Portable and semi-portable fire extinguishers must be inspected, maintained, and tested in accordance with the inspection, maintenance procedures, and hydrostatic pressure tests required by Chapters 7 and 8 of NFPA 10. Inspector observed a fire extringuisher on the second deck of the engine room missing the nozzle horn (Vessel has more than required complement).

Company's TSMS states that there must be signage to restrict access from aft deck while underway. Inspector observed no signage, rails, guards, or equivalent.

Vessel is required to have a steering system that is designed/constructed to operate a vessel. From 07-10 March 2022, the steering system has failed twice. Even after repairs were made and cleared through the internal TSMS Option and approved by the TPO. However, after the second steering failure and repairs made, the system was identified to still not be fully operational by the Coast Guard. The vessel was permitted to return to Blakeley Boat Works and undergo further repairs. Prior to operation, the company must complete a root cause analysis to determine these failures, provide a repair proposal, and complete an external survey with the Coast Guard in attendance.
