Starboard clutch failed during working operations. Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Troubleshoot and conduct repairs to the satisfaction of the attending Coast Guard marine inspector.

Starboard main engine has and exhaust leak. Machinery must be designed and maintained to provide safe operation of the towing vessel and safety of the crew. Conduct repairs of the starboard main engine exhaust to the satisfaction of the attending Coast Guard marine inspector.

Starboard battery trickle charger was plugged into outlet that may easily be unplugged resulting in battery low charge so that the starboard generator would not start. Electrical connections must be suitably installed to prevent them from coming loose through vibration or accidental contact. Hardwire trickle charger to the satisfaction of the attending Coast Guard marine inspector.

Vessels Stern light needs to be relocated from under the second deck back a couple feet to provide a better view. Rule 21

Means must be provided for stopping each fan in a ventilation system serving machinery spaces and for closing, in case of fire, each doorway, ventilator, and annular space around funnels and other openings into such spaces. Vessel was missing covers for their ventilation system feeding air into the engine room.

On 29 November 2021 the ITV LUDLOW experienced a starboard MDE failure while conducting fleeting operations IVO Ohio River mile 626.4 LDB at Mill Creek Power Plant. It was determined that the starboard MDE will need to be replaced. Vessel required to conduct repairs at Mill Creek Power Plant mile 626.4 Ohio River and contact the Coast Guard duty marine inspector at [phone withheld], when repairs are completed and before placing vessel back into service. In the interim until the ITV LUDLOW can be replaced by another fleet boat at Mill Creek Power Plant the ITV LUDLOW is authorized to conduct limited fleet work of holding hopper barge to the dock so loading of fly ash can continue.

Vessel had emergency tow procedures established but could not provide records of appropriate training. Emergency procedures related to the tow are developed and the appropriate training provided to the crew for carrying out their emergency duties. Implement training for emergency tow procedures and provide records to the same to the satisfaction of the attending Coast Guard Marine Inspector.

Installed fire pump does not dischareg the required 50 psi.

Newly installed fire detection system was not certified by a certified PE.

Vessel general alarm in engine room is missing the required placarding.

VESSEL WAS MISSING THE MASTHEAD AND STERN LIGHTS.

THE VESSEL DIDN'T HAVE IT'S OFFICIAL NUMBER PERMANENTLY MARKED ON IT'S HULLOR STRUCTURAL MEMBER.
