11134 - Life Saving Appliances - Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO observed 03 ring lifebuoy lights were inop. PSCO expanded inspection of ring lifebuoy lights and discovered the lights had incorrect batteries installed. - 74 SOLAS (20) III/20.2 17ac

07120 - Fire Safety - Such doors shall be arranged so that positive closure is assured by self-closing. PSCO observed self-closing door for emergency escape truck in the engine room would not properly close and latch, allowing for the passage of smoke and flame. - 74 SOLAS II/34(b)(iii) 17ac

03199 - Water/Weathertight Conditions - The ring, lines and letters shall be painted in white or yellow on a dark background or in black on a light ground. PSCO observed portside deckline was not painted contrast in color. - ICLL 1966 Annex I/8 40ac

The condition of the ship and it's equipment be maintained to conform with the provisions of the present regulations to ensure the ship in all respect will remain fit to proceed to sea without danger to the ship and persons on board. Fire doors in engine room are missing latch locking mechanisms. Ship is awaiting parts. Fire doors include; fire door going down the stairs to engine control room. Fire door to shop across from ECR, emergency escape door from ECR and fire door between ECR and engine room. Invoice # 58429, estimated arrival- week 14 of 2020. 74 SOLAS (14) I/II 40bc

The periodical surverys reffered to in paragraphs (a)(iii) shall be endorsed on the cargo ship radio certifcate. Certficate was issued 07JUN2019 by Flag, already had surveyors signature on the blocks on back page not allowing blocks to be properly signed and dated when surveys take place.

An annual survey within three months before or after each anniversary date of the certicate including a general inspection of the structure, equipment sysemts, fitting arrangement and material. IOPP issued on 23JAN2020 by Flag, already had surveyors signature on the blocks on back page not allowing blocks to be properly signed and dated when surveys take place.

One approved cover of a highly visible color capable of protecting the occupants against injury by exposure. There were 2 covers of which were in poor condition for the lifeboats. The covers had gaping holes and duct tape that served as temporary repairs. 74SOLAS III/11.9.xxvi 17bc

All exposed metal parts of electrical machines or equipment which are not intended to be “live” but are liable to become “live” under fault conditions... shall be so constructed and so installed that danger of injury in ordinary handling shall not exist. One electrical outlet next to the Masters office desk was completely pulled off the wall with exposed wires. Cables that were previously used for water pumps on the lower level of the engine room were still present (2 pumps total) and could cause shock if “live” and touched inadvertently. Those cables are dead ended and wrapped in only electrical tape. The attached cable to one of these pumps is heavily worn such that the protective sheathing was broken. Other small junction boxes were loose allowing unprotected parts of cabling to be pulled out. 74SOLAS II-1/27(a)(I)(1) 16bc

Several fire doors around the ship were in disrepair. The latching bolts were broken, missing or sealed with epoxy when all 3 bolts in each door intended to be operational. The hardware on several doors were broken. Some doors were difficult to close when provided closing arrangements. One did not latch at all (engine control room door) and could be pushed open. The fire doors noted include: the fire door going down the stairs to the engine control room (ECR), the fire door to the shop at the bottom of the stairs, the emergency escape door from ECR, and the fire door between ECR and the engine room. 74 SOLAS(14) I/11 16bc

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company (10.1). And meeting these requirements the company should ensure that (10.2); inspections are held at appropriate intervals (10.2.1); any non-conformity is reported with its possible cause if known (10.2.2); appropriate corrective action is taken (10.2.3); in records of these activities are maintained (10.2.3). The company should identify equipment and technical systems, the sudden operational failure of which may result in hazardous situations. The safety management system should provide for specific measures aimed at promoting the reliability of such equipment or systems. These measures should include the regular testing of standby arrangements and equipment or technical systems that are not in continuous use (10.3) According to ships procedures M1 - M13, the lifeboats and all associated equipment are visually inspected weekly (10.2.2). As required by ships procedures in accordance with requisition number 151/19, the crew submitted a request for new medication on 28 August 2019; however, the order has not been completed (10.2.3). The ships safety management systems do not include procedures for training on installing the lifeboats canopy. Additionally, procedures are not in place for regular inspections on class a fire doors found in the engine room (10.3) Deficiencies one, two, three, and observations above are objective evidence that the vessel failed to fully implement the requirements of the SMS. An internal audit is required within three months with a focus on crew inspection procedures of the safe life-saving and fire fighting and requisitions. 74 SOLAS 14 IX/ 3.1 ISM 10.1, 10,2, 10.3 705c (3 months)

