation of fire pump pressure gauge. Renew dripping seal on fire pump.

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. Prove proper operation of the visual indicator for the high level water alarm in the lazarette.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. First Aid Kit contained expired Medications.

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places. No lifejacket placard was posted.

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions. Emergency Instructions were not mounted on board vessel.

Ring life buoys shall be of first class workmanship and free from any defects materially affecting their appearance or serviceability. All three ring life buoys were found to be significantly weather and faded to nearly white on one side.

Install dampers for forward engine room ventilation.

Prove proper operation of the PA system.

Post COI and Stability Letter.

Properly label child PFD storage locker.

Provide first aid kit meeting requirments in 46 CFR 184.710
