Each distribution panel or switchboard on a towing vessel must be: totally enclosed and of dead-front type. Marine Inspector observed that several switchboards were missing switch covers. Take appropriate action to cover all exposed parts on the switchboards and provide proof to the OCMI.

Vessel rep reported fuel discharge from starboard bow fuel tank. Marine Inspector shall observe fuel tank upon being emptied. Provide repair plan and make repairs to the satisfaction of the attending Marine Inspector.

To stop the flow of fuel in the event of a fire or break in the fuel line a remote fuel shut off must be...operable from a safe place outside the space where the valve is installed. Vessel has 4 fuel shut off pulls. upon testing 2 of the 4 wires broke before the valve was able to close completely. Repair and reconfigure all shut offs to close completely from outside the space.
