33CFR151.10 (b) When within 12 nautical miles of the nearest land, any discharge of oil or oily mixtures into the sea from a ship other than an oil tanker or from machinery space bilges of an oil tanker is prohibited except when all of the following conditions are satisfied—

Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the Officer in Charge, Marine Inspection. The operation station fire extinguisher was found to be empty due to pressure loss.

The discharge ends of fuel tank vent pipes must be fitted with removable flame screens or flame arresters. The flame screens must consist of a single screen of corrosion resistant wire of at least 30 × 30 mesh. The flame screens or flame arresters must be of such size and design as to prevent reduction in the net cross sectional area of the vent pipe and permit cleaning or renewal of the flame screens or arrester elements. Vessels fuel tank vent hose was being vented in the engine space with no flame screen.

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. The main engine suction strainer needs to be mounted.

Boundaries that separate machinery spaces from accommodation spaces, service spaces, and control spaces must be lined with noncombustible panels or insulation approved in accordance with § 164.009 in subchapter Q of this chapter, or other standard specified by the Commandant. A hatch exposed to the weather must be watertight, except that the following hatches may be weathertight; a hatch in a cabin top. The hatch in the cabin top, forward of the Collison bulkhead is not weathertight. 46CFR179.360(a)(2) The vessel will be restricted from carrying passengers after September 16, 2022 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Each survival craft must be stowed so that when the vessel sinks the survival craft floats free. Vessels inflatable buoyant apparatus lashing was found impeading the survival craft ability to float free . 46CFR180.130(a)(2)

Vessels described by 46 CFR 175.110(c)(Ocean Routes) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. The vessel will be restricted from carrying passengers after December 27, 2022 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. Vessel's wasn't hard to read on the lifering in the pilot house.

Each battery used in an item of lifesaving equipment, except inflatable survival craft equipment, must be replaced during the annual inspection if the expiration date of the battery has passed. Battery was expired in the lifering.

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. Vessel's forward bilge alarm was not working.

Connections must be made to battery terminals with permanent type connectors. Spring clips or other temporary type clamps are prohibited. Vessel's batteries had wing nuts on them.

Vessel electrical system is significantly altered from original approved plans and does not meet the requirements of 46 CFR 176.700. The vessel will be restricted from carrying passengers after December 15, 2019 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector. 19DEC2019 update: Initial deficiency was issued 24MAY2019, The deficiency was extended to 15DEC19 at owners request and willingness to remove inverter, freshwater wash down pump, dockside battery and TV. The owner has submitted plans to MSC around the beginning of December 2019 but are not yet approved or implemented on the vessel. The vessel is restricted from carrying passengers until Electrical installation plans are approved and implemented on board the vessel to the satisfaction of an attending Coast Guard Marine Inspector. 25FEB2020 update: Electrical installation plans are must be approved from MSC and implemented on board the vessel to the satisfaction of an attending Coast Guard Marine Inspector. The vessel will be restricted from carrying passengers after 1 April 2020 if these updated conditions are not met.

Vessel collision bulkheads has non watertight penetrations and does not meet the minimum requirements as described in 46CFR179.310(a)(2) that each collision bulkhead Must not be fitted with any type of penetration or opening except penetrations may be made if they are located as high and as far inboard as practicable and they have a means to make them watertight. The vessel will be restricted from carrying passengers after June 5, 2019 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Vessels aft bulkhead of the engine room is not sealed with an approved material (Fire Stop) to ensure that if the fixed fire extinguishing system is activated, it will effectively extinguish the fire as designed and to prevent the release of the agent into any of the adjacent spaces. The vessel will be restricted from carrying passengers after June 5, 2019 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Vessels Fire bucket handle was wasted and did not meet the minimum requirements as described in 46CFR181.610. Vessel must have at least three 9.5 liter (2 1 /2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words ‘‘FIRE BUCKET'' must be stenciled in a contrasting color on each bucket. Provide new fire bucket.

Provide a means to manually activate the pre-engineered fixed gas system per 181.420(a)(2) to the satisfaction of CG MI prior to carrying passengers.

Vessels Portable watertight containers for distress flares smoke signals were not marked correctly and does not meet the minimum standard as described in 46CFR185.614 that containers shall be clearly marked in legible contrasting letters at least 12.7 millimeters (0.5 inches) high: ‘‘DISTRESS SIGNALS''. Vessels wet exhaust support mounts are combustible and do not properly support exhaust pipe and does not meet minimum standard as described in 46CFR182.430(b). Vessel has one Life jacket light that was expired and does not meet the minimum standard as described in 46CFR180.75 that Each life jacket carried on a vessel on oceans, coastwise, or Great Lakes route, must have a life jacket light approved in accordance with § 161.012 in subchapter Q of this chapter. Provide new life jacket light.

Properly affix retro-reflective material on (06) life jackets in accordance with 46 CFR 185.604(h) and IMO Resolution A.658(16). Affix retro-reflective material.

Make permanent repairs and demonstrate proper operation of the bilge high level alarm in the machinery space in accordance with 46 CFR 182.530. Ensure the VHF radio meets DSC capabilities by interfacing the radio with the vessels installed GPS in accordance with 47 CFR 80.1069(a)(1). The vessels MMSI number and GPS position must be programed/interfaced with the VHF Radio.
