A safety system shall be provided to ensure that serious malfunction in machinery or boiler operations, which presents an immediate danger, shall initiate the automatic shutdown of that part of the plant and that an alarm shall be given. It was reported by the ship’s Captain that the oil mist detector for the main propulsion engine is showing fault. Vessel to effect permanent repairs per manufacturer’s procedures and operational tests to the satisfaction of the attending Class surveyor at next port Busan, Korea after sailing from Guam COTP Zone. Pending satisfactory repairs/tests, the engine room must be fully manned and safety measures implemented to ensure the safe operation of the vessel and safety of the crew.

The marine employer shall ensure that all persons directly involved in a Serious Marine Incident (SMI) are chemically tested for evidence of dangerous drugs and alcohol in accordance with the requirements of 46 CFR 4.06. Crew unable to find drug/alcohol testing requirements following a Serious Marine Incident (SMI) to include the collection of drug-test specimens of each individual must be conducted within 32 hours of when the SMI occurred, unless precluded by safety concerns directly related to the incident in vessel's SQEMS. The marine employer shall ensure that all individuals engaged or employed on board a vessel are fully indoctrinated in the requirements of this subpart, and that appropriate vessel personnel are trained as necessary in the practical applications of these requirements. Submit proof of procedures included in vessel's SQEMS.

After any survey of the ship under Article 14 has been completed, no change shall be made in the structure, equipment, arrangements, material or scantlings covered by the survey, without the sanction of the Administration. While conducting hot work on the aft port side mooring deck, a hole through the deck allowed slag to fall below which resulted into a fire in the store room. Hole on the main deck shall be repaired to the satisfaction of the RO prior to departure.

74 SOLAS (2009) Chapter III, Regulation 20.3: Maintenance, testing, and inspection of life-saving appliances shall be carried out based on the guidelines developed by the Organization and in a manner having due regard to ensuring reliability of such appliances. Incorrect battery used in each floating electric water light attached to ring lifebuoy. Replace with battery as per manufacturer’s recommendation.

74 SOLAS (2009) Chapter III, Regulation 20.3/LSA Code Chapter 1, Paragraph 1.2.2.7: Maintenance, testing, and inspection of life-saving appliances shall be carried out based on the guidelines developed by the Organization and in a manner having due regard to ensuring reliability of such appliances. Replace deteriorated reflective tape on lifebuoys with new SOLAS approved and remark vessel’s name in block capitals of the Roman alphabet per SOLAS Chapter III, Regulation 7.1.4.

74 SOLAS (2009) Chapter III, Regulation 11: In addition to and as part of the markings required under regulation II-2/28.1.10, routes to muster stations shall be indicated with the muster station symbol, intended for that purpose, in accordance with the recommendations of the Organization. Replace faded and severely weathered IMO symbols on exterior of house with new and SOLAS approved.

74 SOLAS (2009) Chapter III, Regulation 10: Containers, brackets, racks and other similar stowage locations for life-saving equipment shall be marked with symbols in accordance with the recommendations of the Organization. Replace faded and severely weathered IMO symbols for lifesaving equipment on deck and exterior of house with new and SOLAS approved.

74 SOLAS (2004) Chapter II-1/Part A-1, Reg. 3-1: In addition to the requirements contained elsewhere in the present regulation, ships shall be designed, constructed and maintained in compliance with the structural, mechanical, and electrical requirements of a classification society. Replace missing screen on vertical air vent forward of bosun locker.

74 SOLAS (2004) Chapter II-1/Part A-1, Reg. 3-1: In addition to the requirements contained elsewhere in the present regulation, ships shall be designed, constructed and maintained in compliance with the structural, mechanical, and electrical requirements of a classification society. Weather tight doors for hospital space, paint locker, bosun locker and port/starboard exterior door on C-deck do not close properly to seal the accommodation or compartment from ingress of water or flooding during bad weather. Repair weather tight doors to close properly and seal spaces from ingress of water.

SOLAS 74(2014 Amend.) I/11: The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Effect repairs to the number one Fuel Oil Purifier.

Make repairs to damaged plate to satisfaction of attending CG inspector prior to departing next port, Guam.

Vessel OWS incapable of processing water, device requires service or replacement. Vessel is able to retain oily-water onboard with sufficient capacity to discharge at a port with reception facility after any leg of scheduled voyage. Vessel is authorized to retain oily-water onboard. Repairs or new installation should be made by 31 MAY 2019. Servicing records should be provided to the Coast Guard after repairs or installation is completed.

Repair or replace pressure relief valve for forward air compressor; valve did not operate when tested using hand relief mechinism. (to be cleared prior to use of compressor).

Replace liquid relief valve located between compressor and air reciever with vapor type relief valve; valve was found to be improper type. (to be replaced prior to use of compressor).

Make repairs to damaged railings to #1 cargo crane

Starboard gangway missing locking pins. Crew using fabric ratchet straps and non-standard hardware in an attempt to secure lader rails.

Vessel Master failed to submit NOAs for Saipan and Guam in the required timeframe.

The vessel reported that the automation system has intermittently shut down the #2 and #3 generators for unknown reasons (system is issuing a stop command not associated with a critical shutdown) and which cannot be reproduced by the crew.

Belt failed, debris flew though safety cage and damaged the radiator/cooling system.

Repair No. 2 ballast pump.

Properly make repairs to vessel's communication failure between the Kongsberg AC4 in the Pilothouse and the ERC, so the vessel is fully controllable from the Pilothouse and does not periodically shut down the MDE.

Vessel failed to submit Notice of Arrival IAW 33 CFR 160.212

Demonstrate proper operation of main engine throttle control from bridge.

Numerous items related to the vessel's PSTP's were found to be insufficient or incorrect. Noted in the 835.

The charging motor to the bow thruster starting circuit breaker is inoperable.

The lifeboat safety and launching policy in the vessel's SMS identifies a procedure that is not in compliance with mandatory rules and regulations.

Bridge Navigation Watch Alarm System (BNWAS) is inoperable.

Doors used for means of escape have externally lockable doors preventing emergency egress from several areas in the superstructure. Remove locking hasps.

Hole was discovered adjacent to the A-60 door leading into the engine room. Repair IAW A-60 boundary requirements.

Official number is not permanently affixed to the interior of the hull.

Public address system is inoperable in the forward house.

The bilge pump motor in cargo hold #7 does not meet the explosion proof requirements to allow dangerous cargo with flammable, combustible, or explosion characteristics per DOC-DG. Replace bilge motor and no DG shall be carried in cargo hold #7 until replacement.

Hold back device was discovered holding the main engineroom door open at an A-60 boundary.

Oily Water Separator was found inoperable.
