If your vessel's Certificate of Inspection is renewed less often than annually, the marine inspector must check the settings of the safety or relief valves on all pressure vessels, except cargo tanks, twice every 5 years. No more than 3 years may elapse between any check and its immediate predecessor. Vessel rep did not have proof of testing for relief valve on air receiver in generator space.

The fire main system must be operated and the pressure checked. There is a hole in the fire main between the first overboard flange and the first elbow before the pressure gage.

There shall be at least two means of escape where the crew is normally employed. The switchboard room fwd exit door is obstructed from fully opening, hitting a patch in the deck. 46 CFR 91.10-5(a)

Every crewmember with emergency duties assigned on the muster list must be familiar with their assigned duties. During the fire drill in the machinery space (generator room), crew forgot to close ventilation and isolate power before entering the space. Additionally, during man overboard drill, crewmembers were unfamiliar with the rescue process of a person that has fallen in the water. Marine inspector had to prop one of the crewmembers to throw the life ring while waiting for the rescue boat to be on site. Company shall incorporate and establish comprehensive procedures to train all crewmembers, on board the barge and with a position on the station bill, on how to respond to a fire, man overboard, or any other emergency addressed on the vessel’s station bill. Vessel shall provide a copy of such procedures to the Coast Guard along with a copy of the training records for the different scenarios with the member’s trained. 46 CFR 199.180

Except as provided in §§ 80.177 and 80.179, operation of transmitters of any ship station must be performed by a person holding a commercial radio operator license or permit of the class required below. The operator is responsible for the proper operation of the station. None of the crewmembers on board the barge had a radio operators license. None of the crewmembers on board the vessel held a radio operators license. 47 CFR 80.155

Electric installations on vessels must ensure protection of passengers, crew, other persons, and the vessel from electrical hazards. Vessel’s lighting breaker panel by VFD room was missing blank breaker covers on the empty slots thus exposing the bus bar. Vessel shall install breaker covers on the missing slots. 46 CFR 199.180(b)

The operator or agent of each vessel must verify to his or her satisfaction that the PIC of any transfer of fuel oil requiring a Declaration of Inspection— (1) On each inspected vessel required by 46 CFR chapter I to have an officer aboard, and on each uninspected vessel, either: (i) Holds a valid merchant mariner credential issued under 46 CFR chapter I, subchapter B, with an endorsement as master, mate, pilot, engineer, or operator aboard that vessel, or holds a valid merchant mariner credential endorsed as Tankerman-PIC; or (ii) Carries a letter satisfying the requirements of § 155.715 and designating him or her as a PIC, unless equivalent evidence is immediately available aboard the vessel or at his or her place of employment. None of the crewmembers on board the vessel was able to provide a valid merchant marine credential with a Tankerman-PIC endorsement. Additionally, vessel was not able to provide a letter designating a specific member from the crew on board. Vessel shall always have a crewmember licensed or designated as a tankerman PIC by the company when running its machinery equipment. 33 CFR 155.710(e)

There must be ground detection for each: ship's service power system; lighting system; and power or lighting distribution system that is isolated from the ship's service power and lighting system by transformers, motor generator sets, or other devices. Lighting breaker panel by Variable Frequency Drive (VFD) room does not have a ground detection system installed. Install a ground detection system for the aforementioned panel. 46CFR111.05-21(b)(c)(d)

Fuel oil service, fire, cargo and fuel oil transfer and boiler feed pumps must be provided with a pressure gauge on the discharge side of the pump. Fuel pump next to fuel day tank did not have a pressure gauge indicating the discharge pressure. Install pressure gauge on the discharge side of the fuel pump. 46 CFR 56.50-10

The installation of all systems of an electrical engineering or interior communication nature,..., shall be in accordance with the requirements of Subchapter J of this chapter. Systems of this type include the following: Ship's service generating systems Ship's service power distribution systems Ship's lighting systems Emergency lighting and power systems Electric lifeboat winch systems Fire detecting and alarm systems Sound powered telephone and voice tube systems Navigation lights systems Daylight signaling lights Miscellaneous machinery alarms and controls General alarm systems The vessel could not produce documentation verifying that plan review as required by 46 CFR 96.05-1 had been conducted by the Marine Safety Center(MSC). The vessel shall contact MSC and submit all applicable plans and information required by 46 CFR 110.25-1 within 90 days.
