07113 - A cargo ship shall be provided with fire pumps, water service pipes, hydrant and hoses complying with Regulation 56: shall be sufficient for the effective distribution of the maximum required discharge. The ship’s fire main piping on the weather deck was severely wasted showing signs of large size holes. During the operational test of the system significant leaks were discovered which rendered the system unable to properly maintain water supply to the system in order to fight a fire. The water was leaking at approximately 100-150 gallons per minute. 60 SOLAS Ch. II/Reg. 56(c)

Fire extinguishing appliances shall be kept in good order and available for immediate use. The fire hydrant on the second deck (Aft) had a barbecue grill tied off to fire hose connection blocking access, preventing immediate use. 60 SOLAS CH. II/Reg.66

Objective evidence discovered during an expanded ISM exam revealed the following non-conformities: the vessel failed to fully implement the requirements of the ISM Code through their SMS procedures as evident by the following deficiencies indicating that the ship and/or company are not meeting the SMS requirements. Recommend an external audit. 74 SOLAS (2014) IX/3.1, 6.2 ISM Code Part A The company is responsible for ensuring that adequate resources and shore-based support are provided to enable persons to carry out their functions. During a conversation with the current Captain and the previous Captain, it was determined the previous Captain of the vessel made mention to the company that the overall condition of the fire main had been in need of immediate repair for several months. The previous Captain additionally informed the PSCO that the DPA was verbally notified several times dating back to October 2018. ISM Code 10.1, ISM Code 10.2.3 The safety management system should include procedures ensuring that nonconformities and hazardous situations are reported to the company. Although the vessel Captain made verbal notification of the condition of the fire main they did not follow SMS procedures by properly filling out and filing the “Advice of Defect and Repair” form. ISM Code 9.1

Ships constructed before 01july 1998 shall meet the requirements of 1.1 & 1.2:The view of the sea surface from the conning position shall not be obscured: No blind sectors caused by cargo, cargo gear. Vehicles loaded on top of containers are obstructing the required view of the sea. 74 SOLAS (14 Cons.) Ch.V/Reg. 1.2

All Cargoes shall be loaded, stowed and secured throughout the voyage in accordance with the Cargo Securing Manual. Vehicles loaded on vessel are stowed on the roofs of 20' containers that are structurally failing due to the weight of the vehicles. The failure of the roof will cause the vehicles to become unsecured during the voyage. 74 SOLAS (14 Cons.) Ch. VI/Reg. 5.6

PSCO observed pressure relief valves gagged open with hose clamps. In addition, main air receiver without proper safety relief and corroded. 60SOLAS CH 1/11 17bc

All ships 150 gross tonsand above shall be fitted with a BNWAS. Vessel does not have BNWAS as indicated on Equipment FORM E. 74SOLAS(14Cons.) CH V/Reg 19.2.2 17bc

Hoses shall together with any necessary fittings and tools be kept ready for use in conspicuous positions near the water service hydrants or connections. Missing nozzles in hydrant station starboard side engine room. 60SOLAS CH 2/56(f)

A temporary repair was made for a hole found in the ship's hull that was allowing the ingress of sea water in Ballast Tank #1. The vessel must make permanent reparis prior to the next U.S. port after sailing foreign.

1240. The means for securing and maintaining weather tightness shall be to the satisfaction of the Administration. The arrangements shall ensure that the tightness can be maintained in any sea conditions, and for this purpose tests shall be required at the initial survey and may be required at periodical surveys and at annual inspections or at more frequent intervals. The accordion cargo hatch cover is heavily wasted and has at least 5 holes throughout. Several closing devices used to secure the hatch in the closed position are missing or wasted. 0299. After any survey of the hull has been completed, no change shall be made in the structural arrangements covered by the survey, without the sanction of the Administration. The vessel’s last survey was in November 2017 with no areas of concern identified. However, PSCOs found wastage and holes along the steel beams supporting the landings in the ladder wells into the cargo hold. 60 SOLAS 1/11. 30bc

0820. Mechanical and equipment presenting hazards to personnel shall have installations of built-in guards for moving parts. The built in guard on the anchor windlass is wasted exposing the gears and presents a hazard to the crew. ILO 147 ART.4 & PWSA 33 USC 1233(B) 17bc

1299. The Administration shall satisfy itself that the general structural strength of the hull is sufficient for the draught corresponding to the freeboard assigned. Significant wastage and penetrations were discovered in the vessel’s starboard side ballast tanks. The forepeak tank was found to be leaking into ballast tank #1 STBD side. In addition, when ballast tank #1 was drained, a hole approximately 2 inches by 3 inches in the ship's hull allowed for the passage of sea water into the tank. The last survey in November 2017 found no areas of concern in the affected areas. ICLL 66 ANNEX I CH1/REG.1 30bc

Machinery Space openings shall be properly framed and efficiently enlcosed by steel casing. Access openings on such casings shall be fitted with doors complying with reg 12(1). Other openings in such casings shall shall be fitted with equivalent covers, permanently attached in their proper position. Bulkhead leading into engine room has a 10x10 inch square with a temporary cover forward port side.

Fire protection systems and appliances shall be maintained ready for use and shall be properly inspected. 01 out of 02 fire suits onboard was found with tears on leg. 74 SOLAS (14) II-2/Reg 14.1.1-14.1.2

2525 - Masters resp - ISM related def - The company should ensure that master is fully conversant with company's safety management system. Master is not familiar with the familiarization for the procedures to report hazardous occurrences. ISM Code 6.1.2.

2545 - Reports of non-conformities - ISM related def - The company should ensure that any non-conformity is reported. The vessel had a fuel leak from piping that leads to the starboard #1 tanks. The fuel leaked into ballast tank #4. No non-conformities were made to the company. ISM Code 12.1

0940 - Ballast fuel and other tanks- The condition of the ship and its equipment shall be maintainted to ensure that the ship will remain fit to proceed to sea without danger to the ship or persons onboard. The #4 center ballast tank was found to contain fuel oil. 74 SOLAS (14 cons) Ch 1/Reg 11a

1710 -Oil record book- Every ship of 400 GT and above shall be provided with an oil record book. The ORB shall be in the form specified in Appendix III to the Annex. ORB did not identify the quantity and tanks for fuel oil transfer during bunkering. MARPOL (11 cons) Annex I/Reg 17

2199 -Other (MARPOL operational def)- The bunker delivery note shall be kept on board the ship in such place as to be readily available for inspection at all reasonable times and retained for three years. Chief Engineer could not produce any bunkering receipts. MARPOL (11 cons) Annex VI/Reg 18

0740 Pumps - On a cargo ship where a fire in any one comprtment could put all the pumps out of action, there must be an alternative means of providing water for fire fighting. Alternative means for providing water for fire fighting not provided. Fire pump inoperable. SOLAS 60 Reg. 65(b)(ii) 17bc
