Electrical connections must be suitably installed to prevent them from coming loose through vibration or accidental contact. The connections of flexible cable plugs and socket outlets must be designed to prevent unintended separation. Both of the magnetic pick-up cables for each propulsion engine were suspended in the air and improperly secured. Splice and termination of both cables were not insulated or properly secured.

Lifejackets containers throughout the vessel were missing proper marking.
