RIVER CLEANUP

Master reported vessel engine room was flooding and the starboard quarter was completely submerge under water. The cause of flooding is unknown. The cognizant OCMI may require review of a vessels watertight and weathertight integrity. The review may include an examination of a plan that shows the original placement of decks and bulkheads. Provide temporary repair proposal to make vessel safe for transit to repair facility and make vessel temp repair available for inspection. Provide transit plan to include ETD, ETA and repair facility prior to departure to the satisfaction of the attending Coast Guard Marine Inspector.

Vessel flooded and sank at OHR approximately MM 475. Crew identified hole in starboard aft void and made a temporary epoxy patch to stop flooding. The cognizant OCMI may require a review of a vessels watertight and weathertight integrity. The review may include an examination of a plan that shows the original placement of decks and bulkheads. Vessel shall proceed under tow in accordance with the transit plan to Evansville Marine Service dry dock at mm 751. Contact local OCMI prior to conducting repairs to vessel's hull. Owner shall ensure that the vessel is in satisfactory condition and fit for the service for which it is intended, and that it complies with the applicable statutes and regulations for such vessels to the satisfaction of the attending Coast Guard marine inspector prior to departure.

Main engines must be equipped cooling water temperature gauge at the machinery location. Port and starboard MDE cooling water gauges were inoperable.

General alarm placard not in engine room as required under 46 CFR 27.201

Missing Globes / Covers over lights in machinery space

Fire panel zones on control panel not labeled properly as required under 46 CFR 27.203

Globe / Cover missing on lighting fixture located above wet sink on second deck

Receptacle Outlet cover plate missing 2nd deck storage room

Master did not have a restricted radio operators license on board.

Install fire pump per 46 CFR 27.301. Required by 29 APR 2005.

Install fire extinguishing system in the engineroom per 46 CFR 27.303.
