The vessel shall be made available for a Coast Guard COI inspection within 90 days. If an inspection is not completed prior to this date, the action for this deficiency shall be changed to a 60 (Rectify deficiencies prior to movement) and the vessel may no long be permitted to operate.

Identified excessive amounts of oil throughout the engine room bilge, up to 4 inches, beneath the port & stbd engines & generators, fuel manifold, & the other portions of the engine room, posing an imminent hazard to the crew & environment, a serious substandard condition not being pro-actively managed. To stop the flow of fuel in line in the event of a fire or break in the fuel line, a remote fuel shutf-off valve must be fitted on any fuel line that supplies fuel directly to a propulsion engine or generator prime mover. Witnessed the fuel valve with multiple zip ties holding the valve open. 46CFR143.260(a) a)Remove zip ties and prove proper operation of fuel valve. b)Ensure 2 valves directly aft of the fuel tank have remote means of closure, within 14 days.

Equivalent protection may include lifelines, wire rope, chains, and bulwarks that provide strength and support equivalent to fixed rails. In areas where space limitations make deck rails impractical, such as at narrow catwalks in way of deckhouse sides, hand grabs may be substituted. No railing or hand grabs alongside the side deck superstructure, Bulkwarks were roughly 18-24 inches high, creating an unsafe condition for crew members to fall overboard.
