Clear instructions must be provided on the vessel that detail the actions each person on board should follow in the event of an emergency. Each muster list must at least specify: (6) the duties assigned to different members of the crew. Duties to be specified include...(v) mustering the passengers and other persons on board; (7) the duties assigned to members of the crew in relation to passengers and other persons on board in case of an emergency...assigned duties to be specified include: (iii) assembling passengers and other persons on board at muster stations, (iv) keeping order in the passageways and on the stairways and generally controlling the movements of passengers and other persons onboard.
During the man overboard and abandon ship drills, passengers were directed to muster in the main dining room and 3rd deck lounge, accordingly. When Marine Inspectors reported to these spaces as a simulated passenger and queried the crew on who was responsible for mustering of passengers and generally controlling the passengers during these emergencies, no crewmember in attendance acknowledged as the responsible person. The current procedures call for the hotel manager to personally take accountability of all passengers (vessel certificated for 173 overnight passengers) himself. Additionally, during the fire drill crew at the passenger muster station were queried regarding procedures for assisting passengers up/down the stairs; crew responded that the deck team handled carrying passengers via Stryker chairs. However, the deck team are the primary firefighters on the station bill for fighting the fire. Finally, during the abandon ship drill, crew simulated IBAs being deployed to and passengers/crew boarding the IBAs from the stern, while the passenger safety bill posted in staterooms states that the embarkation station is the main deck port side. The stern location also does not appear to meet all requirements for an embarkation station (example: floodlights on emergency power iaw 120.434, etc.). Update the station bill as necessary to address the noted gaps. Ensure all emergency procedures and documents are in alignment regarding embarkation station location(s). Provide updated station bill and signed attestation from master for training on updates to the satisfaction of the local OCMI.

Three Port Side Inflatable Life Rafts were installed incorrectly to the hydrostatic release and vessel.

A fire damper must close against the draft in the duct and be accessible for periodic inspection by means of a hinged or bolted plate in the duct and surrounding bulkhead or deck, if fitted. Fire dampers must also be marked with red letters of at least 0.5 in in height stating “VENTILATION FIRE DAMPER”.
Deficiency: Fire dampers located in the laundry room were neither accessible nor properly labeled.
