A tank vessel or barge underway or at anchor must have all the closure mechanisms on the following openings properly closed, ullage openings and any other tank vessel openings that maintain the seaworthiness condition of the tank vessel and prevent the inadvertent release of oil or hazardous cargo. Deckhands reported that the Port #2 butterworth hatch is leaking vapor. Identify source of vapor release and repair prior to returning to service.
