33CFR151.10 (b) When within 12 nautical miles of the nearest land, any discharge of oil or oily mixtures into the sea from a ship other than an oil tanker or from machinery space bilges of an oil tanker is prohibited except when all of the following conditions are satisfied—

Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the Officer in Charge, Marine Inspection. The operation station fire extinguisher was found to be empty due to pressure loss.

A vessel on a lakes, bays, and sounds, or rivers route must carry: three hand red flare distress signals and three hand orange smoke distress signals. Vessel was missing 01 orange smaoke and 02 red flares.

The discharge ends of fuel tank vent pipes must terminate on the hull exterior as high above the waterline as practicable and remote from any hull openings, or they must terminate in U-bends as high above the weather deck as practicable and as far as practicable from openings into any enclosed spaces. Vent pipes terminating on the hull exterior must be installed or equipped to prevent the accidental contamination of the fuel by water under normal operating conditions. The discharge ends of fuel tank vent pipes must be fitted with removable flame screens or flame arresters. The flame screens must consist of a single screen of corrosion resistant wire of at least 30 × 30 mesh. The flame screens or flame arresters must be of such size and design as to prevent reduction in the net cross sectional area of the vent pipe and permit cleaning or renewal of the flame screens or arrester elements. Vessels fuel tank vent hose was being vented in the engine space with no flame screen.

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. The main engine suction strainer needs to be mounted.

A submersible electric bilge pump may be used as a power bilge pump required by Table 182.520(a) only on a vessel of not more than 19.8 meters (65 feet) in length carrying not more than 49 passengers, other than a ferry, provided that the pump is permanently mounted. Vessels engine compartment bilge pump and alarm switch needss to be permanently mounted and cleaned for proper operation.

A fixed gas fire extinguishing system, which is capable of automatic discharge upon heat detection, may only be installed in a normally unoccupied space with a gross volume of not more than 170 cubic meters (6,000 cubic feet). Vessel has holes in aft engine room bulkhead that allow the passage of discharged agent in a volume greater than the current rating for the installed Pre-engineered fixed gas fire extinguishing system. 46CFR181.400(b)(1). The vessel will be restricted from carrying passengers after September 16, 2022 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Boundaries that separate machinery spaces from accommodation spaces, service spaces, and control spaces must be lined with noncombustible panels or insulation approved in accordance with § 164.009 in subchapter Q of this chapter, or other standard specified by the Commandant. Engine hatch cover that also serves as a passenger sitting area is not lined with noncombustible panels or insulation approved in accordance with § 164.009 in subchapter Q of this chapter, or other standard specified by the Commandant. 46CFR177.410(c)(4) The vessel will be restricted from carrying passengers after September 16, 2022 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

A hatch exposed to the weather must be watertight, except that the following hatches may be weathertight; a hatch in a cabin top. The hatch in the cabin top, forward of the Collison bulkhead is not weathertight. 46CFR179.360(a)(2) The vessel will be restricted from carrying passengers after September 16, 2022 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Each survival craft must be stowed so that when the vessel sinks the survival craft floats free. Vessels inflatable buoyant apparatus lashing was found impeading the survival craft ability to float free . 46CFR180.130(a)(2)

Vessels described by 46 CFR 175.110(c)(Ocean Routes) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. The vessel will be restricted from carrying passengers after December 27, 2022 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel's propeller has sheared off and is no longer on the propeller shaft.

Each vessel certificated to operate on an oceans route in warm water must either be provided with inflatable buoyant apparatus of an aggregate capacity that will accommodate at least 67% of the total number of persons permitted on board; or be provided with life floats of an aggregate capacity that will accommodate at least 100% of the total number of persons permitted on board. Vessel IBA does not have enough capacity to support 67% of the total persons aboard. Vessel is restricted to 29 total persons onboard. 27 passengers and 02 crew.

Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. Vessel's wasn't hard to read on the lifering in the pilot house.

Each battery used in an item of lifesaving equipment, except inflatable survival craft equipment, must be replaced during the annual inspection if the expiration date of the battery has passed. Battery was expired in the lifering.

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. Vessel's forward bilge alarm was not working.