04114 - In cargo ships 5000 tons gross tonnage and upwards, there shall be a self contained emergency source of power. PSCO observed crew unable to start emergency generator after multiple attemps. - 74 SOLAS II-1/26(a)(1) 17ac

15105 - A vessel equipped with ballast water tanks that operates in the waters of the U.S. must maintain a ballast water management plan that has been developed specificaly for the vessel and that will allow those responsible for the plans implementation to understand follow the vessel's ballast water management strategy and comply with the requirements of this subpart. Vessel's ballast water management plan does not include all of the necessary provisions listed in 33 CFR 151 Subpart D. 33 CFR 151.2050(g) 50c

15105 - The master, owner, operator, agent or PIC of a vessel must ensure the maintenance of records. PSCO observed that the ballast water management record book was missing signature attesting to the accuracy of the information provided. 33 CFR 151.2070(a)(2) 50c

15105 - Vessels operating within 03 miles nautical miles of the U.S. must conduct routine visual inspections, once per week or voyage and document in official logbook. Inspections are not being conducted nor recorded as required under section 4.1.1.1 in 2013 VGP. 33 USC 1251 40c

0810-Every ship shall carry appropriate portable atmoshpere testing instrument and suitable means shall be provided for calibration of such instruments. PSCO observed portable gas meters on board were not calibrated for 272 days and no calibration equipment was onboard. 74SOLAS(14)XI-2/7. 40c

PSCO observed flexible ducting missing between emergency generator cooling radiator and outside vent. 74SOLAS(14)I/II(a). 50a

1710 - Oil Record Book - Each operation of this regualtion shall be full recorded with out delay in the ORB Part I, so that all entries in the book appropiate to that operation are completed. - ORB displays entries exceeding maximum input of OWS system according to IOPP Certificate. 1710 - Oil Record Book - The company should ensure that all personnel involved in the company's safety management system have an adequate understanding of relevant rules, regulations, codes and guidelines. - Provided with the dificiencies listed on PSC Form B dated 20OCT17, the vessel is not following procedures concerning proper documentation in the Oil Record Book. 1540 - All ships 500gt and upwards must have a gyro-compass to determine and display their heading. Proper heading between bridge and steering can not be confirmed. SOLAS 14 V reg. 19.2.5 - 17abc

1581 - The exact position of the rudder if power operated, shall be indicated at the principal steering station. The vessel has different rudder indication at the bridge station and steering controls. - SOLAS 74 Ch. II-1 Reg. 29(a)(iv) - 17abc

1999-MARPOL-Vessels operating in emissions control areas shall carry a written procedure showing how the fuel oil changeover is to be done. The vessel does not have changeover procedures onboard.- MARPOL Annex VI/reg. 14.6

0936-Safety in General- Indicators for running indication of the motors of electric and electrohydraulic steering gear shall be installed.The power failure alarm for the steering gear system does not work.-74 SOLAS CH.II-I/30

No tidal current tables or equivalent were found by Coast Guard Bboarding Officer.

NOAA CHARTS OR THE FOREIGN EQUIVALENT FOR THE LOCAL AREA WERE FOUND BY THE COAST GUARD BOARDING OFFICER TO BE NOT CORRECTED UP TO DATE.

The coast pilot for the local area was found by the coast guard boarding officer to be not corrected up to date.

The Coast Guard light list was fond by the Coast Guard Boarding Officer to be out of date and not corrected.

No current tide tables were found o/b by the Coast Guard boarding officer.

Upon inspection Coast Guard Boarding Officer found no block diagram emergency change-over on bridge.

THE VESSEL HAD NO OIL TRANSFER PROCEDURES THAT WERE MADE AVAILABLE FOR INSPECTION, DURRING THE EXAM BY THE COAST GUARD BOARDING OFFICER.

A VIOLATION OF 33 CFR 164.30 IN THAT THE VSL DID NOT HAVE ON BOARD CHARTS 12278, 12281, WHICH WERE REQUIRED FOR THE VSLS ROUTE FROM SMITH PT VA TO BALTIMORE HARBOR.
