While departing, crew observed smoke and leaking air on the M/E. Upon investigation, the starting valve on the M/E unit 1 stuck in open position due to pieces from a broken non-return valve.

001 - Each loading arm and each transfer pipe system, including each metalic hose, must not leak under static liquid pressure at least 1 1/2 times the maximum allowable working pressure. Records show that the crew has been conducting test with air, not static liquid pressure, since 2016. Vessel shall not bunker in the U.S. until test is conducted with static liquid pressure. - 33 CFR 156.170(c)(4)
