A safety system shall be provided to ensure that serious malfunction in machinery or boiler operations, which presents an immediate danger, shall initiate the automatic shutdown of that part of the plant and that an alarm shall be given. It was reported by the ship’s Captain that the oil mist detector for the main propulsion engine is showing fault. Vessel to effect permanent repairs per manufacturer’s procedures and operational tests to the satisfaction of the attending Class surveyor at next port Busan, Korea after sailing from Guam COTP Zone. Pending satisfactory repairs/tests, the engine room must be fully manned and safety measures implemented to ensure the safe operation of the vessel and safety of the crew.

Crew unable to find drug/alcohol testing requirements following a Serious Marine Incident (SMI) to include the collection of drug-test specimens of each individual must be conducted within 32 hours of when the SMI occurred, unless precluded by safety concerns directly related to the incident in vessel's SQEMS. The marine employer shall ensure that all individuals engaged or employed on board a vessel are fully indoctrinated in the requirements of this subpart, and that appropriate vessel personnel are trained as necessary in the practical applications of these requirements.
