During routine thermographic inspection conducted Sept 2021, LA25 Cargo Fan had a 20 degree C rise and was indicated to be repaired as soon as possible. Vessel management had not yet repaired equipment.

Vessel’s open lifeboats showing signs of deterioration The engine cover bolts are missing. There are some damaged areas of fiberglass above the waterline and the step to get into lifeboats are rotted at the bottom. The retroactive tape is missing and/or is an unapproved type. Vessel to repair to the satisfaction of the attending Marine Inspector.

All exposed metal parts of electrical machines or equipment which are not intended to be “live”, but are liable to become “live” under fault conditions, shall be earthed (grounded); and all electrical apparatus shall be so constructed and so installed that danger of injury in ordinary handling shall not exist. Electrical wiring in bridge deck battery room and “C” deck #94 electrical storage locker had improperly cut wiring leaving ends exposed thus causing a potential hazardous condition. Vessel shall terminate wires properly.

Working areas throughout the engine room were unable to hear the audible general alarm when no visual alarm indicators were present and various audible bells were not louder than the background noise created by the generators.
