At initial and subsequent inspection for certification, the marine inspector shall view the vessel afloat and shall conduct the following tests and inspections of the hull structure and its appendages: (1) examine all accessible parts of watertight bulkheads. Marine inspector (MI) observed a 2 inch cutout amidships over the keel beam in a watertight bulkhead. Affect temporary repairs until scheduled dry dock. 46 CFR 176.25-5(a)(1)

Provisions shall be made by means of plugs, covers, etc., to prevent the admission of air into the space protected. MI observed three openings in the watertight bulkheads in the engine room. 46 CFR 181.20-35

All electrical installations shall be to the satisfaction of the officer in charge, marine inspection. MI observed a bonding wire on the steering gear was disconnected. 46 CFR 183.01-15(a)

Each vessel must carry a certificate with a valid endorsement while subject to the Bridge-to-Bridge Act. MI observed the vessel had an expired bridge-to-bridge certificate. 47 CFR 80.1005

A certification that the ship has passed an inspection must be entered into the ship's log by the inspecting technician. The technician conducting the inspection and providing the certification must not be the vessel's owner, operator, master, or employee or their affiliates. MI observed the radiotelephony certificate was expired. 47 CFR 80.59 (a)(2)

Fuel shutoff valves, installed so as to close against the fuel flow, shall be fitted in the fuel supply lines, one at the tank connection and one at the engine. The shutoff valve at the tank shall be operable from outside the tank compartment. STBD fuel tank remote shutoff valve handle found wasted and nearly broken off; replace valve handle so that valve is operational from outside the space. OLD T- 182.20-40(b)(3)

Fuel filter and strainer bowls must be highly resistant to shattering due to mechanical impact and resistant to failure due to thermal shock. Fuel filter for generator found missing flame shield bowl.

Valves for removing water or impurities from diesel fuel in water traps or strainers are permitted. These valves must be provided with caps or plugs to prevent fuel leakage. Valve for generator fuel filter was found to be of the plastic type and not suitable metallic type.

All cable and wire must be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. Multiple wire runs in E/R found supported with plastic supporting devices; replace with metal supports.

The number of penetrations in a watertight bulkhead (WTB) must be minimized. A penetration in a watertight bulkhead must be as high and as far inboard in the bulkhead as practicable, and made watertight. WTB between tank space and galley space found with penetrations for wire runs that were not watertight.

Vessels over 65 feet shall be provided with 2 fixed powered bilge pumps that are self priming, each driven by different sources of power, and must be permanently connected to the bilge main. Vessels is documented as 73ft and fitted with 02 electric pumps (1 bilge/1 fire)that are connected to the bilge main but they are driven off the same source of power (generator). Provide a written plan to the Officer in Charge, Marine Inspection (OCMI) to install a system that would be compliant with the regulations for bilge systems in passengers vessels. This plan must be approved by the OCMI prior to any installation or modification of the vessel. OLD T - 46 CFR 182.25-10(c)

Each life jacket must be marked with Type I retro reflective material approved in 46 CFR 164.018. MI identified 40 adult and 07 child life jackets had worn out retro reflective tape.

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessel's stability letter, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 1 inch numbers and letters at the entranceway to that deck. Vessel had missing placard for maximum passenger capacity on the upper deck.

Existing vessels with a hull of fiberglass or wood shall have a galley hood that meets UL 710 and have a dry or wet chemical extinguishing system that meets NFPA 17 of NFPA 17A. 705c prior to use of griddle.

Provide portable extinguisher for galley that meets 40-B:C UL rating. 705c prior to use of the griddle

Replace 13 Adult and 6 Child lifejackets with appropriate type I jackets.

Replace Retro-reflective tape on 10 identified life jackets.

Vertical ladder leading to engine room was loose and allowed for the climber to teeter and possible fall backwards. Properly secure ladder for safety of access to E/R.

Provide 2 additional orange smoke and 1 red hand flares.

Provide 3 adult lifejackets. 3 of the adult lifejacket are not fit for service. The number of passengers is reduced by 3 until this item is corrected.

Reseal keel cap.

Repair starboard rudder.

Replace 12 @ 4" strut bolts in lazarette.

Clean excessive oil from electrical generator windings.

Log passenger count during embarkation and disembarkation.

Provide EAP training to supervisor and crew.

Provide High Level Alarm light for Fuel Room at operating station.

Provide appropriate charts for area of operation/route.

Remove and destroy 20 adult and 2 child lifejackets which were unservicable.
