A towing vessel must carry adequate and up-to-date charts, maps, and nautical publications for the intended voyage, including: (1) Charts, including electronic charts acceptable to the Coast Guard, of appropriate scale to make safe navigation possible. Vessel was found operating with electronic charting system that does not have a type approval or manufacturer’s declaration of conformity and therefore does not meet the chart equivalency requirement.

A 40-B portable fire extinguisher must be located near the storage room or cabinet. Fire extinguisher for storage cabinet was not mounted near the storage cabinet.

Pressure vessels must be externally examined annually. Relief valves must be tested in accordance with § 143.245. No record of when Pressure vessel relief valve was installed or tested on board.

Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Combustible waste receptacles were observed on board the vessel.

Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems, must: (1) Be maintained to ensure proper operation. Marine Inspectors observed significant shaft seal leaks on both Port and Starboard shaft seals. Complete necessary maintenance.

Each towing vessel must be maintained and operated so as to minimize fire hazards and to ensure the following: (a) All bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable. Marine Inspectors observed an active fuel leak on the fuel oil tank sight glass lower valve with excessive fuel buildup in the bilge directly beneath. Repair valve and remove fuel buildup from bilges.

The original COI must be framed under glass or other transparent material and posted in a conspicuous place on board the towing vessel OR kept on board in a weathertight container and must be readily available. Current COI issued 06 JUN 2023 was not found on board. Place COI on board vessel.

No later than July 22, 2019, the owner or managing operator must implement a health and safety plan. The health and safety plan must document compliance with this part and include record keeping procedures. The owner, managing operator, or master must ensure that all persons on board a towing vessel comply with the health and safety plan. Crew was unable to provide Marine Inspectors the vessel's specific Health and Safety Plan. Additionally, Marine Inspectors could not verify if the on board crew had completed the training requirements contained within 46 CFR 140.515.

Prior to getting underway for the first time on a particular towing vessel, each crew member must receive a safety orientation covering the topics contained within 46 CFR 140.410(b)(1-10). Marine Inspectors could not verify if onboard crew members received proper safety orientations.

The frame or support of any semi-portable fire extinguisher fitted with wheels must be welded or otherwise permanently attached to a steel bulkhead or deck to prevent it from rolling under heavy sea conditions. Marine Inspectors observed semi-portable fire extinguisher not secured to the vessel.

The following list of items must be recorded in the TVR, official logbook, or in accordance with the TSMS applicable to the vessel: (1) Personnel records, (2) Safety orientations, (3) Record of drills and instruction, (4) Examinations and tests, (5) Operative navigational safety equipment, (6) Navigation assessments, (7) Navigation safety training, (8) Oil residue discharges and disposals, (9) Records of inspection of towing gear, (10) Fire-detection and fixed fire-extinguishing records Marine Inspector reviewed vessel's Towing Vessel Record. Latest date of recorded items was 25 DEC 2023. Towing Vessel Record was also missing numerous required entries to include but not limited to: Safety orientations, navigation assessments, records of drills, examination and tests, and operative navigational equipment. Company is to develop, and implement TVR record-keeping process and provide training for all vessel crew members on record-keeping requirements contained within 46 CFR 140.915(a)(1-10).

Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems, must: (1) Be maintained to ensure proper operation. 03 JAN 2024: Vessel experienced a port engine reduction in propulsion. Diagnose cause of port engine shutdown and verify extent of damage. Submit documentation to [email withheld] Sea trials to commence once all documentation is received. Clearance of above issued deficiency will be to the satisfaction of the attending CG Marine Inspector.

A vessel that is exposed to salt water more than 6 months in any 12-month period since the last examination or initial certification must undergo a drydock and internal structural examination at least twice every 5 years, with not more than 36 months between examinations. Vessel has not completed required drydock and internal structural inspections prior to due date of 31MAR23. Vessel prohibited from conducting commercial operations after due date until inspections are complete and subsequent requirements from inspections complete. Vessel permitted light ship movement to facilitate transit to drydocking location. 46 CFR 137.300 (a)(1)

Each life float and rigid buoyant apparatus must be fitted with a lifeline, pendants, a painter, and floating electric water light approved under approval series 161.010. Provide floating electric water light for RBA. Attach painter line on RBA to line attached to weak link, verify line is maintained to allow float free arrangement. 46 CFR 141.325 (c)

Pressure vessel relief valves required to be tested twice every 5 years, with no more than 3 years elapsing since last test. Unable to verify relief valve was tested and unable to verify set pressure of relief valve. 46 CFR 143.245
