A second source of energy shall be provided for an additional three starts within 30 min unless manual starting can be demonstrated to be effective. MI found secondary source of starting the emergency diesel generator inoperable. Vessel is restricted from operations until verified completed by the attending USCG Marine Inspector.

A main source of electrical power of sufficient capacity to supply all those services mentioned in regulation 40.1.1 shall be provided. This main source of electrical power shall consist of at least two generating sets. Vessel is currently operating with one generator because of suspected water contaminated fuel. Fix to the satisfaction of Class/RO and the Coast Guard prior to departing the Miami COTP Zone. SOLAS 20 II-1/41.1.1

All marine engineering detail such as piping, valves, fittings, boilers, pressure vessels, etc., and their appurtenances installed on the vessel, shall be designed, constructed, and installed IAW with the provisions of sub chapter F (Marine Engineering). Power-operated main and auxiliary steering gear must be separate systems that are independent throughout their length. The main steering gear and rudder stock must be capable of moving the rudder from 35 degrees on either side to 35 degrees on the other with the vessel at its deepest loadline draft and running at maximum ahead service speed, and from 35 degrees on either side to 30 degrees on the other in not more than 28 seconds under the same consideration. Duty MIs received a report that the starboard rudder did not respond to helm control and was hard over port. The auxiliary system was able to bring the rudder back to 10 degrees port. The C/E determined the forward ram was discharging h/o within the ram. Conduct permanent repairs to the steering gear system to the satisfaction of the US Coast Guard.

The boundary bulkheads and decks separating the accommodation and control stations from machinery spaces shall be of "A" class construction. The following areas do not meet structural fire boundary requirements: 1) cable penetrations on 02 deck; 2) cable penetrations on 03 deck (2 locations); 3) cable penetrations on aft bulkhead of locking pin room; 4) aft bulkhead of main deck HVAC room around piping; 5) engine room overhead over main engines. 46 CFR 92.07-10(c)

Periodic Safety Test Procedures described in 46 CFR 61.40 must be submitted to the Coast Guard and approved. There is no objective evidence that the PSTPs were approved by the CG. Correspondence from MSC dated 01SEP17 indicates the PSTPs were accepted pending onboard testing by the OCMI. The onboard testing by the OCMI was not completed. 46 CFR 62.20-1(a)(7)

Maintenance of service necessary for the safety under normal and emergency conditions. Ship service generator has suffered unknown failures. vessel shall make permanent repairs to the satisfaction of attending Coast Guard Marine Inspector.

Radar inop due to heavy weather.

Properly repair leak for Fire Monitor water supply line (FIFI). Weld located on piping at bulkhead penetration in srore room 6717 on main deck. Permament repair to be verified to the satisfaction of the attending marine inspector.

Vessel required to prove proper operating of generator sets prior to operating.

Demonstrate bridge buoys release with tug in the notch (Dual Mode Sea Trials) before departing Erie, PA.

Demonstrate "Dual Mode" tug capacity for emergency towing.

Ensure self closing device on WTD/Fire door leading from the machinery space to the steering gear room is properly adjusted.

Prove proper operation of the Intercon System with tug in notch prior to sailing from Erie, PA.

Make appropriate pen and ink changes to Fire Safety Plan and submit for review/approval by USCG or ABS.

Provide as built, all apporved USCG FMEA/DVTP for relevant shipboard systems.
