All exposed metal parts of electrical machines or equipment which are not intended to be “live” but are liable to become “live” under fault conditions... shall be so constructed and so installed that danger of injury in ordinary handling shall not exist. One electrical outlet next to the Masters office desk was completely pulled off the wall with exposed wires. Cables that were previously used for water pumps on the lower level of the engine room were still present (2 pumps total) and could cause shock if “live” and touched inadvertently. Those cables are dead ended and wrapped in only electrical tape. The attached cable to one of these pumps is heavily worn such that the protective sheathing was broken. Other small junction boxes were loose allowing unprotected parts of cabling to be pulled out. 74SOLAS II-1/27(a)(I)(1) 16bc

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Several fire doors around the ship were in disrepair. The latching bolts were broken, missing or sealed with epoxy when all 3 bolts in each door intended to be operational. The hardware on several doors were broken. Some doors were difficult to close when provided closing arrangements. One did not latch at all (engine control room door) and could be pushed open. The fire doors noted include: the fire door going down the stairs to the engine control room (ECR), the fire door to the shop at the bottom of the stairs, the emergency escape door from ECR, and the fire door between ECR and the engine room. 74 SOLAS(14) I/11 16bc

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company (10.1). And meeting these requirements the company should ensure that (10.2); inspections are held at appropriate intervals (10.2.1); any non-conformity is reported with its possible cause if known (10.2.2); appropriate corrective action is taken (10.2.3); in records of these activities are maintained (10.2.3). The company should identify equipment and technical systems, the sudden operational failure of which may result in hazardous situations. The safety management system should provide for specific measures aimed at promoting the reliability of such equipment or systems. These measures should include the regular testing of standby arrangements and equipment or technical systems that are not in continuous use (10.3) According to ships procedures M1 - M13, the lifeboats and all associated equipment are visually inspected weekly (10.2.2). As required by ships procedures in accordance with requisition number 151/19, the crew submitted a request for new medication on 28 August 2019; however, the order has not been completed (10.2.3). The ships safety management systems do not include procedures for training on installing the lifeboats canopy. Additionally, procedures are not in place for regular inspections on class a fire doors found in the engine room (10.3) Deficiencies one, two, three, and observations above are objective evidence that the vessel failed to fully implement the requirements of the SMS. An internal audit is required within three months with a focus on crew inspection procedures of the safe life-saving and fire fighting and requisitions. 74 SOLAS 14 IX/ 3.1 ISM 10.1, 10,2, 10.3 705c (3 months)
