Electrical connections must be suitably installed to prevent them from coming loose through vibration or accidental contact. On 29 November 2021 the ITV LUDLOW experienced a starboard MDE failure while conducting fleeting operations IVO Ohio River mile 626.4 LDB at Mill Creek Power Plant. It was determined that the starboard MDE will need to be replaced. Vessel required to conduct repairs at Mill Creek Power Plant mile 626.4 Ohio River and contact the Coast Guard duty marine inspector at [phone withheld], when repairs are completed and before placing vessel back into service. In the interim until the ITV LUDLOW can be replaced by another fleet boat at Mill Creek Power Plant the ITV LUDLOW is authorized to conduct limited fleet work of holding hopper barge to the dock so loading of fly ash can continue.
