The equipment shall be capable of producing inert gas with an oxygen content no greater than 5% by volume, subject to the special requirments of Chapter 17.
PSCO observed vessel's Inert Gas Generator (IGG) in inoperable. Upon operation by the ship's crew the IGG would experience verious failures and immediately shutdown. The crew was unable to demonstrate operation for a period of 2 hours.
Condition: Improper/Lack of Maintenance

Doors required to be self-closing shall not be fitted with hold-back hooks. However, hold-back arrangements fixed with remote release devices of the fail-safe type may be utilized.
PSCO obersved B-15 door to Officer's mess, crew's mess, and A-deck conference room required to be self-closing per vessel's approved Fire Control Plan fitted with hold-back hooks. Additionally, PSCO observed A-0 self-closing fire door to the galley was unable to close via installed self-closing mechanism.
