The function of... fire detection shall be periodically tested.. by means of equipment producing hot air at the appropriate temperature. PSCO observed heat detector testing device not operational. 74 SOLAS (cons) 2004 II-2/ 7.3.2

The following functional shall be met, fire protection systems...shall be maintained and ready for use. PSCO observed 01 system fault on fire control panel for zone 13 "Forward". 74 SOLAS (Cons) 2004 II-2/ 14.1.1

The company should ensure that all personnel involved in the company's safety management system have an adequate understanding of relevant rules, regulations. codes and guidelines. During PSC exam, PSCO witnessed multiple safety violations and inoperable safety equipment as documented in the objective evidence below. USCG PSCO recommends an external audit of the vessel’s Safety Management System be completed within 14 days and verification of completion submitted to the USCG.

The purpose of this regulation is to maintain and monitor the effectiveness of the fire safety measures the ship is provided with... fire protection systems and fire fighting protections shall be maintained ready for use. PCSO observed 04 fire detectors in the engine room were not operational.

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: protection of openings and penetrations in these divisions. PSCO observed 01 hold back on the purifier room door.

The purpose of this regulation is to maintain effectiveness of the fire safety measures the ship is provided with. the following functional requirements shall be met: Fire protection systems and appliances shall be maintained ready for use. PSCO observed 02 system "General Fault's" on the fire control panel in the engine control room for zone L10.

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Lifeboat mechanical steering was found inoperable when PSCO requested ship’s crew to test. PSCO observed an active leak of hydraulic fluid from the lifeboat’s mechanical steering gear.

Oil fuel pipes, which, if damaged, would allow oil to escape from a storage tank having a capacity of 500 L... shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space. PSCO observed the valve not in the closed position after pulling the line.

In order to achieve the fire safety objective sat out in paragraph 1... the restricted use of combustible material. PSCO observed unsecured hazmat and combustible liquids stored under the fuel tank in the purifier room.

The administration may exempt from paragraphs 7.1.3 and 7.2 cargo spaces. Solely intended for carriage of cargoes that constitute a low fire risk. When such exemptions are granted the administration shall issue a certificate. PSCO observed no exemption certificate. 74 SOLAS (09) II-2/ 10.7.1.4

Oil filtering equipment referred to in paragraph 2 of this regulation shall comply with paragraph 6 of this regulation. In considering the design of such eqiupment and approvals, the admin shall have regard to the specification recommended by the organization. PSCO observed crew members unable to provide OWS history on oil content meter. MARPOL 1/14.7

Before the ship leaves port and at all times during the voyage, all LSA shall br in working oder and ready for immediate use. PSCO observed a significant lube oil discharge during operational test of the rescue boat engine.

The attained annual operation CII shall be documented and verified against the required annaul operational CII to determine operational carbon intensity rating. The vessel was unable to provide an annual operational carbon intensity rating.

1509 - Maintenance of the ship and equipment The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations, and with any additional requirements which may be established by the company. Due to the objective evidence detailed in the below deficiencies, the vessel is not in substantial compliance with the relevant conventions, calling into question the adequacy and implementation of the vessel's SMS under the ISM code. Furthermore, PSCO observed objective evidence that the SMS requirements for maintenance of life saving equipment and fire fighting equipment were not being adhered to. An external audit conducted by Flag or RO is required to determine whether the ship is operating in accordance with the ISM Code. Provide a copy of the completed audit report to USCG prior to departure from port. 74 SOLAS(20) IX/3.1 ISM Code A/10.1

11104 - Rescue Boats The condition of the ship and its equipment shall be maintained to conform to the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO observed rescue boat in poor material condition. Rub rail on bow was peeling off pontoon, seat attachments were coming detached, vinyl on transom was not connected and would allow for water ingress, not suitable for crew. 74 SOLAS (2020) I/11a

07105 - Fire doors/openings in fire-resisting divisions In order to contain a fire in the space of origin, the fire integrity of the divisions shall be maintained and openings and penetrations. PSCO observed 01 damaged A-0 self closing fire door (accommodation deck stairwell door) that would not prevent the passage of smoke or flame. Door latching system was completely missing, rendering door unable to remain shut to prevent the spreading of fire and smoke. 74 SOLAS (2014) II-2/9.1.3

