Every oil tanker of 150 gross tonnage and above and every ship of 400 gross tonnage and above other than an oil tanker shall be provided with an Oil Record Book Part I (Machinery space operations). The Oil Record Book, whether as a part of the ship's official logbook, as an electronic record book which shall be approved by the Administration taking into account the Guidelines developed by the Organization, or otherwise, shall be in the form specified in appendix III to this Annex: Collection of oil residues (sludge). Quantities of oil residues (sludge) retained on board. The quantity should be recorded weekly. IOPP supplement lists port and starboard waste oil tanks under section 3.1. Weekly soundings are only being recorded for port WOT due to non use of starboard WOT. Record weekly soundings for both tanks or work with certificate issuing authority to remove unused tank from IOPP supplement.

Vessels Port side vapor line did not have a stud on the flange face.

Use of the forward thermal fluid heater unit is restricted until you provide tech report. Tech report must verify replacement of external lagging/insulation providing adequate protection to prevent exhaust leaks and verification that tests were conducted to prove proper operation. 46 CFR 31.30-15 16c
