All towing vessels must be operated in a manner that minimizes the risk of down-flooding and progressive flooding. The Marine Inspector (MI) observed there is a hole in the hull due to wastage around the through hull fitting located in the aft compartment on the starboard side. Repair or replace in kind prior to movement.

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. The connections of flexible cable plugs and socket outlets must be designed to prevent unintended separation. Inside the space below the operating station the MI observed electrical plugs and equipment not properly secured and equipment exposed to water. Secure plugs with UL approved devices; secured and protect equipment from water.

The MI observed several watertight and weathertight doors were not sealing properly and were missing breaker bar for the dogs. Also, the hatches above the engines were not secured or sealed. Confirm all doors and hatches are properly sealed/secured.

As far as practicable, the owner or managing operator must implement other types of safety control measures before relying on Personal Protective Equipment. These controls may include administrative, engineering, source modification, substitution, process change or controls, isolation, ventilation, or other controls. The Marine Inspector (MI) observed the oven does not have a latch installed to keep from opening and the stove does not have protective rails installed to keep pots from falling off. Also, the light bulb is not protected from shattering. Install to insure protection of crew.
