Each towing vessel must be equipped with a means to detect smoke in the berthing spaces and lounges that alerts individuals in those spaces. The two aft berthing spaces on the deck below the operating station did not have a means to detect smoke. Install a smoke detection system or individual battery operated detectors meeting UL 217.

No person may operate an oceangoing ship of less than 400 gross tons unless it has the capacity to retain all oily mixtures and discharge to a reception facility or has approved oily-water separating equipment. Vessel has installed a cofferdam and nuisance pump under the port and stbd shaft seals that discharges directly overboard. Remove, or make unusable, the nuisance pump system to the satisfaction of attending marine inspector.

To stop the flow of fuel in the event of a fire or break in the fuel line, a remote fuel shutoff valve must be fitted on any fuel line that supplies fuel directly to a propulsion engine or generator prime mover. Each remote valve control must be marked in clearly legible letters, at least 25 millimeters (1 inch) high, indicating the purpose of the valve and the way to operate it. The valve must be operable from a safe place outside the space where the valve is installed. FIVE(5) PT side valves were not labeled as to what does those valve shuts off.