Connections must be made to battery terminals with permanent type connectors. Spring clips or other temporary type clamps are prohibited. Vessel's batteries had wing nuts on them.

Vessel electrical system is significantly altered from original approved plans and does not meet the requirements of 46 CFR 176.700. Owner must submit to MSC plans as described in 46CFR177.202(b)(4)Electrical installation including: Elementary one-line diagram of the power system, Cable lists, Bills of materials, Type and size of generators and prime movers, Type and size of generator cables, bus-tie cables, feeders, and branch circuit cables, Power, lighting, and interior communication panelboards with number of circuits and rating of energy consuming devices, Type of capacity of storage batteries, Rating of circuit breakers and switches, interrupting capacity of circuit breakers, and rating and setting of overcurrent devices, and Electrical plant load analysis. The vessel will be restricted from carrying passengers after December 15, 2019 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector. 19DEC2019 update: Initial deficiency was issued 24MAY2019, The deficiency was extended to 15DEC19 at owners request and willingness to remove inverter, freshwater wash down pump, dockside battery and TV. The owner has submitted plans to MSC around the beginning of December 2019 but are not yet approved or implemented on the vessel. The vessel is restricted from carrying passengers until Electrical installation plans are approved and implemented on board the vessel to the satisfaction of an attending Coast Guard Marine Inspector. 25FEB2020 update: Electrical installation plans are must be approved from MSC and implemented on board the vessel to the satisfaction of an attending Coast Guard Marine Inspector. The vessel will be restricted from carrying passengers after 1 April 2020 if these updated conditions are not met.

Vessel has an positive DC cable in the house battery tray that is not adequately protect from abrasion and does not meet minimum standard of cable armor shall be secured to the box or fitting and openings though which conductors enter shall be closed as required by 46CFR183.10-20(f). Owner must repair ensure that it meets the provided regulation. The vessel will be restricted from carrying passengers after June 5, 2019 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Vessel collision bulkheads has non watertight penetrations and does not meet the minimum requirements as described in 46CFR179.310(a)(2) that each collision bulkhead Must not be fitted with any type of penetration or opening except penetrations may be made if they are located as high and as far inboard as practicable and they have a means to make them watertight. The vessel will be restricted from carrying passengers after June 5, 2019 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Vessels aft bulkhead of the engine room is not sealed with an approved material (Fire Stop) to ensure that if the fixed fire extinguishing system is activated, it will effectively extinguish the fire as designed and to prevent the release of the agent into any of the adjacent spaces. The vessel will be restricted from carrying passengers after June 5, 2019 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Vessels Fire bucket handle was wasted and did not meet the minimum requirements as described in 46CFR181.610. Vessel must have at least three 9.5 liter (2 1 /2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words ‘‘FIRE BUCKET'' must be stenciled in a contrasting color on each bucket. Provide new fire bucket.

Provide a means to manually activate the pre-engineered fixed gas system per 181.420(a)(2) to the satisfaction of CG MI prior to carrying passengers.

Ensure the pre-engineered fixed gas fire extinguishing system is installed and operable in accordance with manufacturer's instructions and in accordance with 46 CFR 181.420.

Vessels Portable watertight containers for distress flares smoke signals were not marked correctly and does not meet the minimum standard as described in 46CFR185.614 that containers shall be clearly marked in legible contrasting letters at least 12.7 millimeters (0.5 inches) high: ‘‘DISTRESS SIGNALS''. Appropriately mark Portable watertight containers.

Vessels wet exhaust support mounts are combustible and do not properly support exhaust pipe and does not meet minimum standard as described in 46CFR182.430(b). Owner must ensure exhaust pipe is supported by non-combustible hangers or blocks. The vessel will be restricted from carrying passengers after June 5, 2019 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Replace plastic bilge piping discharge through hulls with metallic. 46CFR 182.710. The vessel will be restricted from carrying passengers after June 5, 2019 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Vessel has one Life jacket light that was expired and does not meet the minimum standard as described in 46CFR180.75 that Each life jacket carried on a vessel on oceans, coastwise, or Great Lakes route, must have a life jacket light approved in accordance with § 161.012 in subchapter Q of this chapter. Provide new life jacket light.

