The design, construction, equipment and operations of ships certified to carry noxious liquid substances in bulk identified in chapter 17 of the International Bulk Chemical Code shall be in compliance with the International Bulk Chemical Code when the chemical tanker is constructed on or after 1 July 1986. Vessel's Cargo Authority Attachment contains IBC Code chapter 17 and 18 cargoes, though the vessel has not been surveyed for nor issued international certification for the carriage of NLS. Vessel’s Procedures & Arrangement (P&A) Manual and Cargo Authority Attachment must be revised and approved as appropriate through the Marine Safety Center (MSC). Vessel may not load NLS or IBC Chapter 17 cargo until it is in compliance with the January 1, 2021 revisions to MARPOL Annex II and the IBC Code. MARPOL Annex II/11.1

All electrical installations in hazardous locations must comply with the general requirements of section 33 of IEEE 45-1998 (incorporated by reference; see 46 CFR 110.10-1), and with either Articles 500 through 505 of NFPA NEC 2002 (incorporated by reference; see 46 CFR 110.10-1) or with the IEC 60079 series (as defined in 46 CFR 111.105-1 and incorporated by reference; see 46 CFR 110.10-1). A cable penetration from the weatherdeck to the void underneath the administration office was not vapor tight. Deficiency is to be rectified prior to the carriage of cargo.

Make proper repairs to port side shell and side long IWO frame #1 - #2.

Gouge out and drill stop crack on the weld seam IWO port side frame #1 - #2.

Prove proper operation of start air relief valves. 46 CFR 54.15-10(g)

Blanks must be provided for the openings located in the navigation light panelboard IAW 46 CFR 111.40-1

Cargo hoses shall be stenciled with the most recent hydro test conducted on 19MAY2015 IAW 33 CFR 154.500(f).

Remote emergency shutdowns for cargo pump prime movers not operational. Solenoid valves need to be replaced. Upon repair, emergency shutdowns shall be tested in the presence of a USCG marine inspector IAW 46 CFR 32.50-35.

Navigation lights need to be replaced: stern light, (2) mast head lights not working properly. IAW COLREGS, no transitting after sunset.

Program software not available during time of inspection for shutdown/alarm testing of Detoit Diesel series 60 engines (#1 and #2). Service report must be provided, attesting to the satisfactory testing of both engines, or satisfactorily tested in the presence of a USCG marine inspector IAW 46 CFR 58.05-1.

Vapor tightness for both machinery spaces compromised due to holes located on the rubber boots leading from the #1 and #2 cargo pumps to the machinery spaces housing for pump prime movers. Rubber boots need to be replaced and verified by the attending marine inspector IAW 46 CFR 111.105

Test #1 and #2 house air relief valves.

Secure exhaust leak on stb cargo pump.

Submit proposal for permanent & proper repairs of forepeak lower void damage identified stbd of centerline (sideshell, deck & bottom longs, brackets, & transverse frame) & in way of #2 port transverse frame & sideshell aft of frame. Repairs to be completed prior to carrying cargo. Barge authorized transit to Seward, Alaska.

Provide required testing of piping & hoses onboard. 33 CFR 156.170 - Prior to transferring cargo in U.S.

Repair high level alarm on #3 starboard cargo tank. 46 CFR 39.20 - Prior to loading cargo in U.S.

Provide current COFR - 33 CFR 138 - Prior to entering EEZ of U.S.

Provide required testing of portable IM Tanks - 46 CFR 98 - Prior to using

Re-mark load line IAW int'l load line certificate.

Install tachometer for generator.

1 Port wing tank, bay 4, V/W swash bulkhead IWO SSL 1 as marked.

3 Port wing tank, bay 5, C/R frame 5 as marked.

3 Port wing tank, bay 6, C/R frame 6 as marked.

C/R sideshell IWO 3 port back to good metal.

Vessel is restricted to the carriage of grade A (max. 25 psia Reid) and Lower Grade Flammable or Combustible Liquids Identified in 46 CFR Table 30.25-1 as Pollution Category I and cargoes listed in the IBC Code identified as “Other Substances (OS)” until the Cargo Authority Attachment has been verified by the Marine Safety Center for suitability to carry cargoes regulated under MARPOL Annex II.

Remove extension cords and install outlets where needed in ships office.

Make temporary repairs to Port Navigation light kick pipe, at next drydock maker pernament repairs.

Install lagging on both pump engines manifolds as discussed.

Combustible materials stowed on top of the electrical switchboard.

Port and Stbd cargo pumps leaking at the hard pipe on deck.

Lube oil gauge on generator inoperative.

#1 Stbd stripping pump missing drip shield.

As per letter to Sector NY dated 12/23/05, Vsl's #'d relief valves # 1 P/S, 2P, and 4S and have not been tested IAW regs. Have valves properly hydro tested and submit proof of said test to Sector NY prior to usage of cargo pumps they cover. To the satisfaction of USCG Marine Inspector.

As per letter to Sector NY dated 12/23/05, junction box for #1 stbd cargo pump is not suitable for installation in the required hazard zone. After installation of proper junction box, submit proof to Sector NY prior to usage of cargo pump 1S which it covers. To the satisfaction of USCG Marine Inspector.

Circuit not labeled.

Permanently secure refrigerator to deck or bulkhead.

Lubricate/free up release pin and hinge for anchor cable stay.

Provide original IOPP certificate for USCG signature or apply for new one.

Provide original COD for USCG inspection.

UPON ARRIVAL BROOKLYN, NEW YORK CONDUCT UNDERWATER SURVEY OF BARGE TO THE SATISFACTION OF THE ATTENDING MARINE INSPECTOR AND OCMI NEW YORK.
