To stop the flow of fuel in the event of a fire or break in the fuel line, a remote fuel shutoff valve must be fitted on any fuel line that supplies fuel directly to a propulsion engine and the valve must be operable from a safe place outside the space where the valve is installed. During testing of the emergency fuel shutoff system, the starboard #3 quick-closing valve did not operate when activated from its remote location.

These devices must be suitable for the expected route. Both the port and starboard doors leading from the weather deck into the galley were found to have significant wastage and corrosion around the door frames. This condition compromises the weather tight integrity of the deck-house.
