Whenever a defect is doscovered, either of which affects the safety of the ship or the efficiency or completeness of equipment other than life-saving appliances, the master or owner of the ship shal report at the earliest opportunity to the Administration, the nominated surveyor or recognized organization responsible for issuing the relevant certificate. If the ship is in a port of another Contracting Government, the master or owner shal also report immediately to the appropriate port State authorities and the nominated surveyor or recognized organization shall ascertain that such a report has been made. No notifications to the U.S. Coast Guard, local agent, or Administration were made regarding the low suction sea chest outlet valve defect prior to arrival. It was determined that the sea chest valve is not holding properly, likely due to some debris between the valve seat and valve flap. The valve is still in service. All other valves and the system as a whole operates as intended.

The machinery, boilers, and other pressure vessels associated piping systems and fittings shall be a design and construction adequate for the service for which they are intended and shall be so installed. M/E Jacket cooling pump domestic 3-way valve is actively leaking and being drained via flex hose, flag dispensation letter to be issued for transit to Long Beach, CA. Additionally, Cooling fresh water condenser is spraying water.

Resolution: The 3-way valve has been removed and the packing of the stem shaft has been replaced. The valve is no longer leaking. Additionally, the cooling fresh water condenser leaks has been corrected by removing the flanged elbow. New thread seal was administered.

Oil filtering equipment shall be of a design approved by administration and will ensure any oil mixture discharged does not exceeed 15 ppm. Monitering equipment does not display proper date or time and crew is not able to print or display stored data. PSCO reccomends the issuance of a new calibration certificate.

The daily bilge pump has been replaced with diaphragm pump fed with a flex hose from general supply compressed air system.