Properly affix retro-reflective material on (06) life jackets in accordance with 46 CFR 185.604(h) and IMO Resolution A.658(16). Affix retro-reflective material.

Make permanent repairs and demonstrate proper operation of the bilge high level alarm in the machinery space in accordance with 46 CFR 182.530. The vessel will be restricted from carrying passengers after August 29, 2018 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Replace one (01) life jacket that has a broken clip in accordance with 46 CFR 180.71. The vessel will be restricted from carrying passengers after August 29, 2018 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Properly mark all life jacket storage containers/locations in accordance with 46 CFR 180.78 and 46 CFR 185.604(f) with the type life jacket (adult or child) and the quantity. The vessel will be restricted from carrying passengers after August 29, 2018 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Provide and install suitable brackets in way of all portable fire extinguishers in accordance with 46 CFR 181.500. The vessel will be restricted from carrying passengers after August 29, 2018 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Ensure the VHF radio meets DSC capabilities by interfacing the radio with the vessels installed GPS in accordance with 47 CFR 80.1069(a)(1). The vessels MMSI number and GPS position must be programed/interfaced with the VHF Radio. The vessel will be restricted from carrying passengers after August 29, 2018 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Make permanent repairs to the wasted hatch/cover located above the fish hold in the operating station in accordance with 46 CFR 177.100. The hatch/cover in its current condition poses a threat to crew and passenger safety. The vessel will be restricted from carrying passengers after September 21, 2018 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Evaluate the areas of delamination and Damage identified below Provide a repair proposal, listing the materials, procedures and layup intended for repairs that is acceptable to this office to the satisfaction of an attending Coast Guard Marine Inspectors. 46CFR177.300 1. Properly align the starboard side running light to an even horizontal position with the port side running light. Under the current configuration, the starboard side running light is approximately 3 feet lower on a horizontal plane.

The single side band radio was operational however the antennae was not connected due to the connection being corroded.

The bilge pump in the forward lower cabin compartment does not work in the automatic mode and the alarm does not work. The bilge pump in the forward engine room does not work in the automatic or manual mode. These pumps need to be operational.

The operating "brain" for the system is bad and needs to be renewed. System will not shutdown engine when tested.

The mounting bracket for one of the B-1 extinguishers in the Pilot House is broken and needs replaced.

The life float has deteriorated to the point that the grasping lines are breaking out of the holding devices fabricated into the life float. This life float is no longer serviceable and will need to be replaced.

There were eleven (11) life jackets that the lights (chem. lights) were out of date and need replacement. The passenger count is reduced to nine (9) plus two (2) crew until the lights can be replaced.

Vessel was using center fuel tank. Noted on vessel original stability letter, NOT to use center fuel tank, only Port & Stbd wing tanks. Remove all fuel out of center tank, prior to carrying passengers.

Vessel has an old 2 inch hose laying in bilge, connected to overboard discharge fitting, out of use. Plug or remove.

Install tethering device on aft hatch.

Connect two dogs on aft hatch for proper closing.

Remove PVC caps and install proper metal tank caps.

Remove and renew all fuel hoses with approved hose.

Install cowling around fuel oil cut off valves to prevent hands getting burned during fire.

Renew all bilge pump hoses to proper marine grade approved hose.

Stencil name of vessel on apparatus.

Install two means of natural ventilation to the machinery space and install closing devices on each.

EPIRB hydrostatic release is out of date. Renew.

Repair cracked welds in aft awning piping.

Renew line on water lights.

All bridge window sealant material is loose and missing. Have windows professionally sealed.

Remove all dead-end wires and or end in approved electrical box.

Forward winch control wiring is exposed to passengers, place in an approved electrical box.

Remove aluminum access plate installed on deck over e/r battery boxes and install material of same kind as deck. Make insert flush with deck to avoid trip hazard.

Center fuel tank did not have an emergency shutoff valve.

Battery box was not iaw 46 CFR 183.350(b) & (e)

Vessel has a thru hull ball valve in the fwd compartment bilge that is no longer being utilized and is not plugged.