11113 - Launching arrangements for rescue boats The rescue boat embarkation and launching arrangements shall be such that the rescue boat can be boarded and launched in the shortest possible time. PSCO observed crew unable to lower rescue boat, due to lack of proper maintenance of falls. PSCO was informed that rescue boat and davit inspections are conducted every Saturday, last inspection was 25MAR23. 74 SOLAS (20) Ch III Reg 17.1

11108 - Inflatable liferafts Each liferaft shall be stowed with a float-free arrangement complying with the requirements of paragraph 4.1.6 of the Code. PSCO observed that the 05 liferafts had non-SOLAS approved weak links attached. These non-SOLAS approved arrangements are unsuitable for launching requirements for the vessel's area of operation, thus compromising the safety of the crew in an emergency situation where they would need to abandon ship. 74 SOLAS (20) Ch III Reg 13.4.2

04103 - Emergency, lighting, batteries and switches Muster and embarkation stations shall be adequately illuminated by lighting supplied from the emergency source of electrical power. Emergency lighting around the main superstructure and embarkation station areas were of a bayonet base type style light bulb with a 24volt/20-watt capacity. When illuminated, PSCO observed that the light bulbs did not put out adequate illumination to light pathway from main superstructure to embarkation/muster station in case of any emergency. 74 SOLAS (20) Ch III Reg 11.4

07110 - Fire fighting equipment and appliances Maintenance, testing, and inspections shall be carried out based on the guidance developed by the Organization. PSCO observed multiple portable fire-extinguishers, emergency escape breathing apparatuses, and fire-locker inspection tags that were not completed for the month of February 2023. Although, the monthly SMS check-sheet had been marked as completed. 74 SOLAS (2004) II-2/14.2.2.1

03109 - Machinery space openings Machinery space openings in position 1 or 2 shall be properly framed and efficiently enclosed by steel casings of ample strength. PSCO observed the watertight door leading into emergency generator space to be missing the lower left swivel guide for the no. 5 dog arm. The door was able to be closed and secured, but unable to complete a watertight seal due to the dog arm being able to lock in place. ICLL (88) CH.2/17.1

Each liferaft shall be stowed fully equipped as required by the code.....If a weak link is used in a float free arrangement it shall break under strain of 2.2 +- 0.4kN. All painter lines were found attached to the ship's structure with no weak ink as required by liferaft approval letter. 74SOLAS20 III/13.1.4 LSA IV 4.1.6.2

PSCOs noticed stream of water leading from stbd engine block and when notified C/E could not explain. Source of leak is unknown.

Oil fuel lines shall be screened to avoid oil spray or leakage onto sources of ignition. EDG lines were not suitably screened.

For any vessel that is equipped with ballast water tanks and bound for ports or places in the United States, ballast water report must be submitted no later than 6 hours after arrival at the port or place of destination, or prior to departure from that port or place of destination, whichever is earlier. Vessel did not submit a Ballast Water Management Report to the National Ballast Information Clearinghouse within the required time. 33 CFR 151.2060(b)(3)

A lighting system shall provide illumination throughout those parts of the ship normally accessible and used by the crew. Safety flood lights and poop deck lights have been found damaged. 74SOLAS(09) II-1/41.2.1 Action 10c

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Navigational stern light found inoperable. 74SOLAS(14) I/11(a) Action Taken: 10c

Rescue boats shall have sufficient mobility and maneuverability. Rescue boat outboard engine inoperable. 74SOLAS(14) III/31.2 LSA 5.1.1.7 Action 17ac

Rescue boat embarkation and launching arrangements shall be such that the rescue boat can be boarded and launched in the shortest possible time. Rescue boat davit found sheered off, deeming it inoperable. 74SOLAS(14) III/17.1 Action 17ac

An embarkation ladder complying with the requirements of paragraph 6.1.6 of the Code shall be provided at each embarkation station. Embarkation ladder found missing from its embarkation station and damaged. 74SOLAS(14) III/11.7 Action 17 ac

The construction ad materials of all means of access and their attachment to the ship's structure shall be to the satisfaction of the Administration. Catwalk gratings found missing. 74SOLAS(09) II-I/3-6.2.3 Action 16ac
