During initial COI it was noted that the vessel considers itself to be a dual-mode ITB. NVIC 2-81 CH.1 describes a dual mode ITB as;
1. A tug and barge are equipped and rigged with necessary gear for hawser towing. To include a towing engine or bits, hawser and bridle.
2. A tug that has the capability to separate safely in a timely fashion at a predesignated sea state and shift to towing on the hawser.
3. Vessels are required to submit the design information to the Coast Guard that reasonably supports the ability of the Dual Mode ITB to "routinely" disconnect at sea. The vessel states that the towing capabilities are for emergency use only. Vessel is required to obtain ITB "push mode or dual mode" determination form either ABS or Towing Vessel National Center of Expertise (TVNCOE). 46 CFR 137.120(a)

During the initial COI it was noted that the vessel is equipped with special design features or a specially designed connection system for the integration of the tug and barge meeting the definition of an integrated tug and barge (ITB) in 46 CFR 10.107. ITBs are separated into two categories push-mode or dual-mode. Provide documentation to establish the ITB category in accordance with NVIC 2-81, CH-1, GUIDELINES FOR INTEGRATED TUG BARGE COMBINATIONS within 60 days from the date of this CG-835. Specifically, documentation to establish the maximum sea state the tug can safely disconnect and reconnect based on engineering assumptions reviewed by the USCG Marine Safety Center or ABS. The ability of the ITB to separate and shift to a stern or alongside tow at a designated sea state using the lesser of the manning proposed by the company with the exception of the LBS route endorsement.

During the steering test when using number #2 steering pump, from hard port to hard stbd, when the rudder reached the rudder stopper the steering pump would not respond. After the #2 steering pump did not respond after hitting the rudder stop the #1 steering pump had to be used to bring back the rudders to midship. Repair the number #2 steering pump to fully functional.

The vessel has been both in satisfactory service insofar as structural adequacy is concerned and does not cause the structure of the vessel to be questioned by either the OCMI, or TPO engaged to perform an audit or survey.
A fracture directly above the stbd rudder, under deck transverse frame. It was originally identified in 20 Nov 2021. The crack continued to propagate another 1.5 inches as seen on 18 March 2022. The area of the fracture needs to be cropped out and replaced.
