All arrangements used for pilot transfers shall efficiently fulfill their purpose of enabling pilots to embark and disembark safely. While attempting to embark from the dock, the pilot was not able to safely climb the gangway due to the severe angle of incline. The gangway moved as the ship lightered causing the gangway to become shift in an unsafe maner. 74 SOLAS (14) V/2.1 ICLL 1/26.2

The condition of the ship and its equipment shall be maintained to conform with the present regulations to ensure the ship in all respects will remain fit to proceed to sea without danger to the ship or persons onboard. Vessel has multiple structural damage in WBT No. 1,2,3,4 STBD side with a hole in the WBT No. 2 hull plating. Provide class report and plan to fix/temporarily repair damaged areas prior to departure.

Resolution: Class issued condition of class requiring temp repairs for vessel to head to dry dock for permeneant repairs.
